About this program
The Department of Health and Human Services Immediate Office of the Secretary (CFDA 93.012) funds comprehensive women's heart health care programs targeting high-risk populations, with the most recent grant awards disbursed in November 2005.
This program is inactive. No grant awards have been made in the past twenty years. The portfolio comprises six Block Grants totaling $539 thousand in federal dollars, all awarded in 2005. The funding cadence was six awards in 2005 alone, with no subsequent activity recorded.
The program funds enhancement of existing women's heart health care services in hospitals, clinics, and health centers serving high-risk women—specifically those aged 60 and older, racial and ethnic minority women, and women in rural communities. Funded programs must integrate five interrelated service components: Education and Awareness, Screening and Risk Assessment, Diagnostic Testing and Treatment, Lifestyle Modification and Rehabilitation, and Tracking and Evaluation. The underlying goal is to reduce heart disease mortality and morbidity among women by demonstrating whether comprehensive, women-centered, culturally competent, multidisciplinary, and continuous care improves outcomes. All awarded grants have been Block Grants.
The Department of Health and Human Services Immediate Office of the Secretary has funded 100 percent of awards under this program. Individual grant awards in the 2005 cohort ranged from approximately $89 thousand to $90 thousand. Representative awards include Heart Health in Action ($90 thousand, awarded September 2005), Enhanced Women's Heart Advantage ($90 thousand, awarded September 2005), and Heart of a Woman ($89,425, awarded November 2005).
No recent grant opportunities are present in the source. The program shows no active solicitation or ongoing funding activity.
Objective and description
- Objective
To provide funding to improve and enhance existing women's heart health care programs in hospitals, clinics, and/or health centers and to enable the programs to track and evaluate outcome data. Each grantee shall enhance an existing women's heart health care program so that it provides a continuum of heart health care services through the integration of the following five interrelated components: Education and Awareness, Screening and Risk Assessment, Diagnostic Testing and Treatment, Lifestyle Modification and Rehabilitation, and Tracking and Evaluation. Grantees shall also target high-risk women in at least one of the following groups: women aged 60 years or older, racial and ethnic minority women, and/or women who live in rural communities. These programs will offer comprehensive heart health care services that are women-centered, culturally competent, multi-disciplinary, continuous and integrated. The goal of these programs will be to reduce heart disease mortality and morbidity among women and to increase the number of high-risk women who receive quality heart health care, including education, prevention, screening, diagnosis, treatment and rehabilitation. These programs will be demonstration projects; as such, they will provide the evidence necessary to evaluate whether comprehensive women's heart health care programs are effective in improving heart disease outcomes in high-risk women.
