About this file

This document is an Affirmation and Disclosure Form, revised in March 2022, designed to ensure compliance with Executive Orders regarding offshore services and data location restrictions. The form requires contractors to disclose the principal business locations, service performance locations, and data storage locations for both the primary contractor and any potential subcontractors, with an emphasis on preventing services, data storage, or purchases from Russian institutions or companies.

The form mandates that contractors must immediately notify the State of any changes in service or data location before, during, or after contract execution. Failure to provide complete location information may result in contract non-award. The document includes provisions allowing the State to immediately terminate the contract if services are performed outside the United States without a formal waiver. By signing, the contractor affirms understanding these requirements and acknowledges that this form is incorporated into any potential state contract, creating a binding obligation to maintain transparency about service and data locations.
