About this file

This is an Exhibit D Subcontractor Inclusion Plan form for Competitive Solicitation No. 02926 – Nutritionals & Supplements, issued by the State of Washington's Enterprise Services division. The solicitation concerns the purchase of nutritional and dietary food products and dietary supplements, including powdered and liquid formulas for essential and non-essential nutrients, vitamins, and dietary management products designed for newborns, the elderly, and individuals with medical conditions requiring specific nutritional supplementation. The document serves as a mandatory submission requirement only for bidders who intend to utilize subcontractors if awarded a contract; bidders planning to perform all contract work independently are not required to submit this exhibit.

The Subcontractor Inclusion Plan requires bidders to identify anticipated percentages of contract spend across multiple subcontractor categories, including OMWBE-certified minority-owned, woman-owned, combination-owned, minority woman-owned, and socially and economically disadvantaged businesses, as well as certified veteran-owned businesses and Washington small businesses. Bidders must list specific subcontractor names, Federal Tax Identification Numbers, and commit to genuine outreach efforts targeting small, veteran, and diverse businesses registered with Washington's Electronic Business Solution (WEBS). The document emphasizes that small and veteran-owned business participation goals are voluntary with no minimum participation requirements or preference scoring based on subcontractor diversity commitments; however, awarded bidders failing to submit an Inclusion Plan may be precluded from using subcontractors and must report on their outreach efforts during annual contract management meetings. The plan must be submitted to Enterprise Services' Procurement Coordinator at DesContractsTeamFir@des.wa.gov.
