About this file

This document is a certification form related to Equal Employment Opportunity (EEO) reporting requirements, issued in April 1969 and applicable to federal contractors and subcontractors. The form requires bidders and proposed subcontractors to certify their compliance with Executive Orders 10925, 11114, and 11246 regarding equal opportunity reporting, specifically noting whether they have previously participated in contracts subject to these orders and filed the necessary reports with relevant government agencies.

The document outlines specific reporting requirements, noting that Standard Form 100 (EEO-1) is currently the primary report required by Executive Orders. It emphasizes that contractors who have not filed required reports may be prevented from being awarded contracts, with 41 CFR 60-1.7 (b)(1) stipulating that delinquent reporting can block contract awards. The form provides contact information for the Joint Reporting Committee in Norfolk, VA, and highlights that most contracts under $10,000 are exempt from these reporting requirements.
