STATEMENT OF WORK

DESIGN-BUILD (DB)

Project Title: Replace Fire Alarm Control Panel, Bldg. 8195

Project Location: Vandenberg AFB, CA

Base Project Number: XUMU 18-1001B

Date: November 30, 2018

1.0 INTRODUCTION

The Prime Contractor, hereafter referred to as the Contractor, shall conduct all research and field investigation required to prepare a complete and usable 100% design and construction for the effort described in this document and all attachments. Following the acceptance of the 100% design, the Contractor shall construct and install the

100% design as a solution to the situation described in this document. This Statement of Work (SOW) sets forth the requirements for a Design-Build (DB) delivery method for this construction effort at Vandenberg AFB, CA.

The Contractor is to provide the government with a complete engineering design, construction, and warranty for providing a new fire alarm panel and code compliant alarm and detection system in Building 8195.

1.1 CURRENT SITUATION:

The existing Fire Alarm Control Panel (FACP) has exceeded its life expectancy and is no longer supported by the manufacturer. The equipment has been in place for approximately 25 years and is becoming too expensive to maintain in its current condition. Also, the level of fire protection is being degraded by the system’s excessive age.

2.0 SCOPE OF WORK

The Contractor shall perform the services described in this document in two phases; the design phase and the construction phase.

2.1 CONTRACTOR PROPOSAL: As part of the proposed effort on this contract, the Contractor shall implement the following tasks for the completion of this project. The Contractor’s proposal shall include information on each type of equipment they are proposing to install (i.e. cut sheets from the manufacture) along with a basic sketch of planned improvements to what is required/recommended as part of this SOW.

2.2 GENERAL DESCRIPTION OF PROJECT:

The Contractor shall investigate, design and build (DB) a solution to bring the below issues up to code compliance and to meet all base, federal and user requirements. The Contractor shall develop a design with sufficient information to allow a construction contractor to propose, design and construct a design-build solution meeting these performance requirements. The Contractor shall design the proposed solution to the situation described. The Contractor’s scope includes, but is not limited to, all investigation, design and installation to:

(1) Replace the Fire Alarm Control Panel (FACP) and provide detection to bring the entire facility up to all applicable standards and codes. The contractor shall ensure that the new fire alarm is compatible with the present alarm receiving system, and complies with all applicable Building Codes and ADA.

The Contractor shall be responsible for the professional quality, technical accuracy and coordination of all designs, drawings, specifications and other documents or publications upon which construction is based. The design of the fire detection system shall be accomplished, reviewed, approved and sealed by engineers who are licensed professionals with a minimum of six years of experience in the state or California.

(2) The design and construction of this project shall conform to the requirements of UFC 3-600-01, NFPA 72, NFPA 101, and any other applicable codes and regulations.

(3) The Contractor is responsible for the complete project design, shall retain all responsibilities, and be recognized as the Designer of Record.

(4) The design and construction shall include but not be limited to demolition of the existing fire detection system, and installation of new pull stations, smoke detection devices, strobes, horns, and a fully addressable Hochiki America Fire Alarm Control Panel or approved equivalent. Contractor must provide all non-proprietary fire alarm equipment. At the completion of the project, the contractor must provide the government with ALL fire alarm programming materials including but not limited to device programmers, security codes, access codes, key codes etc. All fire alarm devices must be capable of reporting "Contact Identification" to the existing Radionics D6600 receiver via DACT.

(5) The Contractor is responsible for determining the number and location of smoke detection devices, pull stations, and strobe devices needed in each area of the facility in order to meet code requirements.

2.3 ENVIRONMENTAL REQUIREMENTS:

(1) Investigate, design and meet any requirements for work.

(2) Construction General Permit and Storm Water Pollution Plan (SWPPP) required for site disturbance over 1 ac.

(3) Low Impact Development (LID) standards shall be applied.

(4) Meet requirements as defined in the AF Form 813(if applicable), AF Form 332, and Environmental

Specification Section 01 57 20.

(5) Field investigate work may require AF Form 103 Work Clearance Request, see 01 00 00 General

Requirements for more information.

2.4 GENERAL DESIGN REQUIREMENTS:

(1) All designs shall comply with DoD Unified Facility Criteria (UFC) and Unified Facility Guide

Specifications (UFGS).

(2) All designs shall comply with the Vandenberg AFB Installation Facilities Standards which may be found on the Whole Building Design Guide website or by contacting the PM.

(3) All designs shall research and comply with all applicable DOD Instructions, Air Force Instructions, Engineering Technical Letters (ETL), 30SW Instructions (SWI), etc.

(4) All designs shall comply with local, State (California) and Federal standards included but not limited to those outlined in attached specifications.

(5) The Contractor shall ensure all equipment, mechanical, electrical, architectural elements, etc. meet all applicable codes, standards, etc.

