a. Required Questions. Bidders bidding on Category 1 – Mentors must complete REQ ID’s M-1 through M-5. Bidders bidding on Category 2 – Program Administrators must complete REQ ID’s PA-1 through PA-5.

b. Supporting Information. Please make sure to include references in Exhibit A-2 – Bidder Profile that align with the information provided in this exhibit. Enterprise Services may request additional information or references to help verify the information provided by the bidder in this exhibit. A bidder’s failure to provide requested information to Enterprise Services within five (5) business days may result in disqualification.

c. Bidder’s Commitments. Please note the bidder’s responses to some of the performance requirements below (e.g., customer service) will be added to the resulting contract, so bidder is cautioned to indicate commitments that they can meet throughout the term of the contract. If a contractor is not able to honor these commitments throughout the contract term, they may be subject to early contract termination that could impact their ability to work with the State in the future. Please see Exhibit D – Contract.

d. Word Limit. Limited responses may receive lower scores than those that demonstrate knowledge of the subject matter. Each question has stated word count limits, any text in excess of those stated word limits will be deleted prior to forwarding the submittals to the evaluation team. Each written response must be complete and will be evaluated on its own merits. Unless specifically instructed, Bidders may not include links to external content or include embedded documents. Any such additional materials will not be evaluated.

e. Company Identifiable Information. The State intends to conduct anonymous evaluations to minimize risk of any potential bias. Bidders must not include any company names, logos, employee names, or other specifically identifiable information in their responses to this exhibit. The State will review all bid submittals prior to forwarding them to evaluators, and if the State finds company identifying information in the submittal, Enterprise Services will notify the bidder, and allow them two (2) business days to correct and return their response. Bidders will not be allowed to alter any other information other than to remove the identifying information. If the bidder does not make the required adjustments, Enterprise Services will make modifications to remove the identifying information and deduct two (2) points from the bidder’s score for this exhibit.

Contract Performance Experience. The State is seeking a Contractor with extensive industry knowledge and experience in bidding and winning government contracts.

The State is seeking a Contractor who can create high-quality deliverables (reports, assessments, etc.) written in accessible, viewable formats that are professional , consistent in appearance, provided in timely manner, and which can be used to track progress and results throughout the program.

Added value considerations: Provide and address two sample deliverables from the contracts identified in Question No. 2 above “Contract Performance Experience” that demonstrate the bidder’s strategic approach, subject matter expertise, and ability to create an accessible, professional, and a well-organized work product. For each sample, provide the following:

Sample Work Product - The State is seeking a Contractor who can monitor and track progress of the program and participants. The Contractor will gather data and create a report format. The Contractor will also be expected to provide a summary of the data to evaluate and measure the performance of the program.

Added value considerations: For the proposed Program Administrator who will be providing these services under the contract, provide two (2) sample deliverables that demonstrate their written communication style, subject matter expertise, and ability to create a professional and well-organized work product. For the samples, provide the following:

(d) attach a PDF file of the sample deliverables or enter text and any images into the space provided to the right; one (1) spreadsheet style report, chart, or graph, etc. showing data points over time & one (1) written summary analysis.

(e) Describe the data provided

(f) Provide a spreadsheet style report, chart, or graph, etc. that tracks at least four (4) different metrics over at least a six (6) month period

(g) Provide a summary analysis to include recommendations relevant to the data provided

To meet minimum acceptable standard [0%]: At a minimum, bidders’ samples must demonstrate composed written and/or drafted deliverables conveying subject matter expertise and the ability to communicate content to the reader.

(a) Provide at least two (2) samples – one (1) report sample and one (1) sample summarizing the report

(b) Describe the data content in the report sample

Bidders who provide no samples or provide samples that do not demonstrate any subject matter expertise or ability to effectively craft the deliverables may be disqualified from further consideration.
