SUMMARY OF WORK

PROJECT 22-D09H

J. DAVIS - INDIVIDUAL SEPTIC AND WATER SERVICE LINE INSTALLATION

PART 1 INTRODUCTION

1.01 SUMMARY OF WORK TO BE PERFORMED

A. Install a new septic system with a concrete septic tank and a gravel absorption drainfield with four 150 LF laterals.

1. The slope of the terrain will require three drop boxes, one between each lateral.

B. Install approximately 75 LF of 1” PEX water service line connection from existing water stub out at the house to the existing 3” PVC water main.

1. Water main will need to be live tapped.

2. Install new meter box, meter assembly, and pressure reducing valve (PRV).

C. A pre-proposal site visit is recommended but not required. All site visits shall be coordinated with the Project Manager.

1.03 NEED FOR PROJECT

A. The homeowner newly acquired the property and is building a new house on it. There is no existing water or septic infrastructure on the site.

PART 2 WORK TO BE COMPLETED

2.01 GENERAL

A. Work Execution Requirements

1. The contractor is responsible for determining the exact location of any and all utilities. The contractor is responsible for protecting utilities from damage during construction.

2. The contractor is responsible for obtaining applicable plumbing, septic and electrical permits required to complete the scope of work.

3. All debris generated during construction shall be removed from site.

4. All areas disturbed by construction activities shall be restored to the condition prior to construction.

5. A minimum of five days of continuous dry weather conditions is required prior to commencing work, unless explicitly waived by the project manager.

6. The contractor shall notify the Project Manager and receive written approval from the Project Manager prior to installing material quantities in excess of those specified on the price schedule.

7. Variation from this approved statement of work and specification requires prior written approval from the Project Manager for this contract.

8. The contractor shall keep the IHS Project Manager notified and advised of all work to be done or work in progress one week in advance so that proper inspection can be performed.

9. All work is subject to inspection by the IHS Project Manager or their representative.

10. All work is to follow Occupational Safety and Health Administration and other specified regulations.

11. The contractor shall provide a warranty of one year for all work completed in project, from date of substantial completion.

B. Technical Requirements

1. Work under this contract shall be performed in compliance with the codes, references, and standards listed below. If conflicts exist between these references, consult the IHS PM for direction.

a. The Oregon State Residential Plumbing codes and standard rules apply.

b. Oregon Administrative Rules (OAR Chapter 340-Division 71- Onsite

Wastewater Treatment Systems; ORS 454 - Sewage Treatment and Disposal

Systems).

c. Indian Health Service Measurement and Payment (see Attachment).

d. This project is subject to the Davis-Bacon Act; prevailing wages do apply.

e. TERO and other fees do apply.

2.02 CONCRETE SEPTIC TANK

A. The contractor shall install a new concrete septic tank.

B. Products

1. Septic Tank

a. Two-compartment tank.

b. Precast concrete material.

c. Capacity below outlet: 1,500 gallons.

d. 18-inch influent and effluent service access manholes.

e. Tank shall meet requirements of ODEQ Construction Standards 340-073-

0025.

f. Inlet and Outlet

1) 4-inch PVC Schedule 40.

2) Sanitary tees.

3) Compatible with and installed per ODEQ Construction Standards 340-

073-0025.

4) Outlet tee shall be compatible with effluent filter.

5) The invert of the inlet fitting must be at least 1 inch and preferably 3 inches above the invert of the outlet fitting.

2. Access Risers and Covers

a. 20-inch diameter water tight service riser above each access manhole.

1) If tank bury depth is 36 inches or greater, diameter shall be 30-inches minimum.

2) Adhere riser to tank per manufacturer’s instructions.

b. Riser cover must be gasketed and securely fastened.

c. Polylok or approved equal.

3. Effluent Filter

a. Maximum filter opening: 1/16-inch.

b. Dimensions: 3.25-inch diameter and 18-inch length.

c. Filter handle constructed of PVC or stainless steel.

