About this file
This is an Offeror Self-Performance & Subcontracting Disclosure Checklist that establishes compliance requirements for contractors responding to a small business set-aside solicitation for Roll Up Door Biocontainment Enclosures (Solicitation 1232SA26Q0815) issued by the Department of Agriculture Agricultural Research Service. The checklist enforces limitations on subcontracting in accordance with FAR 52.219-14, requiring prime contractors to disclose their performance approach and subcontracting strategy.
Prime contractors must indicate whether they will self-perform the required work or utilize subcontractors. If subcontracting is planned, contractors must identify each projected subcontractor, provide their legal business name, Unique Entity ID (UEI), projected NAICS code for subcontracted work, and certify that each subcontractor is a verified small business concern listed in SAM.gov. The prime contractor must also certify compliance with the 50 percent limitation on subcontracting, ensuring that no more than 50 percent of the government-paid contract amount goes to firms that are not similarly situated, with any further subcontracting by similarly situated entities counting toward this ceiling. Non-compliance may result in termination for default, negative CPARS ratings, or other government remedies. The checklist requires completion in full with an authorized official's signature, printed name, title, and date; incomplete or non-responsive submissions may render the quotation ineligible for award.
