About this file

This document is Attachment C to a Request for Proposal (RFP) for the Center of Early Education and Care (CEEC) Modular Building Procurement project at the University of Massachusetts Amherst in Massachusetts. The attachment outlines mandatory certifications that all contractors bidding on this public construction contract must complete and sign prior to submission. The certifications address four primary areas of compliance required under Massachusetts General Law.

Contractors must certify tax compliance with Massachusetts General Laws Chapter 62C, Section 49A(b), confirming adherence to all state tax laws, employee and contractor reporting requirements, and child support remittance obligations. Additionally, bidders must certify the bid is submitted in good faith without collusion or fraud with any other party, confirm they are not debarred from performing public construction work in Massachusetts under applicable state debarment provisions, and attest that all employees assigned to the project will have completed a minimum 10-hour Occupational Safety and Health Administration (OSHA)-approved construction safety and health course prior to beginning work. Contractors must provide documentation of employee course completion with the first certified payroll report for each worker. An authorized signatory must sign and date the certification form, including the contractor's printed name and title.