1) Filter handle must be accessible from riser.

d. Acceptable products equivalent to:

1) Zabel A1800

2) Orenco 4” Biotube Filter

C. Execution

1. Install tank in accordance with manufacturer’s recommendations.

2. Place tank level on a solid flat level subgrade.

a. If over excavation is necessary to remove unsuitable subgrade soils, backfill to proposed grade with sand.

3. Seal inlet and outlet with temporary plugs until connections are made to the inlet and outlet lines.

4. Set the top of the tank a minimum of 12-inches below finished grade. Do not exceed 24-inch cover depth unless tank is designed for deeper bury depth and project manager approves.

5. Install access risers and terminate so access cover is 6-inches above finished ground surface.

6. Do not drive over the tank during and after construction.

7. Grade over tank to promote positive drainage away from top of tank.

8. Perform leakage test of tank.

a. The contractor shall provide water for the tank before leaving the site the day of installation.

b. Fill tank with water and soak for 24 hours.

c. Refill tank and measure leakage.

d. Repair or replace tank if leakage exceeds one gallon in two hours.

2.03 SEWER SERVICE LINE

A. The contractor shall install the 25 LF of sewer service line and cleanout from the house to the septic tank.

B. Products

1. Pipe:

a. Schedule 40 PVC, DWV pipe and fittings conforming to ASTM D2665, or

b. SDR 35 PVC pipe and fittings conforming to ASTM D3034

2. Fittings: solvent welded or gasketed joints.

3. Cleanout: Schedule 40 PVC DWV pipe and fittings conforming to ASTM D2665.

a. 4-inch solvent weld Schedule 40 PVC female adapter.

b. 4-inch diameter threaded slotted white polypropylene flush plug with threaded brass insert.

C. Execution.

1. Install 25 LF of new solid sewer pipe from the house to the septic tank.

2. Minimum cover over solid sewer pipe is 24-inches.

3. Minimum slope between the house and the septic tank is 1/8-inch per foot or 6 inches, whichever is greater.

4. 90-degree bends in the pipe between the house and the septic tank are not allowed.

5. Install new double cleanout between the house and the septic tank.

a. Install a 4-inch sewer wye in the sewer service line and connect riser of the same material from the wye to the ground surface.

1) Attach a schedule 40 PVC DWV female adapter to the end of the riser.

2) Install cleanout so cap is flush with finished grade.

2.04 GRAVEL ABSORPTION DRAINFIELD

A. The contractor shall install a gravel absorption drainfield with four 150 LF laterals, a distribution box, and a drop box between the second and third lateral.

B. Products

1. Solid Sewer Pipe and Fittings

a. SDR 35 PVC pipe and fittings shall conform to ASTM D3034.

2. Perforated Pipe

a. PVC pipe conforming to ASTM D2729.

3. Synthetic Gravel Cover

a. Synthetic gravel material shall be TYPAR Style 3151 or approved equal.

4. Gravel

a. Gravel shall be clean and may vary in size from ½ to 2-1/2 inch, with not more than 5 percent fines below the ½-inch size.

5. Distribution and Drop Boxes

a. Construct distribution/drop boxes out of concrete or injection molded HDPE.

1) Honey combed concrete boxes and lids will be rejected and replaced at

Contractor’s expense.

b. Box shall be supplied with a 4-inch inlet, outlet and two outlets for trenches.

c. All outlets shall be a minimum of one-inch lower than the inlet.

C. Installation

1. Construct drainfield in accordance with the Project Engineer’s design and drawings.

2. Solid Sewer Pipe:

a. Install ASTM D3034, SDR 35 solid sewer pipe from the Schedule 40 PVC outlet stub at the septic tank to the beginning of the drainfield.

1) Minimum cover over solid sewer pipe is 10 inches.

2) Minimum distance from septic tank to drainfield is five feet.

3) Solid sewer pipe from septic tank outlet to drainfield shall have a minimum drop of 2 inches or 1/8-inch per foot slope, whichever is greater.

