About this file

This is an Exhibit C Market Basket spreadsheet for Washington State's Nutritionals and Supplements RFI, establishing evaluation criteria for nutritional and dietary food products including powdered and liquid formulas, vitamins, and dietary supplements designed for newborns, elderly individuals, and patients with medical nutritional deficiencies. The market basket comprises five primary product categories: Oral Nutrition Supplements for Adults (standard, high-calorie/high-protein, disease-specific, and clear liquid options), Infant & Pediatric Formulas (standard, premature/transitional, hypoallergenic/amino acid, and metabolic formulas), Enteral Nutrition products for tube feeding (standard and specialized formulas), Nutrition Modulars & Therapeutic Support Products (protein modulars, fiber supports, wound healing nutrition, and dysphagia management), and Medical Foods for inherited metabolic disorders. The solicitation requires bidders to submit pricing on desired line items with minimum flavor requirements specified for most products, with asterisk-marked items evaluated on lowest price only within their subcategory.

Bidders must complete all applicable columns including manufacturer information, brand, SKU number, price per case, and unit of measure per case, with pricing reflecting generic brand or better quality and including all transportation costs to the distribution point and bidder rebates. The evaluation methodology assigns point values to each line item based on expected purchase frequency, with final bidder cost calculated by dividing the price per case by the units of measure per case to determine comparable pricing across different package sizes and substitutes. Additional scoring factors include Minimum Order Threshold (25 lbs minimum acceptable down to 0 lbs full value), Minimum Free Shipping Threshold (350 lbs to 0 lbs), Emergency Shipping capability (4 days acceptable to 1 day full value), Regular Shipping timeframe (14 days to 2 days), and High Volume Discount provisions requiring bidders to specify minimum order size in cases and applicable percentage discounts. Substitute products must be approved by the state and must serve the same medical purpose with comparable nutrients, while all pricing is subject to economic price adjustments per the contract terms.

COLUMNColumn TitleDefinition
AProduct DescriptionName of product. If you do not offer the listed brand of product, but can provide a comparable product of a different brand, provide a bid price on the "Substitute Product" column. Provide details of your comparable product in the column D "Substitute Product."

For products marked with *. Within that subcategory only the lowest priced product of the * marked products will be evaluated, if you can provide both on contract, bid on both.

BPoint ValueThe points towards total evaluation score that this product is worth. The point value is derived from expected purchases, items that are expected to be purchased more frequently are worth more points. Point values are rounded to the nearest whole number to make evaluation and scoring more efficient.
CPack SizePacking and Size of Product. If you sell this product in a different pack size, list the product in column D "Substitute Product" with the different pack size in column E "Pack Size of Substitute."
DSubstitute ProductDescription of product that is similar to the one listed in Column A, "Product Description" that will serve the same medical purpose, and contains comparable nutrients. This column will be evaluated as substitute for the listed product. If you are bidding on the listed product, leave this column blank. Substitutes will be reviewed and approved/denied by DES.
EPack Size of SubstituteProvide the pack/case size that you prefer to sell your comparable product in. If it comes in cases of multiple packs (ex. 6-8oz shakes/box, 6 boxes/case=total 36-8 oz. shakes) please provide that information. DES will calculate the price per ounce (or other metric) to evaluate all bids evenly. This column should be left blank if not bidding a "Substitute Product."
FFlavorsMinimum number of flavors required for this product listed in product description, in this column list all flavors you can provide at price listed in column P, "Unit Price." Flavors listed will be added to Exhibit B contract - Exhibit B "Prices for Included Goods/Services."
GBidder Item No.Bidder item number for product
HManufacturerOriginal Manufacturer name
IBrandBrand of Origin
JManufacturer SKU #SKU # given by Manufacturer
KPrice Per Case (1)The actual invoice price of a product for one case of a product that the Bidder agrees to provide to purchasers delivered (not including applicable shipping fees listed in "Additional Costs" or sales tax). This price includes any transportation costs necessary for delivery to the distribution point. This price includes all Bidder rebates, allowances and incentives (or any other term used for price reductions) other than high volume discount listed in "Additional Costs". This cost will be subject to Economic Price adjustments per Exhibit B contract - Section 3.3 "Economic Adjustment."
LUnit of Measure(UOM) Per Case (Net Weight)Ex: Oz. per case, 8oz drink with 24/CS=192. Use the same unit of measure the pack size for this line item is measured in, (liters, liquid ounces, pounds, grams) even if you are providing a substitute product that uses a different UOM. This is required to determine final evaluation price.
MDefault UOM Per CaseThis is the default UOM Per Case based on the Pack Size (Column C). Ex. 8oz drink with 24 drinks per case = 192.
NFinal Bidder CostThe final Bidder Cost will be calculated by taking the price for one case of a product and dividing it by the UOM per case. This column will auto populate. The formula utilizes the Price Per Case (Column K) divided by the Default UOM Per Case (Column M) IF UOM Per Case (Column L) is blank. If UOM Per Case (Column L) has an entry the formula will divide by this value instead. This is the evaluated price.
