ITEM REFERENCE AND REMARKS ~ NARRATIVE REPORT

Wingwalls - The wingwall piles are decaying on the right side of Abutment 1 and on both sides of Abutment 3. The interior pile on the right side of Abutment 3 and the outside pile on the left side of Abutment 3 (Photo 10) are severely decayed. The outside wingwall pile on the right side of Abutments 1 and 3 are decaying due, in part, to the absence of the protective metal hood. (Photo 11) Caps - The cap at Bent 2 is beginning to decay adjacent to a split in the near face of the cap between Piles 2 and 3.

Waterway Obstructions / Drift - High water has deposited debris on the bearing plates. (Photos 4 and 12)

The approach roadway has begun to rut in a wheel-path at the far end of the bridge.

Remarks

Curbs - There are small, shallow spalls on the face of the curb at the point where holes were drilled for the bridge rail attachment. (Photo 1)

Bridge Railing - The thrie-beam bridge rail is bent on the top on both sides of Span 1 as a result of being struck by a fallen tree. (Photo 13) Two of the bridge rail splices are not supported by posts.

Drains and Drainage, Cleanliness - Debris accumulating on the roadway surface and in the gutterlines is preventing proper deck drainage. (Photo 2)

Girders - Span 1, Girder 14 has a 4”x3”x1” section of concrete broken out of the bottom 2.5’ from Bent 2. (Photo 3)

Bearing Devices - The steel bearing plates are rusting and becoming corroded in some areas but continue to serve their intended purpose. (Photo 4)

Erosion or Scour, Channel Scour / Erosion - Minor scour of the streambed is occurring adjacent to Bent 2 and on the bank adjacent to Abutment 3. (Photo 5)

Posts / Piles / Tie Backs - Abutment 1, Pile 5 was replaced with a 12”x12” mudsill column on each side of the original pile. (Photo 6) This repair is functioning well at this time, but the embankment under the footings is vulnerable and needs to be monitored. Bent 2, Piles 1 and 2 were spliced. (Photo 7) The original pile stub supporting the splice at Bent 2, Pile 1 is becoming decayed. It is decayed to a depth of 1.5" in approximately half its circumerence, resulting in approximately 20% section loss. (Photo 8) Abutment 1, Pile 3 and Abutment 3, Pile 4 (Photo 9) are decaying.

Approach Guardrails - A 12’ 6” section of guardrail has been damaged on the right side at the end of the bridge.

(Photo 14) This is probably the result of being struck by mowing equipment. One of the 6"x8"x14" timber, guardrail blocks is split on the left side at the far end of the bridge. (Photo 15) Object Markers - Abutments are marked with Type 3 object markers and the guardrail ends are marked with adhesive, reflective sheeting applied to the terminals. The adhesive object marker is missing from the guardrail terminal on the right side at the end of the bridge.
