About this program

The Department of Health and Human Services Health Resources and Services Administration (HRSA) funds the development of new, accredited rural physician residency programs through CFDA 93.746, with the most recent grant opportunity (HRSA-27-046) posted on August 7, 2026.

The program provides start-up funding to establish sustainable rural residency programs in qualifying medical specialties: family medicine, internal medicine, psychiatry, general surgery, obstetrics and gynecology, and preventive medicine. Funded residency programs must train residents in clinical settings located in rural areas as defined by HRSA's Federal Office of Rural Health Policy for more than 50 percent of their total residency time, with the goal of increasing physician workforce capacity in underserved rural communities. Long-term program sustainability must be supported by viable sources such as Medicare, Medicaid, or other public or private funding; HRSA provides only start-up capital.

The program offers two distinct tracks: Rural Residency Planning and Development (RRPD) awards for new program development, which are not renewable, and Rural Residency Planning and Development Technical Assistance awards, which support up to 5-year project periods and may be renewed non-competitively after initial funding contingent on satisfactory performance, available appropriations, and approved continuation applications.

Eligible applicants include domestic organizations across a broad range of entity types: higher education institutions (public and private), state and local governments, tribal governments and organizations, 501(c)(3) and non-501(c)(3) nonprofits, for-profit organizations, small businesses, and faith-based organizations. Domestic eligibility encompasses the 50 states, the District of Columbia, Puerto Rico, and insular possessions including Guam, the U.S. Virgin Islands, American Samoa, the Northern Mariana Islands, the Federated States of Micronesia, the Republic of the Marshall Islands, and the Republic of Palau.

Applications are submitted electronically through Grants.gov and evaluated by an objective review committee; final funding decisions rest with the HRSA program official. The opportunity posted in August 2026 (HRSA-27-046) represents active solicitation under this program.
