About this file
This is a 1995 Dam Safety Inspection Report related to the Hancock Creek Dam located in Michigan. The document serves as a supporting file for the state and local contract opportunity 751/26442.JBB, which seeks a comprehensive dam evaluation. The inspection report provides historical assessment data and technical findings regarding the structural condition and safety status of the dam facility as of 1995, establishing a baseline for current evaluation efforts and identifying any deficiencies or areas of concern that may require remediation or further investigation.
The report functions as a key reference document for potential bidders responding to the dam evaluation solicitation, providing critical historical context and prior inspection findings that inform the scope and requirements of the current evaluation project. This baseline data assists in determining what aspects of the dam require updated assessment, whether structural, hydrological, seismic, or operational evaluations are necessary, and what recommendations from the 1995 inspection may need to be revisited or validated through the new evaluation process.

INTRODUCTION
The purpose of this inspection report is to evaluate the structural condition and hydraulic capacity of this dam as required by Part 315 Dam Safety, Natural Resources and Environmental Protection Act, 1994 PA 451, as amended;
MCL 315.21 to 315.29. The report is limited to a visual investigation and review of any previous inspection reports, plans, and data which are available. This reporl should not be considered an in-depth engineering investigation.
The Hancock Creek Dam was inspected by the Dam Safety Unit, Land and Water Management Division, pursuant to the requirements of Director's Letter No. 65.
All reference to "right" and "left" in this report are based on the observer facing downstream.

CONCLUSIONS AND RECOMMENDATIONS
There were no deficiencies observed during the inspection that would lead to failure of the dam. The dam has adequate spillway capacity under existing conditions.
The following recommended actions should be completed as soon as practical:
l) Add 1 foot of compacted fill across entire embankment, seed, and mulch to promote good grass growth.
2) Remove log from spillway.
PROJECT INFORMATION
The Hancock Creek Dam was constructed in 1951 to control a wildlife flooding created by a beaver dam. Reduced copies of construction plans are on file with the Dam Safely Unit.
The Hancock Creek Dam has a 338 ft. long earth fill embankment with a structural height of 12 feet. The dam creates a head of about 7 feet and an impoundment with a surface area of approximately 70 acres.
The principal spillway consists of a concrete drop box inlet with a 7 ft. wide stoplog section. The concrete inlet structure is founded on 8 gauge steel sheet piling. The drop inlet is connected to a 40 ft. long 60 inch diameter corrugated metal pipe (CMP) outlet pipe. A wooden plank walkway and a pipe handrail provides access to the stoplogs.
The embankment consists of unspecified earth fill with an upstream slope of 3 horizontal to 1 vertical, downstream slope of 2.5 horizontal to 1 vertical, and a top width of 10 feet. The embankment foundation consists of rock overlain by sands and gravels up a 5 feet thick, and covered with topsoil or muck varying up to 2 feet thick. Drawing notes indicate that unsuitable foundation material was to be excavated prior to placing embankment fill.
Previous inspection reports were completed in 1978, 1979, 1988, and 1991.
Previous recommendations included adding 1 foot of fill on top of the embankment. This recommendation does not appear to have been implemented.
SITE INVESTIGATION
The following discussion of the physical condition of the dam and appurtenances is based on observations and photographs obtained on the date of the inspection.

The embankment is grass covered and in good condition. There were no other indications of deficiencies in the embankment.
The concrete of the drop inlet structure is cracked and displaced in the headwall just to the left of the outlet pipe. The crack does not appear to be worse than noted in previous inspections. Another crack in the headwall is located just to the right of the outlet pipe. The headwall is also cracked directly above the crown of the pipe. The cracks do not appear to be cause for concern at this time, but should be monitored in subsequent inspections.
The outlet pipe is in good condition. No leaking or deflected joints were observed.
STRUCTURAL STABILITY
Based on the conditions observed during the field inspection, the dam appears to be stable. No further structural stability analysis is warranted at this time.
SPILLWAY CAPACITY
The contributing drainage area to the dam is approximately 6.4 square miles.
The design discharge for this dam is the 1 % chance flood discharge of 360 cubic feet per second (cfs). The existing spillway capacity is 200 cfs, with 1 foot of freeboard.
Previous analyses has determined that the design flood event could be attenuated in the impoundment, with a maximum discharge of 210 cfs, with approximately 0.6 ft. of freeboard. It is recommended that 1.0 foot of fill be added to the top of the embankment to provide additional freeboard.
OPERATION AND MAINTENANCE
Operation of the dam is the responsibility of the District 2 Wildlife Division. A written Operation and Maintenance (O&M) Plan is on file at the District Office.
EMERGENCY ACTION PLAN
No Emergency Action Plan (EAP) exists for this dam. Since the dam has a low hazard potential rating, an EAP is not required.
