About this file
This memorandum requests approval for contractor employees to be issued Department of State FOBs to enable remote access to sensitive but unclassified systems. It details the rationale and responsibilities for issuing, using, returning, and accounting for FOBs. The contracting officer's representative may approve or deny each request.
The federal contract opportunity solicits proposals to provide inventory management and warehouse services for critical supplies used in passport production. The selected contractor must maintain an electronic ordering system; operate a warehouse within 30 miles of the National Passport Center in Portsmouth, New Hampshire; and provide staff sufficient to fill orders and ship supplies Monday through Friday. The contractor assumes ownership of the existing cloud-based system and is responsible for licensing and maintenance. Historical demand data will be used to forecast future needs. The Department of State, Bureau of Consular Affairs, Passport Services Directorate requires these services to efficiently produce passports nationwide.

Authorization Rationale
I am recommending that the above listed contractor employee(s) be issued a DoS FOB in order to conduct business offsite. I recommend approval of this request as I believe it to be in the best interest of the government because of the following reasons – (insert rational) ------
If this request is approved, the contractor employee will be requested to complete an on-line application for teleworking which will be reviewed by the COR for consideration. The contractor employee will be required to sign a hand receipt for the FOB issued. A copy of this document will be retained by the contractor employee, GTM, COR and the prime contractor. When the contractor employee returns the FOB to the COR or GTM, a hand receipt will be will be issued and a copy will be sent to the GTM, COR and prime contractor. The FOB will be paid for by the government and will be treated as government furnished equipment. Should the contractor employee lose the FOB or otherwise fails to return it to the COR or GTM, the cost of the FOB will be deducted from the Contractors invoice.
In the event, that the FOB request is disapproved by the COR and there is a disagreement between the GTM and COR on the disapproval of a FOB request, the Contracting Officer shall review the request, after receiving the recommendation of the Office Director and make the final decision.
Program Office and Contractor Responsibility
The Program office (OFFICE SYMBOL) agrees to absorb any costs associated with the issuance, care and maintenance FOB. The GTM or the COR will be responsible for the issuance and collection of the FOB.
Pursuant with FAR, 52.245-1 “Government Property” the Contractor will be responsible for the control, use, preserve, protect, repair and maintain Government property in its possession.
