About this file

This document is an Iran Free Procurement Certification Form required by Pennsylvania's Procurement Code Sections 3501-3506 for vendors seeking contracts valued at $1,000,000 or more with Commonwealth entities. The form provides two options for vendors - either to certify they are not on the list of persons engaged in investment activities in Iran, or to demonstrate they have received an exemption from this certification requirement. The vendor must complete the appropriate option and provide the required signatures. The document outlines penalties for providing false certifications, including civil penalties, contract termination, and future ineligibility to bid. The form does not provide details on any specific contract opportunity, but rather establishes the certification requirement for all such high-value contracts with the Commonwealth of Pennsylvania.
