By signature in Block 15B Contractor releases the Government from all claims relating to this modification. This settlement is complete accord and satisfaction. Addendum to Amendment 2

REVISIONS, PRE-BID INQUIRIES, REQUEST FOR INFORMATION TRAIL CREEK BRIDGE CONSTRUCTION

REVISIONS

Revision-003

There is any error in the calculation for class 2 rip rap, pay item 25101a. The schedule of items has 36 cubic yard of class 2 rip rap.

After rechecking the calculations the volume should be 110 total cubic yards. An updated Schedule of Items is attached. See guardrail rip rap detail sheet 9, and channel section detail sheet 16 for rip rap locations. The class 6 rip rap volumes stay the same.

Revision-004

The proposed staging area on the north side of Trail Creek and west side of FSR 52 can be cleared and built up using road excavation.

The trees from the clearing may be used in stream simulation and any excavation placed there may be left. The excavation will need to be shaped after the project to blend with adjacent ground and to maintain drainage from the roadside ditch; maximum ditch invert elevation will be the top of the culvert at 5+50. The limits of this staging area are approximately stations 4+00 to the spring culvert near sta 5+50, as agreed with the Contracting Officer in the field.

Pre-Bid Inquiries (PBI’s) for Contract Bidding

PBI-008

Question: Is there a limit to the excavation vehicles we can use to haul waste material?

Answer: The contractor may propose to use vehicles over highway load limits and apply for an overload permit from the Forest Service. Any vehicle under highway load limits won’t need a permit.

PBI-009

Question: The government furnished material includes crushed aggregate and rip rap. Will the contractor be required to crush any aggregate?

Answer: No the government has crushed aggregate in the Hoodoo pit for contractor use. Rip rap may be sorted from excavation or hauled from any designated source.

PBI-010

Question: How is the in-stream log jam paid for?

Answer: This is included in pay item 64801, stream simulation.

PBI-011

Question: Will the contractor be able to get a waiver during a Level 4 fire shut down? And how will this affect the project completion period?

Answer: During level 4 shut downs there are several requirements to get a waiver including fire equipment and a water truck. The contractor will be responsible to submit for a waiver and provide the necessary equipment. If this affects the project completion date the Forest Service will adjust as needed.

PBI-012

Question: Can a contractor propose to excavate less asphalt and protect the existing pavement?

Answer: Yes the contractor may propose to excavate less asphalt and roadway on the south side Trail Creek. The contractor will be required to excavate all the designated asphalt on the north side of Trail Creek, to approximately station 7+30. They will then be required to rebuild the roadway and pave both lanes.

PBI-013

Question: Is the Forest Service positive there is no bed rock in the roadway excavation?

Answer: No the Forest Service is only confident on what is shown in the bore hole tests, see sheet 10.

PBI-014

Question: What are the size requirements for Class 2 and Class 6 rip rap?

Answer: The specification is Standard Specification FP-14 705-1 with the max diameter size for Class 2: 15”-21” and class 6: 39”- 45”. The contractor is responsible to meet size gradation requirements within the specification.
