About this file
This document is an Arizona Department of Transportation (ADOT) Forced Labor of Ethnic Uyghurs Ban attachment, which is a compliance certification form related to state procurement requirements. The document pertains to Arizona state law (A.R.S. § 35-394) governing contracts between public entities and companies, specifically addressing restrictions on business interactions involving forced labor of ethnic Uyghurs in the People's Republic of China.
The attachment requires companies seeking to contract with the state to certify that they do not use forced labor of ethnic Uyghurs, do not use goods or services produced by such labor, and do not engage contractors, subcontractors, or suppliers who utilize such labor. In this specific instance, the document is signed by Courtesy Auto of Mesa, located at 1630 E. Auto Park Dr. in Mesa, Arizona, with the signatory being Doyle Buckmaster, Service Director. The form allows for exemptions for sole proprietorships, companies with fewer than ten employees, and non-profit organizations, providing flexibility for smaller business entities in meeting the state's procurement requirements.
