About this file

This document is a Vendor Work Order Request form from the State of North Dakota, designed to facilitate the procurement of vendor services through a state contract. The form provides a standardized template for state agencies to request and document work orders, outlining the process for engaging vendors for specific projects or services.

The work order request form covers critical project details such as scope of work, staffing requirements, agreement type (Time and Materials or Fixed Bid), project timelines, and evaluation criteria. The document specifies a comprehensive proposal submission process that requires vendors to provide project strategies, staff qualifications, references, background checks, and cost proposals. Evaluation of vendor proposals will be based on three key areas: project proposal solution (30 points), qualification and availability of vendor staff (40 points), and cost (30 points). The form also includes provisions for tracking deliverables, determining equipment and office space responsibilities, and establishing reporting mechanisms for project progress.
