14. DESCRIPTION OF AMENDMENT/MODIFICATION
Project Name and Location: TRAIL CREEK BRIDGE CONSTRUCTION This amendment addresses the following (see attached addendum)
1. Solicitation Revisions for project closures and additional warning signs (see attached addendum)

By signature in Block 15B Contractor releases the Government from all claims relating to this modification. This settlement is complete accord and satisfaction. Except as provided herein, all terms and conditions of the document referenced in Item 9A or 10A, as heretofore changed, remains unchanged and in full force and effect.

Addendum to Amendment 1
REVISIONS, PRE-BID INQUIRIES, REQUEST FOR INFORMATION TRAIL CREEK BRIDGE CONSTRUCTION
REVISIONS
Revision-001
To facilitate more efficient work during phases of bridge site preparation and construction, the Contractor will be allowed to schedule working hour closures of the bridge site for up to 4 weeks under the following conditions:
• Warning signs of the closure are posted at Granite, Starkey, Ukiah, and near Haines (along the Anthony Lakes Highway at the intersection with Foothill Road), warning of road closure of 52 road XX miles ahead, and listing hours of closure.
• Closures are scheduled 2 weeks in advance with contracting officer to allow for public notification.
• Closures are from 8 AM through 5:00 PM, with passage through site allowed from 12:00 – 12:30. Passage must be unimpeded before and after working hours and on weekends.
• Emergency vehicles will be allowed through site with minimal delay at any time necessary; detour cannot be taken out of use.
• Closure will be allowed to be scheduled for up to three separate times; not to exceed the total of 4 weeks of closure.
Revision-002
An additional warning sign shall be added along the Anthony Lakes Highway at the intersection with Foothill Road outside of Haines in Baker County. Road closure signs shall be added at the four warning sign locations during the road closure preiod proposed by the contractor.
Pre-Bid Inquiries (PBI’s) for Contract Bidding
PBI-001
Question:Is the contractor required to perform clearing and grubbing and what needs to be done with the trees?
Answer: Yes clearing and grubbing is required where necessary. The trees may be used in the stream simulation, excess trees and slash may be hauled away, merchantible trees may be decked on site as agreed to by the CO.
PBI-002
Question: Do the piles need to be prebored to the required depth of 968.0’? If so, can the piles be vibrated in after preboring?
Answer: Yes the pile holes will be prebored to the required depth, See FSSS 551.09. The piles may be vibrated in as long as they reach the required bearing resistance. Plastic casing may be put in the prebored holes then driven through if required for hole stability during drilling operations.
PBI-003
Question: The required pile tip elevation is 968.0’ with a pile cap of about 994.0, so are the piles about 25’ long?
Answer: The pile holes are prebored to 968.0’ and then the piles are driven to the required bearing resistance, see FSSS 551.09 and FP-14 551. The contractor shall cut off any excess pile length which becomes the contractors property. If depth is more than 50’ and additional pile length is needed, the contractor shall be paid for a splice and continue driving. After completion any excess piles become the contractors property.
PBI-004
Question: On sheet 12 there is a reference to a HP 12 x 74 pile but elsewhere it is a HP 14 x 89 pile?
Answer: This is a mistake, there are two locations on sheet 12 that mention a HP 12 x 74 pile. The piles for the project are HP 14 x 89 as specified in the schedule of items.
PBI-005
Question: Are we allowed to go beyond the existing silt fence on the northwest side of the project to set the temporary bridge?
Answer: No the contractor shall work within the existing silt fence on the North west side of the project, the contractor is not restricted on the south side of Trail Creek.
PBI-006
Question:It looks like the temporary bridge sill and permanent bridge rip rap overlap? Does the contractor need to set the rip rap prior to the temporary bridge sill?
Answer: The contractor can place the rip rap along the permanent bridge after removing the bypass bridge and concrete sill. Neither of these work items have to be completed in the instream work window. The contractor may complete the stream simulation with the temporary bridge in place.
PBI-007
Question: Will the Forest Service conduct fish removal?
Answer: The Forest Service is responsible for fish removal. The contractor will submit a dewatering plan and the Forest Service will place blocking nets and remove fish prior to beginning in stream work.
PBI-008
Question: Will the period of performance by extended to 2021?
Answer: The FS realizes the tight timeframe from suppliers, however, the goal is to finish the project this year with the current period of performance.
PBI-009
Question: Does the stream need to be dewatered prior to removing the existing culvert?
Answer: The stream does not need to be dewatered prior to removing the existing culvert. The contractor will be responsible to remove the existing fill and remove the exisiting culvert in a manner that limits turbidity downstream of the project. Stream simulation may also be done without dewatering as long as turbidity requirements are met.
PBI-010
Question: Does the Forest Service have a way to off load the temporary bypass bridge at the Forest Service compound? What materials is the Forest Service providing for the temporary bridge?
Answer: No, the contractor is responsible for picking up and returning the bypass bridge including the equipment needed to off load the bridge. The Forest Service is providing all the materials for the temporary bridge including the bridge, concrete sills, backwall, guardrail, connection bolts, and wood decking.
PBI-011
Question: There is a material source designated for rip rap, will the contractor be required to sort the rip rap?
Answer: The contractor will need to meet the specification and size requirements for rip rap class. The contractor may sort material from other sources or propose alternative locations for rip rap with approval from the Contracting Officer.
