ACTIVITIES

On February 28, 2020, Penni Leiser, an Environmental Protection Agency (EPA) and Asbestos

Hazard Emergency Response Act (AHERA) Certified Building Inspector with EGE, collected fifteen (15) bulk samples of suspect building materials.

The collected samples were labeled with identification numbers, documented on a chain-of custody form, and submitted via chain of custody protocol to EMC Labs, Inc. (EMC) of Phoe-nix, Arizona for analysis. EMC is approved by the Arizona Department of Health Services pursuant to Arizona Revised Statute (A.R.S.) § 36-495.02 (5) to perform airborne and bulk asbestos fiber analysis (ADHS No. AZ0912). In addition, EMC is accredited by the National

Voluntary Laboratory Accreditation Program (NVLAP No. 101926-0) for bulk asbestos fiber analysis. Sample analysis was conducted via Polarized Light Microscopy (PLM) in general ac-cordance with EPA Method 600/R-93/116. The analytical laboratory report is attached to this letter report.

FINDINGS

According to federal regulations, any material containing greater than one percent (1%) as-bestos as determined by PLM is considered asbestos-containing material (ACM) and subject to abatement procedures. A summary of the samples collected by EGE is presented in the following report and materials identified by lab analysis to contain greater than 1% asbestos by PLM analysis are highlighted in bold:

CONCLUSIONS AND RECOMMENDATIONS

Based on the laboratory results of the sampling conducted on February 28, 2020, the following ma-terials sampled by EGE were identified as containing asbestos at levels greater than 1%.

• Bedroom G & Throughout Brown Gray Tile & Mastic 5% Chrysotile Non-Friable

• Bath I & Kitchen Black Mastic 3% Chrysotile Non-Friable

• Bath I Leveling Compound beneath Tile 3% Chrysotile Non-Friable

No other materials were sampled by EGE. If additional suspect building materials are discovered during renovation/demolition activities, the materials should be sampled and analyzed for asbestos content prior to disturbance or should be assumed Asbestos Containing Material (ACM) and handled as such.

LIMITATIONS

Ever Green Environmental’s conclusions and recommendations, as presented in this report, are based on our sampling and analysis. The samples collected and used for testing, and the observa-tions made, are believed to be representative of the area(s) evaluated.

Any quantities are estimates only and should not be relied upon for bidding estimates. Contractors should verify the quantities prior to bidding any abatement project.
