About this file

This is a Drug-Free Workplace Form required by the Hillsborough County Board of County Commissioners for the Carpet, Luxury Vinyl and Tile Floor Cleaning contract opportunity in Florida. The form certifies vendor compliance with Florida Statute 287.087 and establishes mandatory drug-free workplace requirements for contractors bidding on county services. The contract, valued at $1,332,869.50, covers carpet, custom Cuban cement tile, and luxury vinyl tile plank cleaning at various County facilities over a four-year term. Vendors must submit this completed and signed certification form as part of their bid package to be considered for award.

The Drug-Free Workplace Form requires vendors to publish workplace policies prohibiting controlled substance use, inform employees of drug abuse dangers and available assistance programs, distribute the policy statement to all employees working on the contract, mandate employee notification of drug-related convictions within five days, impose sanctions or require rehabilitation participation for violators, and maintain good faith efforts toward workplace drug-free compliance. This form is a mandatory compliance document and must be executed by an authorized representative of the bidding firm. The contract is designated as a Small Business Enterprise (SBE) Set-Aside, restricting eligibility to qualified small businesses.
