Description
High-impact HIV prevention and surveillance programs for health departments - the proposed program will support both the new york state ending the epidemic initiative, in alignment with ending the HIV epidemic goals, and the national HIV/aids strategy through a comprehensive HIV prevention and surveillance program that prioritizes a whole-person approach addressing health equity and social determinants of health. The program will prevent new HIV infections and improve the health of people with HIV. The primary objectives of the program are to increase knowledge of HIV status, reduce HIV transmission, prevent new HIV infections, improve linkage to care and viral suppression, and maintain elimination of perinatal transmission. The strategies employed will result in diagnosing people with HIV as early as possible and identifying previously diagnosed persons to link them to care to achieve sustained viral load suppression, the prevention of new HIV transmissions by using proven interventions, including condom distribution, pre-exposure prophylaxis (prep), postexposure prophylaxis (PEP), and syringe services programs (SSP), and rapidly responding to potential HIV outbreaks.
