Description
Comprehensive high-impact HIV prevention programs for community-based organizations - basic nwfl, INC. (basic) is a volunteer governed, community-based organization that provides HIV prevention/education and testing, hepatitis prevention and testing, and substance use disorder education to the citizens in the six-county region of the central panhandle of florida (BAY, calhoun, gulf, holmes, jackson, and washington counties). Since its inception in 1989, basic has been recognized by the community as a leader in providing services to vulnerable and disenfranchised populations that have been disproportionately impacted by the HIV/aids epidemic. Basic is the only comprehensive HIV/aids organization in the region that provides a full continuum of HIV/aids services including evidence-based HIV prevention interventions, HIV testing, care/treatment by established referral arrangements, and essential support services including housing assistance for plwh. As the lead agency for coordination of all services under ryan white part b program since 1992, basic provides case management services to approximately 300 plwh annually. The organization sponsors multiple HIV prevention programs, including high impact prevention (HIP) and linkage to care, reaching over 5,700 people each year. Proposed projectin florida, IT is estimated that 12.8% of residents are unaware of their HIV status (state of florida HIV prevention and care plan: 2017-2021, fdoh, 2016). The epidemic is profoundly impacting the african american population, who represent over half (51.6%) of cases in the service area (fdoh, 2018). Based on its strong foundation of experience, expertise in HIV prevention, and extensive community partnerships, basic is well equipped to lead local efforts to end the HIV epidemic by coordinating and implementing community-based strategies to diagnose HIV as early as possible; treat HIV quickly and effectively; prevent new HIV cases; and respond quickly to HIV clusters. Basic will implement several HIP strategies to support the PS21-2102 project: art as, partnership for health ? Medication adherence, taking care of me, promise, sister to sister, HIV testing in non-clinical settings, and social network strategy. Funding will support key staff members, supplies including testing materials, travel, other core program and operational expenses, and a subcontract with pancare of florida, INC., the fqhc partner, to include additional testing, integrated screening, a large-scale testing event, and other prevention and treatment activities. Purposedriven by the guiding philosophy "compassion without judgment," the purpose of this project is to promote optimal health outcomes for people living with HIV (plwh) by increasing access to HIV prevention and care services via high-impact prevention programs, and reducing HIV/aids-related health disparities, with the overall goal of ending the HIV epidemic. To reach those most in need of diagnosis, treatment, and prevention services, and to complement existing programs already reaching those age 13 to 24, basic will target the program to high risk, african american, heterosexual individuals who are age 30 and older. Outcomesbasic has established outcomes that align with the logic model presented in the approach section of the CDC project description. Short-term outcomes to be achieved by the end of the project period are as follows:diagnose outcomes: increased persons who are aware of their HIV status; increased receipt of integrated screenings.treat outcomes: increased receipt of HIV medical care and art among persons with newly diagnosed HIV; increased receipt of HIV medical care and art among persons with previously diagnosed HIV, not-in-care; increased access to partner services.prevent outcomes: increased access to prep; increased availability of condoms. Respond outcome: increased health department and community engagement for cluster detection and response.
