About this file
This is a Bid Cover Sheet for Invitation to Bid (ITB) #S-73000-00017840 issued by the Oregon Department of Transportation for janitorial services at the Prineville DMV facility. The document serves as the standard submission form for qualified contractors interested in providing janitorial services. The cover sheet requires bidders to provide comprehensive company information, including legal business name, DUNS number, business structure, physical and mailing addresses, and primary contacts for bid and contract administration. Bidders must document their experience and qualifications, including number of employees, annual sales, years in business providing comparable services, and Construction Contractors Board License information if applicable. The form specifies that all pricing submitted on the Price Form will remain effective for a minimum of one year following award.
The Bid Cover Sheet mandates extensive certifications and compliance confirmations from bidders, including verification that the firm has reviewed all addenda, accepts all terms and conditions in the ITB and Sample Agreement, and complies with Oregon employment discrimination laws and equal employment opportunity policies. Bidders must certify they are not debarred or suspended from federal or state contracts, have no active exclusions on the System for Award Management (SAM), and have not been convicted of fraud, antitrust violations, embezzlement, or related offenses within the past three years. Additional requirements include conflict of interest disclosures, independent contractor status verification, tax law compliance, and implementation of written policies preventing sexual harassment and discrimination for contracts with anticipated values of $150,000 or more. The form requires submission of three references for comparable projects in size and scope, with contact information and project details for each reference firm.