(6) Whenever two codes conflict, the more stringent code shall apply.

2.5 APPLICABLE DOCUMENTS: The Contractor shall identify and comply with all applicable federal, state, and local statutes; Air Force/ Military instructions, manuals, handbooks, regulations, guidance, and policy letters; Executive Orders; Unified Facilities Guide Specifications (UFGS); Whole Building Design Guide

(WBDG); American Petroleum Institute Codes; National Association of Corrosion Engineers; National Fire

Protection; Steel Structures and Painting Council; National Electrical Code; Uniform Fire Code; Occupational

Safety and Health Administration (OSHA) regulations; Environmental Protection Agency regulations; and

International Building Code including all changes and amendments in effect on the date of issuance of each contract. It is the contractor’s responsibility to identify and comply with all applicable and relevant requirements.

2.6 FIELD INVESTIGATION: After contract award, the Contractor shall conduct additional field investigation that will include, but is not limited to, the following:

2.6.1 Record Drawings: The Contractor shall request/research any available electronic and/or hard copy information from the Air Force Civil Engineering Record Drawing Vault that pertains to each project. The

Contractor shall not rely on the accuracy of the reference information and must conduct independent site verification of all conditions.

2.6.2 Verification of Existing Conditions and Site Survey: Use of existing condition data provided by the Air

Force conveys acceptance and as such does not relieve the Contractor of liability associated with performance as the Engineer of Record. The Contractor shall be solely responsible for verification and validation of existing conditions, coordination of existing conditions in parallel with proposed requirements, and above and below-grade condition assessment. The Contractor shall visit each site and facility to verify existing conditions that are significant to the project. Additional site visits may be required as necessary. The Contractor shall perform instrument surveys, as required. All site visits shall be coordinated with the CO and PM.

2.6.3 Environmental Surveys: If applicable, the Contractor shall prepare, using individuals certified for asbestos, lead-based paint, polychlorinated biphenyls (PCBs), and underground storage tanks (USTs) identification, up-to-date surveys identifying all asbestos, lead-based paint, PCBs, and USTs pertaining to each project. Existing environmental surveys obtained from the Air Force PM shall be used only as a template for updating. Environmental surveys shall be included in the proposed cost.

2.6.4 Hazardous Material Surveys: If applicable, the Contractor shall perform an Asbestos Survey and Lead

Based Paint Survey prior to the construction phase of this project. This information shall be reviewed and a

Hazardous Materials Abatement Plan produced to address these hazardous materials, if found. The Contractor shall conform to the requirements of Vandenberg AFB Specification Environmental Specifications.

2.6.5 Subsurface Features: Unless determined by the scope of work, the Contractor is not required to perform excavation or other physical labor in connection with location of underground utilities, conduits, pipelines, or other subsurface features; however, if this is necessary for verifying existing conditions, the Contractor shall request assistance from the Air Force PM in identification of these subsurface features.

2.6.6 Coordination: The Contractor is responsible for design coordination with the CO and PM, using agency, facility managers, and other base officials.

2.6.7 Surveying/GIS Files: All projects that include surveying and/or GPS data collection must provide results in GIS formats according to the formats designated in Vandenberg AFB Specification Section 01 00 00. This is in addition to any other report or files generated by survey work.

2.7 COMPUTER-AIDED DRAFTING (CAD): The Contractor shall prepare and submit CAD drawing files and GIS files with each of its design submittals.

2.7.1 CAD Project Number: The Contractor shall use the XUMU (or equivalent) project number as the CAD project number.

2.7.2 Electronic Media:

(1) The Contractor shall ensure to submit the electronic files including all drawing files, X-REFS, and images for the project, including PDF and DWG versions.

(2) The Contractor shall ensure the electronic drawings are in compliance with the VAFB CAD

Standards. Coordination of the drawings with the 30CES Design Vault is required.

(3) The Contractor shall submit the electronic drawings on CD-ROM or DVD.

(4) The Contractor shall ensure the electronic drawings are readable by AutoCAD, using Microsoft

Windows format, without any additional hardware or software.

(5) The Contractor shall label their disks and their cases with a minimum of: Contractor firm name, project number, project title, percentage designed, FOUO, and the Air Force PM.

(6) Design drawings and other files for projects become US Government property and shall become part of the 30 CES drawing records or used by the US Air Force.

2.7.3 Drawings: The Contractor shall submit the required size, media, and number of sets, as specified for each submittal. The Contractor must be consecutively number all sheets. When half size (17”x22”) (ANSI Size

C) drawings are referenced the Contractor may substitute a larger size drawing at the approval of the PM.

2.7.4 CAD Project File List: The Contractor shall submit a CAD Project File List. The CAD Project File List is a document that describes all of the files in a project. The Contractor shall ensure the CAD Project File List includes the following specific information:

(1) A description of the project:

a. Project number and title (if multiple projects, list all).

b. Any related projects.

c. Percentage designed.

