About this file
This document is an OMES (Office of Management and Enterprise Services) Form CP 076, which is a "Certification for Competitive Bid and Contract" that must be submitted along with a response to a state solicitation. The form requests general information about the bidder, including whether the bidder is currently engaged in a boycott of goods or services from Israel, whether the bidder is registered with the Oklahoma Secretary of State, and whether the bidder has Workers' Compensation Insurance Coverage. The form also asks if the bidder qualifies as a Disabled Veteran Business Enterprise.
The document outlines the requirements and certifications necessary for a vendor to submit a bid for a state and local contract opportunity in Oklahoma. It does not provide details about the specific products or services being procured, the project overview, quantities, response dates, or contract term. The document is focused on the administrative and compliance requirements for bidders rather than the substantive details of the contract opportunity.
