About this file

This document is an Affirmation and Disclosure Form (Invitation No. 097a-23) from a contractor based in Tigard, Oregon, addressing compliance with state procurement requirements, specifically an Executive Order prohibiting services from Russian companies. The form is associated with the Ohio Department of Transportation (ODOT) and requires detailed disclosure of the contractor's business locations, service performance locations, and data storage sites.

The contractor, with a principal business location at 15615 SW 74th Ave, Suite 100 in Tigard, OR, indicates that services will be performed at 27605 Little Mack Ave in St. Clair Shores, MI, and state data will be located at 8407 Laurel Fair Circle, Suite 400 in Tampa, FL. The form emphasizes strict compliance with regulations that prohibit offshore service performance, particularly in Russia, and requires immediate notification of any changes in service or data location. No specific contract value, goods, or services are detailed in this document, which serves primarily as a compliance and location disclosure instrument for a potential state contract.