(2) A description of the project development:

a. List of drawing numbers from as-built drawings or Base Comprehensive Plan tabs that were used to create the drawing files.

b. Type of site surveys or investigations performed.

c. Site conditions that differ from above drawings.

(3) Drawing files:

a. File names.

b. Descriptions.

c. Files referenced.

2.7.5 VAFB CAD Standards Orientation Briefing Class: The Contractor shall send a qualified CAD representative to attend a 1-hour orientation briefing class covering the use of the VAFB CAD Standards.

(1) The CAD Representative shall be experienced in AutoCAD. In addition to the CAD representative, the Contractor Project Manager or Design Architect-Engineer may also attend the VAFB CAD

Standards Orientation Briefing to become familiar with the CAD requirements.

(2) The VAFB CAD Standards Orientation Briefing may be taken during the Project Orientation Visit or during field investigations, but should be completed prior to the 35% Design Submittal.

(3) The VAFB CAD Standards Orientation Briefing must be scheduled with the Air Force CAD Manager at least 1 week in advance. Upon class completion, the Contractor Project Manager shall notify the

Air Force Project Manager of class completion.

(4) The Contractor shall obtain all Vandenberg AFB manuals and the CD-ROM package from the Air

Force CAD Manager during the VAFB CAD Standards Orientation Briefing.

(5) The Contractor may request additional VAFB CAD Standards Orientation Briefings. These will be performed as available.

2.7.6 CAD Compliance: The Contractor shall ensure that all submittals meet the CAD requirements of the

30CES Drawing Vault. The Contractor shall submit a batch check result with all submittals on the CD/DVDs.

2.8 NON-GRAPHIC DOCUMENTATION: The Contractor shall use a computer to produce all non-graphic documentation as follows:

(1) Microsoft (MS) Word 2010 format shall be used for all word processing applications. A word processing program other than MS Word 2010 may be used, but the files must be saved in MS Word

2010 format and the Contractor must ensure that the documents are saved without loss of data or structure. All word processing documents must use 11-point Arial as the font.

(2) MS Excel 2010 format shall be used for all spreadsheet applications. A spreadsheet program other than MS Excel 2010 may be used, but the files must be saved in an MS Excel 2010 format and the

Contractor must ensure that the documents are saved without loss of data or structure.

(3) Non-graphic documentation requirements may change at any time during the design for the convenience of the Government.

2.9 AF FORM 103 WORK CLEARANCE REQUEST: The Contractor shall adhere to all 30 CES Work

Clearance Request Requirements. These can be found on the AF Form 332 (Available from the CO or the PM after award) and the AF Form 103 (completed by the contractor). The Base CES Customer Service (Building

11439) will assist the Contractor with efforts to fully complete the Base Civil Engineering Work Clearance

Request (AF Form 103). The Base CES/CEO will assist in locating underground utilities and issue the AF Form

103 to the Contractor. It is the Contractor’s responsibility to have a completed and approved Form 103 prior to beginning any construction activities including clearing, laydown, mobilization, grubbing, exploratory geophysical work, etc. Coordination of the AF Form 103 requires a physical presence to carry the document around to multiple agencies on VAFB for coordination.

2.10 MONITORING REQUIREMENTS: Any and all monitoring (including but not limited to UXO monitoring, biological monitoring, avoidance of biologically or culturally sensitive areas) is the responsibility of the Contractor. This may be required based on what is described in the AF Form 103 and AF Form 332. It is the responsibility of the Contractor to follow all the proper requirements when performing investigative and research activities. 30CES may be able to provide some assistance with monitoring but this does not relieve contractor of responsibility.

3.0 DESIGN PHASE

The Contractor shall complete development of plans and specifications in accordance with defined levels of effort necessary to construct the project and provide as-built drawings (both electronically and in hard copy) for retention by the Government. The Contractor shall be responsible for implementation, research, coordination, and execution of all regulatory reviews, ensure technical adequacy of the final design, and provide quality control of all phases of the contract. Any concept design provided to the contractor is for reference and design coordination use only. It may not reflect final design conditions and it shall not be assumed to meet all specifications or CAD standards. The Contractor is responsible for all research, interviews, and investigation required for the project.

3.1 DESIGN REVIEW CONFERENCES: The Contractor shall attend two scheduled design review conferences: 65% Design Review Conference and 95% Design Review Conference. Additional conferences will be necessary only if there are issues that cannot be resolved by telephone. The design review conferences will be held at Vandenberg AFB to accomplish the following:

(1) Review design progress.

(2) Provide guidance and design criteria.

(3) Ensure that design criteria and regulations are met.

(4) Discuss and resolve design review comments from the Government.

