About this file
This is a General Bid Attachment form for a state and local contract opportunity in Massachusetts involving interior plaster rehabilitation and painting work at a memorial building. The project scope includes rehabilitation of lath and plaster with repairs to ceilings, soffits, coffers, and walls in the South Parlor and G.A.R Hall, followed by priming and painting of all surfaces in these areas. Additionally, the project includes demolition of existing acoustical ceiling tile in the kitchen area and installation of a new acoustical ceiling system. The bid attachment identifies unit price line items for plaster repairs, specifically: Plaster Area Repair (10 square feet), Crack in Plaster Repair (5 linear feet), and Hairline Crack in Plaster Repair (5 linear feet). The contractor must submit unit prices for each item, which will be carried from selected filed sub-bidders and incorporated into the Schedule of Values as part of the final contract amount.
The selected contractor must execute the contract and furnish Performance and Payment Bonds, each equal to 100% of the Contract Price within five business days of presentation by the Awarding Authority, with bond premiums included in the Contract Price. The contractor certifies that all employees will have completed a minimum 10-hour OSHA-approved construction safety and health course prior to beginning work. Bid security becomes the property of the Owner as liquidated damages if the contract and bonds are not executed within the specified timeframe. The contractor must also certify non-debarment status under Massachusetts General Laws Chapter 29, Section 29F. If estimated quantities are not fully utilized, the unused portion shall be credited to the Owner via change order; conversely, any additional quantities beyond those listed shall be reimbursed at the established unit price value.
