Summarize the scope and deliverables provided, including timeline and outcome. Include similar services performed s those required by this solicitation. State the name(s) of any of Offeror’s key personnel proposed in Attachment 6, who worked on this project:

The Pinedale Estates Domestic Water Improvement District (PEDWID) is a public water system south of Pinedale, in Navajo County, Arizona. Pinedale Estates is an unincorporated community approximately 5 miles southeast of Pinedale along Pinedale Road/Rim

Roade near State Route 260. PEDWID (AZ04-09040) was formed in 2017 having previously been a privately owned water system, known as Sitegreaves Water Company, that was co-regulated by ADEQ and the Arizona Corporation Commission (ACC).

The water system presently serves a population of 208 people through 85 service connections from one Main Well, with new connections pending from a recently obtained 2nd well (Dog House well). The Main Well has EPA exceedances for selenium.

Therefore, the DWID has elected to install a new well in a location where previous research indicates the groundwater likely will not exceed the MCL for selenium. Greg Carlson Engineering prepared plans and permitted (ATC) the new well, prepared the bid documents, and administered the bid. The new well has been drilled and the NSA results indicate that no treatment for EPA regulated contaminants is necessary.

In addition, Greg Carlson Engineering prepared construction plans for approximately 2,700 lineal feet of new 6-inch PVC waterline to connect the new well to the system. Greg Carlson was the Project Manager/Engineer and Juan Feria was the Design Draftsman.