(5) Prepare a short presentation to the government giving a quick overview of the design utilizing MS

Powerpoint or equivalent.

3.2 DESIGN SUBMITTAL REQUIREMENTS: In addition to the requirements of applicable UFC’s, the

Contractor shall include the follow in each design package:

(1) Executive summary with statement of work and all modifications.

(2) Table of Contents

(3) Minutes and sign-in sheets of all meetings pertaining to the development of the design.

(4) Previous government review comments and contractor responses.

(5) All appropriate Engineering Calculations.

(6) Design Drawings. All electronic copies of design drawings shall be provided in both PDF and

AutoCAD formats. The AutoCAD drawings shall comply with VAFB CAD Standards.

3.3 PROJECT PHASING: The Contractor shall accomplish this project in phases as agreed upon by the

Base civil engineers (CE) and production staff. All telecoms and meetings shall be coordinated by the Contractor with the CO and PM. The Contractor shall submit meeting minutes within 2 working days after each teleconference or meeting. The Contractor shall complete the work as described in this document.

3.4 KICKOFF MEETING/CHARRETTE: The Contractor shall attend a design “Charrette” on the day of or the day after the Kickoff Meeting/Site Survey. The design “Charrette” will allow the Government and/or

Contractor to discuss any specific concerns not previously addressed. The “Charrette” maybe combined with the

Kickoff Meeting at the discretion of the PM. The Contractor shall document decisions and meeting minutes and provide with the 35% Design below.

3.5 35% DESIGN SUBMITTAL: The Contractor shall provide a narrative design approach detailing a thorough understanding of the design effort required, without restating the provided design that includes an understanding of the design process, and applicable documents, codes, UFCs and disciplines with the proposal package. The narrative, combined with any provided concept, current or old design, and any documentation from the Charrette will be the 35% design. The 35% Design shall include a list of expected Specification Sections that are anticipated, this list does not represent a final list, but allows for initial coordination. This shall be submitted electronically for record purposes within 21 days of the Charrette. This is not a formal submittal for review, but rather a written submission for use during the 65% Design Review.

3.6 65% DESIGN SUBMITTAL: The 65% Design Submittal shall be representative of a 100% complete design and include all major scope items from the Contractor. The 65% designation does not mean a 65% complete design. Rather, it denotes a milestone requiring design review. This submittal shall include all details to ensure a full environmental/regulatory review by 30CES Installation Management Flight. The Contractor shall prepare the 65% Design Submittal that will include the following:

3.6.1 Design Analysis: The Contractor shall prepare the 65% Design Analysis by documenting the decision-making process. The Design Analysis shall include the following at a minimum:

(1) Table of Contents: The A-E shall provide a table of contents. All pages of the Design Analysis including the appendices shall have a uniform numbering system. The first page of each section and appendices should be included at a minimum.

(2) Executive Summary: The Contractor shall provide a written narrative overviewing the project scope and key project information. The Executive Summary shall be at the front of the Design Analysis. It shall also be provided as a separate PDF file on the submitted discs. The executive summary shall be usable to explain the scope of the project and general level of disruption and impact to base and surrounding locations.

(3) Investigation Report: The Contractor shall provide a written narrative summarizing all investigative, research and survey work performed to complete the design. This narrative shall include when it was accomplished, where it was performed and what the relevant result of the work. This section is only a narrative and the supporting data and reports shall be in the Engineering Data section.

(4) Written Explanation of Major Design Decisions: The Contractor shall provide a written narrative describing key design decisions and discussing the engineering reasons behind these. Each decision shall be in its own paragraph or subsection. The discussion shall include at a minimum: why the decision was needed, the decision itself, what the main alternative(s) were, and what code/engineering led to the choice. This section shall include any code conflicts and how the design addresses the conflict.

(5) List of Section Titles and Numbers: The Contractor shall prepare a List of Section Titles and

Numbers to be used in each project in accordance with the Master Format - Master List of Section

Titles and Numbers from the Construction Specifications Institute (CSI) Manual of Practice. The list shall only include the titles deemed appropriate by the Contractor.

(6) Recycled Materials List: The Contractor shall provide a preliminary list of recycled materials that can be used in the project. The Air Force Affirmative Procurement Program (AFAPP) mandates use of recycled materials found in the Comprehensive Procurement Guideline (CPG). The Environmental

Protection Agency (EPA) lists the types of products to be included in the AFAPP available on-line at www.epa.gov/cpg/products.htm. This shall be an appendix.

(7) Engineering Data: The Contractor shall provide calculations and other supporting engineering data or analyses for all items, including but not limited to equipment or system sizing. The data shall sufficient to justify any special requirements that require deviation from normally accepted commercial or Government standards in addition to normal engineering calculations. This should be an appendix.

