ITEM REFERENCE AND REMARKS ~ NARRATIVE REPORT

Posts / Piles / Tie Backs - Bent 2, Pile 5 and Bent 3, Pile 3 have a new pile driven beside the original pile. The original pile at Bent 2, Pile 5 remains solid and capable of carrying load; however, the original pile at Bent 3, Pile 3 is not. This pile has failed (Photo 12), which has increased the pile spacing between Pile 2 and the new pile driven adjacent to Pile 3 to 7.8'. This should be considered when rating the bridge to determine its load carrying capacity.

Abutment 5, Pile 2 is severely decayed at the ground line. It has decayed to a depth of 4” to 5”. A core approximately 4” to 6” in diameter remains. (Photo 13) Abutment 5, Pile 3 is decaying internally. (See Sketch) Wingwalls - One of the wingwall piles on the right side of Abutment 1 is decayed at the ground line (Photo 14), but continues to support the wall.

Caps - The cap at Bent 3 has a 2”x2”x1’ decayed area adjacent to a check between Piles 1 and 2. (Photo 15) No significant decay was noted.

Remarks

Deck Slab / Panels / Joists - The deck surface is slightly worn. (Photos 1 and 2) Aggregate in the concrete is exposed. There are small sections of concrete breaking out of the deck surface at the corners of the deck units as follows:

Some units have developed “D” cracking adjacent to the longitudinal joints as follows:

Span 1, Unit 3 has a 14”x4”x5” section of concrete broken out of the end over Abutment 1. This area has been filled with asphaltic material. Small sections of timber planks were attached to the backwall below this hole. (Photo 3) These were probably put in place to support a patch.

Curbs - The curbs have sections of concrete broken off of the ends at the point where the pipe bridgerail posts are embedded. An attempt was made to patch these areas, but the product used did not bond to the concrete in some areas. The patches on the left side of Span 1 and one of the patches on the left side of Span 4 remain intact. The other patches on Span 4 have fallen out. (Photo 4) The curbs were previously broken on the left side of Spans 1 and 3, and on both sides of Span 4. These areas were patched and a section of angle iron was welded to the post and turned down the face of the curb for additional support. (Photo 5) One of these angle irons on the right side of Span 4 has broken partially away from the post. (Photo 6) This could snag the wheel of an errant vehicle if not repaired. The curb is cracked adjacent to a bridge rail post on the left side of Span 2 (Photo 7) and the right side of Span 4.

Drains and Drainage, Cleanliness - Dirt and vegetative debris accumulating in the gutterlines is preventing proper deck drainage. (Photo 8)

Girders - Span 1, Girder 1 has a 20”x3”x2 ½” section of concrete broken out of the bottom 8’ from Abutment 1.

(Photo 9) The steel plates embedded in the ends of the girders are corroding adjacent to the bearing plates. (Photo 10) Bearing Devices - The steel bearing plates are corroding (Photo 11), but continue to serve the purpose for which they were intended.

Erosion or Scour, Channel Scour / Erosion, Observed Lateral Movement - The stream bank is scouring under Span 1 and at Bent 2 on the right side of centerline.

Remarks

Waterway Obstructions / Drift - There is a scrap 4”x10” timber lodged between Piles 2 and 3 at Bent 2. This could cause drift to accumulate. There is a small accumulation of drift on the right side of centerline on the far side of Bent 4.

The roadway surface was previously reported to be raveling adjacent to the Abutments. These areas have been patched with asphaltic material. (Photos 16 and 17) There is also a 5'x8' patch adjacent to Abutment 5 on the left side of centerline. (Photo 17) This is assumed to have been placed to address settlement in this area. The roadway is slightly lower than the bridge deck adjacent to Abutment 5.

Bridge Railing - The pipe bridge rails are rusting, but no significant corrosion was noted. The rail on Span 4 has been replaced. This rail system is inadequate. The condition of the concrete curbs also affects the adequacy of this system.

Signing - The approach roadway and the bridge deck are marked with a double yellow line painted at the centerline.

The striping has worn away on the deck and the approach roadway at the end of the bridge.

Object Markers - Object markers are in place. The object marker on the right side of Abutment 1 has been vandalized by gunfire, but the vandalism is minor. The object marker on the right side of Abutment 5 is bent at the top, but continues to serve its intended purpose.
