About this opportunity
The Health Resources and Services Administration is soliciting applications to develop new, accredited, sustainable rural physician residency programs, with applications due July 8, 2026 at 11:59 p.m. Eastern Time. HRSA expects to award approximately 15 grants totaling $11.25 million, at up to $750,000 per award, with funding distributed fully in Year 1 for use over a three-year performance period from August 1, 2026 through July 31, 2029.
Eligible applicants include state and local governments, institutions of higher education (public and private), nonprofit organizations (501(c)(3) and non-501(c)(3)), tribal governments (federally recognized and non-federally recognized), small businesses, for-profit organizations, and special district governments. Applicants must develop residency programs in one or more of six qualifying medical specialties: family medicine, internal medicine, preventive medicine, psychiatry, general surgery, and obstetrics and gynecology.
Residency programs must train residents with greater than 50 percent of clinical training at rural sites as defined by HRSA's Federal Office of Rural Health Policy and achieve Accreditation Council for Graduate Medical Education accreditation by the end of the performance period. Funding covers start-up costs only; recipients must secure long-term sustainability through Medicare Graduate Medical Education payments, state Medicaid, private funding, or combinations thereof.
Merit review employs a 100-point scoring system: need (15 points), response approach (30 points), performance reporting and evaluation (10 points), impact and sustainability (25 points), resources and capabilities (15 points), and reasonableness of budget request (5 points). Recipients must collaborate with the HRSA-funded RRPD Technical Assistance Center, attend annual program meetings, track resident outcomes for at least five years post-graduation, and submit quarterly progress and annual performance reports.
CFDA 93.746.

The RRPD program provides start-up funding to create new rural residency programs in qualifying medical specialties. Long-term sustainability funding must come from viable and stable sources, such as Medicare, Medicaid, and other public or private sources. Qualifying medical specialties are:Family medicine.Internal medicine.Preventive medicine.Psychiatry.General surgery.Obstetrics and gynecology. For this notice of funding opportunity (NOFO), rural residency programs:Are accredited physician residency programs.Train residents in clinical training sites that are physically located in a rural area as defined by HRSA"s Federal Office of Rural Health Policy (FORHP) for greater than 50 percent of their total time in residency.Focus on preparing physicians to practice in rural communities.