(8) Environmental Surveys: The Contractor shall provide the environmental surveys and include abatement specifications if asbestos, lead-based paint, PCBs, or underground storage tanks (USTs) are affected by work pertaining to any project. This should be an appendix.

(9) Interviews: The Contractor shall document all discussions with the Air Force Project Manager, using agency, and other base officials including names, position titles, and telephone numbers.

Correspondence should be an appendix. Investigative interviews to determine design requirements etc. should be included in a separate appendix and referenced in the narrative.

(10) Manufacturer Data: The Contractor shall provide catalog cuts or other manufacturer data of major equipment items. This should be an appendix.

(11) Charrette Documents: The Contractor shall include the Charrette documents as a reference for design review. This should be an appendix.

(12) Schedule of Material Submittals: The Contractor shall complete an AF Form 66, Schedule of

Material Submittals. The AF Form 66 indicates all material submittal requirements throughout the specifications listed by description of Item, Contract Reference, and Type of Submittal. The AF

Form is available in MS Excel. This should be an appendix and be available as a separate file.

(13) Other Supporting Information: Any other items determined necessary by the Contractor should be included in an organized manner as individual appendices.

3.6.2 Drawings: The Contractor shall submit half size (17” x 22”) (ANSI Size C) copies of updated Drawings plotted on bond. Drawings shall comply with Vandenberg CAD Standards. It is the Contractor’s responsibility to contact the VAFB 30CES Vault after award to obtain these standards.

3.6.3 CAD Project File List and Computer Files: The Contractor shall make final corrections from the 65%

Design Review Conference and all comments provided by the PM and submit an updated CAD Project File List and Computer Files as outlined in section 2.8, Computer-Aided Drafting (CAD).

3.6.4 Specifications: The Contractor shall prepare the Specifications in sufficient detail for a construction contractor to determine the type and quality of materials and methods required for installation of these materials.

The Specifications shall be bound or in notebook form and shall follow these guidelines:

(1) The Contractor shall use UFGS as the foundation for specifications. When UFGS does not apply, the

Contractor may use any appropriate Government or commercial guide specification service or their own specifications; however, the specification format shall strictly follow the Construction

Specifications Institute (CSI) Manual of Practice chapters on Master Format - Master List of Section

Titles and Numbers, 3-part Section Format, and Page Format.

(2) The Contractor shall specify generic characteristics of construction materials and shall avoid the use of proprietary “brand name” specifications, unless Vandenberg AFB requires a specific manufacturer for base-wide compatibility.

(3) The Specifications shall be clearly written and shall not conflict with the drawings. The Specifications shall be thoroughly edited so that they do not contain provisions that do not pertain to any project in this contract. Only include the applicable Section Titles and Numbers. Do not include Section Titles and numbers that are not applicable or will not be used.

(4) The following Vandenberg AFB standard specification sections shall be inserted into the

Specifications for all projects:

a. Section 01 00 00 General Requirements

b. Section 01 44 00 Contractor Quality Control

c. Section 01 57 20 Environmental Requirements

(5) Sections 01 00 00 and 01 57 20 require editing and coordination with the Air Force Project Manager for specific base requirements and restrictions. Section 01 00 00 provides a project description and other conditions such as occupancy, work schedule, work stoppages, security, utilities, safety, construction and welding permits, and submittals. Section 01 57 20 specifies environmental compliance and monitoring and disposal of removed materials. Special requirements may be added.

All changes from the base document must be documented until the 100% when a clean specification shall be provided. Any editing of the 01 57 20 are not valid until expressly approved from the PM.

(6) If applicable, the contractor shall use the Environmental Surveys to prepare the scope of asbestos, lead-based paint, PCB, and UST abatement and identify the locations of abatement requirements on the Drawings. The cost of all abatement shall be itemized separately in the abatement cost estimate.

(7) The Contractor shall become familiar with the Vandenberg AFB standard specification sections to ensure that the remaining specification sections do not conflict with or duplicate these provisions.

(8) The Contractor shall include VAFB standard specification Section 01 44 00 Contractor Quality

Control, or more stringent quality control section, as approved by the PM and the CO.

3.6.5 CAD Project File List and Computer Files: The Contractor shall make final corrections from the 35%

Design Review Conference and all comments provided by the PM and submit an updated CAD Project File List

3.6.6 Resubmittals: The Contractor may be required to resubmit the 65% Design Submittal if after review the

Government determines the submitted design does not meet the requirements set forth in the contract for the 65%

Design Submittal, if major scope items have not been addressed or if it contains sufficient errors that a 95%

Design Submittal could not be achieved without major revisions, as determined by the Government. Any submittal that upon review by the Government has technical errors or omissions will be rejected and shall be required to be resubmitted. Each resubmittal shall be labeled as “65% RX” with X being the corresponding resubmittal number.