3. Absorption Bed

a. Maximum trench depth: 36 inches.

b. Minimum depth: 24 inches.

c. Trench width: 36 inches.

d. Maximum trench length: 150 feet of perforated pipe in any given path.

e. Minimum distance between center line of trenches: 10 feet.

f. Rake to a depth of 1-inch all smeared or compacted surfaces of sidewalls and bottoms and remove loose material before gravel is placed.

g. Place gravel the full width of the trench and lay the perforated pipe level.

h. Gravel shall be two inches above and below the pipe.

i. Cover the top of the gravel the full width of the trench with synthetic filter material.

j. Backfill trench to a depth of 3 to 6 inches above existing grade.

k. Leave each premise in a neat and orderly condition, restoring it as near as possible to its original condition and to the approval of the Project Engineer.

4. Distribution and Drop Boxes

a. Install a distribution box between the septic tank outlet and the drainfield.

b. Install a drop box before the second, third, and fourth laterals (3 drop boxes in total) to ensure minimum bury is met.

2.05 WATER SERVICE LINE

A. The contractor shall install 75 LF of 1” PEX from the community water main to connect to the water stub out at the home. The contractor shall install a saddle and corporation stop to live tap the main. A curb stop and box shall be installed approximately 10 LF from the water main connection. The contractor shall also install and individual meter box, meter, setter, and AMR register.

B. Products

1. Water Service Line Pipe

a. 1” cross-linked polyethylene (PEX) pipe.

b. AWWA C904 and ASTM F876.

c. 160 psi pressure rating.

2. Fittings

a. ASTM F1807 and F1960 brass fittings joined by compatible crimp or expansion methods respectively.

3. Saddle

a. Minimum pressure rating of 200 psi.

b. Buna-N rubber gaskets.

c. Stainless steel nuts and bolts.

d. Minimum 4 inches wide for 3-inch diameter pipe and 5 inches wide for 4-inch diameter pipe.

e. Stainless steel single bolt saddle clamp equal to Ford FS101 or approved equal.

4. Corporation Stop

a. No-lead brass, meeting NSF-61, with pack joint connections.

b. Non-flare connections for all pipe types, equal to Ford pack joint compression couplings with stainless steel stiffeners.

c. Ford F1600-3-NL or approved equal.

5. Curb Stop

a. No-lead brass, meeting NSF-61, with pack joint connections.

b. Non-flare connections for all pipe types.

c. Minneapolis pattern.

d. Ford B44-444M-NL or approved equal.

6. Curb Stop Box with Stationary Rod

a. Cast iron Minneapolis pattern, length to match bury depth.

b. Stainless steel stationary rod.

c. Pentagon plug.

d. A.Y. McDonald 5614 or approved equal.

e. Curb box lid: A.Y. McDonald 5607L or approved equal.

f. Supply one pentagon wrench and one combination key equal to A.Y.

McDonald Catalog #304B, 36 inches long or approved equal.

7. Water Meter

a. Neptune T-10 5/8” x ¾” positive displacement meters or approved equal.

8. AMR Register

a. Neptune R900i MIU or approved equal.

9. Meter Box

a. Oldcastle Fibrelyte FL12 Fiberglass Meter Box or approved equal.

b. All no-lead brass appurtenances.

c. Included in the pit: 18” meter setter, meter, AMR register, PRV, and inlet angle shut off valve.

d. Minimum depth: 36” below grade.

e. Center locking lid with 4-inch thick foam insulating pad and corrosion resistant bottom plate.

10. Meter Setter

a. Copper tandem setter for 5/8” x ¾” meter and PRV with meter setter height of

18”.

b. Equal to A.Y. McDonald 752-218QF22 44 HxH TAN MSTR SPBVxFPDCK

11. PRV

a. Utilize a ¾-inch pressure reducing valve with outlet pressure of 60 psi.

b. Adaptable to 5/8” x ¾” meter setters.

c. No-lead construction with stainless steel seat, adjustment and cage screws as well as an integral strainer screen.

d. Equal to Watts LFU5B-Z3.

C. Installation

1. The contractor shall furnish and install water service lines at the locations on the plans. Water service line shall be installed from the water stub out at the house to the water main tapping location.

2. Inspect pipe and fittings for defects. Remove materials from the site that are defective, damaged, used, unsound, or that do not otherwise meet the specifications.

3. Bury the line a minimum of 24” below finished surface and a maximum of 36” below finished surface.

a. Insulate where installed above recommended frost line of 18”.

