About this file
This is a contractor prequalification renewal document issued by the State of Utah Division of Facilities Construction and Management (DFCM) for Stage I prequalified contractors whose certifications are expiring within the next 60 days. The renewal packet applies to contractors prequalified in the disciplines of Electrical, General, Mechanical, Paving, Roofing, Site/Civil, and Hazmat. Contractors must submit their renewal documents to dfcm_vbs@utah.gov to maintain eligibility to bid on Stage II prequalified solicitations. The document requires submission of company information, identification of an authorized representative with legal signature authority, and active contractor's licenses for construction contractors.
The renewal process requires contractors to certify their continued status as responsible entities capable of performing DFCM service contracts, maintain financial solvency with adequate resources to meet contractual obligations, limit bidding to work they can perform, disclose any changes in ownership or significant leadership changes, confirm compliance with E-Verify employment authorization requirements, and attest to the absence of debarment, suspension, ineligibility, or voluntary exclusion from any government agency. Contractors unable to certify to any statement must provide written explanation. DFCM reserves the right to require any or all contractors to reapply for Stage II prequalification at any time, and incomplete submissions without active contractor's licenses will not be accepted.