3.7 95% DESIGN SUBMITTAL: The Contractor shall prepare the 95% Design Submittal by updating and completing the 65% Design Submittal. The 95% design package shall be in a 100% design state with the exception of any additional Government corrections and modifications. This shall include, at a minimum, the following:

3.7.1 Design Analysis: The Contractor shall make final corrections from the 65% Design Review Conference and all comments provided by the PM and submit an updated Design Analysis. The Design Analysis shall be bound or in notebook form. In addition to 65% Design Submittal requirements, the Design Analysis shall include the following:

(1) Annotated Review Comments: The Contractor shall annotate the design review comment sheets received at the 65% Design Review Conference indicating the full disposition of each comment.

Provide the reference location where any changes were made to the contract documents. This is an addition to previously submitted Annotated Review Comments from the 65% Design Submittal. This section shall include all comments from all the Design Review Conferences and the comments shall be ordered in reverse chronological order with the most recent comments first.

3.7.2 Drawings: The Contractor shall submit half size (17” x 22”) (ANSI Size C) copies of updated Drawings plotted on bond. Drawings shall comply with Vandenberg CAD Standards. It is the Contractor’s responsibility to contact the VAFB 30CES Vault after award to obtain these standards.

3.7.3 Specifications: The Contractor shall make final corrections from the 65% Design Review Conference and all comments provided by the PM and submit updated specifications. The Specifications shall be bound or in notebook form.

3.7.4 CAD Project File List and Computer Files: The Contractor shall make final corrections from the 65%

Design Review Conference and all comments provided by the PM and submit an updated CAD Project File List

3.7.5 Resubmittals: The Contractor may be required to resubmit the 95% Design Submittal if after review the

Government determines the submitted design does not meet the requirements set forth in the contract for the 95%

Design Submittal or if all changes from the 65% Design Review Conference or if all the comments provided by the PM were not completed. The 95% will not be accepted if there are any outstanding technical errors or omissions. Each resubmittal shall be labeled as “95% RX” with X being the corresponding resubmittal number.

3.8 100% DESIGN SUBMITTAL: After final acceptance of the 95% Design Submittal by the Air Force

Project Manager, the Contractor shall submit the required originals and number of sets of copies plus all

Computer Files as outlined in section 2.8, Computer-Aided Drafting (CAD). The 100% Design Submittal shall include all requirements from 95% Design Submittal with all the required updates including but not limited to all licensed Professional Engineer stamps and seals. This shall reflect fully complete and ready for construction documents. After final acceptance of the 100% Design Submittal by the PM, the contractor shall submit the required originals and number of sets of copies plus all Computer Files. This shall include, at a minimum, the following:

3.8.4 CAD Project File List and Computer Files: The Contractor shall make final corrections all comments provided by the PM and submit an updated CAD Project File List and Computer Files as outlined in section 2.8, Computer-Aided Drafting (CAD).

3.8.5 Resubmittals: The Contractor may be required to resubmit the 100% Design Submittal if after review the Government determines the submitted design does not meet the requirements set forth in the contract for the

100% Design Submittal or if all the comments provided by the PM were not completed. Each resubmittal shall be labeled as “100% RX” with X being the corresponding resubmittal number.

3.9 DESIGN DEFICIENCIES: Government approval of the 100% design submittal does not relieve the

Contractor from the responsibility to comply with all applicable documents as identified in paragraph 2.5. The

Contractor shall be responsible to correct design deficiencies and provide appropriate revised drawings and specifications to the Government within 5 calendar days of notification by the Contracting Officer. These design deficiencies may be identified at any time between approval of the 100% design submittal and final inspection.

The Contractor shall be responsible for performing any rework of construction required to correct the design deficiency at no additional cost to the Government.

4.0 CONSTRUCTION PHASE

Upon the approval of the CO and issuance of the construction Notice to Proceed, the Contractor may commence on-site construction activities. The Contractor shall complete the work as described below:

4.1 ON-SITE FIELD OVERSIGHT: The Contractor shall provide the manpower, equipment, material, services, and transportation necessary to ensure oversight services throughout accomplishment of this design/build contract. It is the responsibility of the Contractor to establish and maintain open communications between salient authorities associated with accomplishment of this contract. During all aspects of work both the

Site Superintendent (SS) and the Contractor Quality Control Manager (QCM) shall be on site. The SS and QCM shall be two separate individuals unless expressly allowed for otherwise according to the PM and CO.

4.2 PERMITS: It is the responsibility of the Contractor to identify all permits, clearances and other documents required for construction as part of the design phase of this project. The Contractor shall obtain and pay for the identified permits, clearance and other documents prior to construction.

