BLUFFDALE QUALITY ASSURANCE PROGRAM
1.1 Scope
A. Bluffdale Quality Assurance Program defines policies, elements, activities, and guidelines to assure that the materials and workmanship in all roadway construction projects conform to the requirements of the approved plans and specifications. The program consists of a Materials Acceptance Program and Independent Assurance requirements. All quality assurance personnel are expected to be familiar with all aspects of the Quality Assurance Program (QAP) and the Independent Assurance (IA) requirements.
PART 2 QUALITY ASSURANCE PROGRAM OVERVIEW
Bluffdale Quality Assurance Program implements a schedule of activities and requirements to cover testing personnel competency and equipment operations during both owner performed and contractor performed testing programs. The program includes two levels of assurance.
1. Material Acceptance Testing
2. In place Testing
2.1 MATERIALS ACCEPTANCE DECISION BASED ON MANUFACTURER INFORMATION
A. This applies to off-the-shelf or off-site manufactured materials where production of products/materials is independent of project considerations. Covered under this program are all piping and fixtures, geotextile fabric, landscape materials, electrical wiring and equipment, etc.
2.2 MATERIAL ACCEPTANCE BASED ON UDOT QUALITY MANAGEMENT PLANS (QMPs)
A. This City will use the information from the UDOT QMP to for acceptance of supplied materials such as Portland cement, asphalt binders and emulsions, steel reinforcement, hydrated lime, pavement marking paint, etc.
2.3 MATERIAL ACCEPTANCE USING SAMPLING AND TESTING
A. This applies to all mass-produced materials where large lots or batches can be tested and can represent multiple projects. Materials covered under this approach include

Portland cement, asphalt binder and emulsions, steel reinforcement, hydrated lime, pavement marking paint, etc.
1. Testing Category 1 – City Performed Testing. City employees or their representatives perform acceptance testing according to the Bluffdale Minimum Sampling and Testing Requirements (BMS&TR) and project specifications. The validation of any testing program follows Section 007210, Material Dispute Resolution and the Independent Assurance Program
2. Testing Category 2 - Quality Testing Performed by Contractor/Supplier (QC) The City may use Contractor / Supplier quality control (QC) sampling and testing as part of the acceptance decision when material quality is validated by Bluffdale through additional testing and data verification in accordance to this document and Section 007210, Materials Dispute Resolution.
2.4 INDEPENDENT ASSURANCE PROGRAM
A. The Independent Assurance program sampling according to the UDOT Minimum
Sampling and Testing program and testing laboratories that follow the UDOT Laboratory Qualification Program (LQP).
2.5 INSPECTOR AND TEST TECHNICIAN QUALIFICATION
A. All inspection and testing of materials and construction must be performed by personnel properly trained according to the UDOT Inspector Qualification Program or a program approved by the Engineer and that the inspections be completed according to an APWA or UDOT procedure or another defined procedure as approved by the Engineer.
2.6 LABORATORY QUALIFICATION
A. The material testing laboratories must be qualified according to UDOT testing lab qualification program. Submit all qualifications certifications to the Engineer.
PART 3 MATERIALS ACCEPTANCE PROGRAM
3.1 MATERIALS ACCEPTANCE DECISION BASED ON MANUFACTORER INFORMATION
A. Submit all certifications for off-the-shelf or off-site manufactured materials where production of products/materials being placed/installed/incorporated into the project, including piping and fixtures, geotextile fabric, landscape materials, electrical wiring and equipment, etc.
B. Acceptance of materials based on:

1. Manufacturer’s product data, specifications, and recommended installation instructions
2. Manufacturer’s labeling
C. Submit all other material certifications to the City prior to installation, and completion of a visual inspection form describing installation details. Submittals and inspection forms must identify all pertinent product data indicating conformance with standards and specifications.
3.2 MATERIAL ACCEPTANCE BASED ON UDOT QUALITY MANAGEMENT PLANS (QMPs)
A. Supply Materials from qualified suppliers and producers under the UDOT Qualified Supplier List based on the following UDOT Materials Quality Management Plans:
1. 502 Cement
2. 503 Reinforcing Steel Epoxy
3. 504 Reinforcing Steel

12. 514 Hot Mix Asphalt
B. Provide documentation, including bills of lading and a visual inspection form identifying the quantity of material used and the qualified source.
3.3 MATERIAL ACCEPTANCE USING SAMPLING AND TESTING
A. Submit to the City proper plat material certifications showing that the material/products meet City Standards for any and all materials coming from or used by a plant that is supplying construction materials for a City projects
3.4 SOURCE TESTING
A. Submit a certificate of compliance to the City before using or placing the materials.
The City may verify the certificates.
3.5 FIELD TESTING
A. Field testing is required for all field placed/installed/constructed items including Portland cement concrete, hot mix asphalt and soil/aggregate applications. Have only