4. Install warning tape 12” below finished surface.

5. Provide sufficient slack in the water service line to prevent pulling fittings apart and to allow for contraction due to temperature changes.

6. Install saddle and corporation stop at tapping location on existing water main.

a. Connections shall be live tapped through the corporation stop with an approved tapping machine.

1) Tapping equipment shall be disinfected prior to use on the water main.

b. Existing water main material is assumed to be 3” PVC. However specific location and material shall be verified by contractor.

c. Damage to the existing main shall be repaired at the contractor’s expense.

d. Utility staff shall be present during the tapping operation to inspect work.

7. Install curb stop, box and stationary rod.

a. Set curb stops on a solid concrete block 4 inches thick by 8 inches wide by 16 inches long placed on undisturbed earth.

b. Install stationary rod on curb stop.

c. Set the top of curb boxes flush with finished grade elevation.

d. Support curb box during the backfilling operation to prevent movement and maintain a vertical position.

e. Provide on curb wrench and one pentagon key.

8. Install meter pit in the location designated on the plans or as instructed by the project engineer in accordance with manufacturers’ recommendations and the attached standard details.

a. Level meter pit on two 4”x8”x2” concrete blocks.

9. Install meter and AMR register per manufacturer’s recommendations.

10. Disinfect water service line.

a. Disinfect the new water service facilities prior to connecting to the corporation stop and the home. Near the home, at a location approved by the project manager, the water service line will be extended to the surface and capped with a lead-free brass ball valve.

b. Introduce sufficient chlorine solution into the corporation stop end to the water service line to produce a minimum chlorine residual of 200 ppm when filled.

c. After connection to corporation stop and pressurizing, flush the service line at the ball valve after three hours.

d. After service line is sufficiently flushed, obtain a water sample for bacteriological testing. Repeat process until a negative bacteriological report is received.

e. Connect the water service line to the home upon receiving a negative bacteriological report.

f. Waste flushed disinfection water in an environmentally safe manner. The method used is subject to the approval of the project manager.

11. Test water service line for leaks.

a. Turn on corporation stop and apply main pressure to the service line in the presence of the IHS representative before backfilling.

b. Repair all visible leaks and retest the line until test is successfully completed at no cost to the contract.

12. Backfill material shall be sand, gravel, or loose soil that is free of rocks and debris.

Backfill shall be free of stones larger than 3 inches.

2.06 ROCK REMOVAL (OPTION ITEM)

A. If rock is encountered during excavation, rock removal costs will be added at unit price per cubic yard of rock removed.

B. Test pits completed in May 2024 showed clay loam to at least 2 feet below surface and up to 6 feet below surface. In two test pits, sandy loam was found below 3 feet. Rocky cobbles were encountered below 3 feet in all three test pits.

C. Definitions:

1. Solid Rock: Solid continuous masses of igneous, metamorphic, or sedimentary rock that, in the opinion of the Engineer, cannot be removed without drilling, wedging, prying, sawing, jacking, hydraulic hammering or blasting. Soft, disintegrated rock that is capable of being excavated with rippers, picking, or other scarifying action is not considered solid rock.

2. Loose Rock: Individual boulders or loose stones, each with volume in excess of 1 cu yd. Excavated boulders or rock fragments with a volume of less than 1 cu yd shall not be classified as Loose Rock for payment purposes.

2.07 HOMEOWNER O&M TRAINING

A. The contractor shall provide a printed binder of O&M manuals for all equipment and components installed under this contract. Manuals shall consist of:

1. Product manuals including installation, operation, and O&M tasks including regular maintenance tasks, troubleshooting issues, and replacement parts.

B. The contractor shall provide onsite O&M training and equipment walk-through to the homeowner to include:

1. Regular use of the system and standard operating procedures.

2. Troubleshooting if the system is not functioning as it is supposed to.

C. Contractor shall schedule the homeowner O&M training walkthrough with the homeowner and project manager / engineer.

2.08 PROJECT COMPLETION TIME

A. The period of performance (POP) is 45 calendar days from issuance of the Notice to

Proceed (NTP).