4.3 DELIVERY/WARRANTY: The Contractor shall complete all inspection and commissioning requirements prior to final inspection. Following final inspection, the Contractor shall deliver to the Government the signed and sealed as-built drawings in both hard copy and reproducible format. The Contractor shall issue a warranty in accordance with FAR 52.246-21. All parts and product warranties shall be provided with all paperwork completed by the Contractor on the Government’s behalf.

4.4 AS-BUILTS: Within 14 days after the final inspection, the Contractor shall deliver to the Government the signed and sealed as-built drawings in both hard copy and reproducible format. The Contractor shall provide as-built drawings (both electronically and in hard copy) for retention by the Government. The Contractor shall ensure the final as-built drawings meet all Vandenberg AFB (VAFB) CAD standards. The Contractor shall not assume any provided design meets all CAD standards. The Contractor is solely responsible for ensuring the accuracy and completeness of the As-Built drawings. The Contractor shall ensure at least one copy of the as-built drawings is submitted on Mylar.

4.5 SURVEYING/GIS FILES: All projects that include surveying and/or GPS data collection must provide results in GIS formats according to the formats designated in Vandenberg AFB Specification Section 01 00 00.

This is in addition to any other report or files generated by survey work.

4.6 FINAL PAYMENT: A minimum of 5% of the contract cost shall be withheld until the As-builts meet any and all requirements by the 30CES Drawing Vault. All corrections shall be made prior to final payment.

5.0 GENERAL REQUIREMENTS

5.1 QUALIFIED PERSONNEL: The Contractor shall ensure that only qualified, competent personnel carry out the tasks outlined in this SOW. Competent is defined as registered professional or, where registration is not applicable, trained, and/or certified in their respective field.

5.2 AIR FORCE PROPERTY: All materials gathered and/or developed in the performance of these tasks listed shall be returned to and become property of the Air Force; and shall not be used and/or distributed by the contractor without specific written permission of the PM.

5.2.1 Material resulting from demolition activities shall be disposed of by the Contractor through the 30CES

Qualified Recycling Program, the Defense Logistics Agency (DLA) Disposition Services or off base landfill, as appropriate. The VAFB Base Landfill is no longer available for projects at Vandenberg AFB.

5.3 DESIGN AND CONSTRUCTION REQUIREMENTS: The Contractor shall ensure the project conforms to all applicable Federal, State and local laws, regulations and base-specific standards and applicable

Unified Facilities Criteria (UFC). Where there is a conflict between the referenced standards, the most stringent shall take precedence. All work accomplished by the Contractor shall comply with these criteria unless a waiver is obtained from the Air Force when conditions warrant such consideration. The Contractor is responsible for all research, interviews, and investigation required for the project.

6.0 GOVERNMENT FURNISHED ITEMS

The following is a list of specific items of support that the Base CE will provide:

None specific to the project.

6.1 STAGING AREA: The Base CE will identify space on the jobsite or in a base contractor staging area where the contractor may deliver non-hazardous materials within the installation. An approval from Base

Planning as part of the AF Form 103 is required for laydown areas.

6.2 EXISTING ENGINEERING PLANS AND AS-BUILTS: The Base CE will provide the Contractor access to any existing, available and allowable engineering plans, drawings, diagrams, aerial photographs, digitized map files, etc., to facilitate evaluation of the site(s), as needed or requested. The primary method of access is for the Contractor to be allowed into the 30CES Design Vault for research on available documents.

Access to the 30CES Design Vault will be granted to the Contractor after award. The Contractor’s use of any existing plans, drawings, diagrams, etc. does not relieve the Contractor from ensuring the design matches existing site conditions or complying with Vandenberg AFB CAD Standards.

6.3 PRECONSTRUCTION MEETING: The Base CE will host a pre-construction (i.e., Kick-Off) meeting to advise and coordinate on local procedures, safety and security requirements, and project coordination activities.

The PM will arrange for a suitable Kick-Off meeting location with teleconferencing capabilities to ensure CO participation. The PM will ensure the appropriate base agencies attend the Kick-Off meeting to provide necessary information or briefings to facilitate the success of the contract effort.

6.4 BASE REQUIREMENTS: The Base CE will advise the contractor of the location and processes to obtain the following as needed:

(1) Personnel identification badges, vehicle passes, and/or entry permits.

(2) Information on base fire prevention/security practices and procedures.

(3) Access to utilities (electrical, water, sewer, phone, etc.) This includes a supply (e.g., fire hydrant or stand pipe) of large quantities of potable water.

(4) Security escorts for work in restricted areas.

(5) Safety training classes for the purpose of allowing access within secure or hazardous areas.

(6) A paved area where equipment can be cleaned.

(7) A set of keys to locks as necessary.

6.5 30CES CIVIL ENGINEERING PROJECT MANAGER (PM): The Base CE will provide a PM who will review and certify the Contractor’s progress by signing AF Form(s) 3064 and 3065 and Monthly Status

Reports for the Contractor. (Note: If the PM does not agree that the progress is as represented by the Contractor, he/she shall return the documents to the Contractor for correction.) The Government may utilize a Title II inspector to perform on-site inspections on behalf of the PM. If used, the Title II inspector does not have the authority to sign or approve work on behalf of the Government or direct the contractor to perform work outside of the terms of the contract.

7.0 DELIVERABLES:

7.1 PROJECT MANAGEMENT PLAN (PMP): The Contractor shall submit a Project Management Plan that includes a description and flowchart of personnel involved with the project and a thorough description of construction management. The construction management narrative shall include timing of permitting, lead time for material, use date for user, etc.

7.2 ACCIDENT PREVENTION PLAN (APP): The Contractor shall submit an Accident Prevention Plan that analyzes and addresses all safety concerns according to all specifications and other requirements.

7.3 QUALITY CONTROL PLAN (QCP): The Contractor shall submit a Quality Control Plan that analyzes and addresses all safety concerns according to all specifications and other requirements. The Quality Control Plan shall include a copy of a certificate of training in Construction Quality Management for Contractors (US Army

CoE/ NAVFAC) for each of the quality control managers.

7.4 CONTRACT PROGRESS SCHEDULE (AF 3064): The Contractor shall submit a signed AF 3064

Contract Progress Schedule, depicting an overall contract progress schedule for the main elements of work for the period of performance as described in the Table of Deliverables above. The Contractor shall also provide a line graph depicting actual construction progress (solid line) versus scheduled construction progress (dotted line) throughout the period of performance. The CE PM will sign the AF 3064 and send to the Contract Specialist for coordination and approval by the CO.

7.5 CONTRACT PROGRESS REPORT (AF 3065): The Contractor shall submit AF 3065 as described in the Table of Deliverables above. This report shall be signed by the contractor’s on-site representative in the

Remarks section of the AF 3065. The work elements and percentages of the total job identified on the AF 3065 shall be identical to the work elements and percentages on the AF 3064. If the Contractor is more than 5% behind schedule, the Contractor shall note in the Remarks section the reason why and what is being done to get back on schedule. The CE PM will sign the AF 3065 and send to the Contract Specialist for coordination and approval by the CO. If the Contractor requires reimbursement of a high dollar value item, the item shall be identified on the

AF 3065, an appropriate percentage applied, and the item must be received and stored on site.

7.6 MONTHLY STATUS REPORT (MSR): The Contractor shall submit Monthly Status Reports that communicate activities accomplished during the previous period, document discussions, identification of

30 Nov 2018 problems, proposed solutions, corrective actions taken, outstanding issues, and record issues or concerns. The

Monthly Status Report shall include a financial tracking table containing columns with the following headings:

(1) Task Breakout

(2) Current Project Status (summary of work previously completed and completed during the current reporting period).

(3) Task Value

(4) Effort % Complete

(5) Dollars % Expensed To Date

(6) Dollars Earned This Month

(7) Dollars Earned To Date

(8) Expected work activities for the following month

An AF 3064, AF 3065 signed by the CE PM and CO, and a Monthly Status report shall accompany each invoice.

The AF 3064, AF 3065, Monthly Status Report and invoice shall all cover the same period of time.

7.7 KICK-OFF MEETING: It is the responsibility of the Contractor to coordinate the date and time of pre-construction (i.e., Kick-Off) meeting with the PM and CO. During the meeting, the Contractor shall present a complete overview of the project and review the schedule, highlighting any issues (review periods, material submittals, etc.) which are key to maintaining the schedule. The Contractor shall take meeting minutes to include the recording of the attendance list.

7.8 CHARRETTE: The Contractor shall perform a Charrette as described in the Table of Deliverables. The

Charrette will allow the Government and/or Contractor to discuss any specific concerns not previously addressed.

The Charrette shall not be concluded until the Contractor has a complete, workable design concept.

7.9 PROGRESS MEETINGS: The Contractor shall perform Progress Meetings as described in the Table of Deliverables. These meetings will include all relevant base POC’s and Contracting Personnel on an as needed basis. These meetings will cover the progress made in the past period and will include any pertinent issues which could affect future contract progress. The following items shall be covered:

(1) Meeting Attendees and Contact Information

(2) Project Percent Complete (Actual and Scheduled)

(3) RFI’s and Statuses

(4) Material Submittals and Statuses

(5) Three Week Look Ahead

(6) Schedule

(7) Safety

(8) Resolution of Existing Issues

(9) New Business

7.10 FORMAT OF DELIVERABLES: The Contractor shall provide an electronic copy of all deliverables, formatted for use on the latest version and compatible with the Government’s version of Microsoft Word, Excel, Adobe Acrobat, and AutoCAD. The Contractor shall date all deliverables. The Contractor will be provided samples of the deliverables, upon request.
