The scope includes preliminary medical evaluation screenings of new employees during onboarding, conducted by state-licensed physicians, physician assistants, or nurse practitioners. Evaluations must comply with MIOSHA General Industry and Construction Safety and Health Standard Part 451 and 29 CFR 1910.134 (OSHA Respirator Medical Evaluation Questionnaire framework). The contractor is not responsible for fit testing. Anticipated usage through May 31, 2026, includes 313 respiratory health questionnaires at MVHDJJ, 2 pulmonary function testing days at MVHCT, and 148 respiratory health questionnaires at MVHGR. All activities must be delivered within two business days of order receipt. The contractor must designate a dedicated Program Manager available 8:00 a.m. to 4:30 p.m., Monday through Friday, maintain HIPAA-compliant questionnaire receipt methods, and comply with ADA accessibility standards for digital activities.

The contract term runs October 26, 2026, through October 31, 2029, with options for two additional one-year renewals at the State's discretion. Pricing is firm for the entire contract term. Payment is through Electronic Funds Transfer within 45 days of invoice receipt. The contractor must maintain Commercial General Liability ($1 million each occurrence, $2 million aggregate), Umbrella/Excess Liability ($5 million each occurrence, $10 million aggregate), Automobile Liability ($1 million per accident), Workers Compensation per law, Employers Liability ($500,000 each accident and disease), and Medical Malpractice Insurance ($200,000 each occurrence, $600,000 annual aggregate). Federal provisions apply, including Byrd Anti-Lobbying Amendment certification (for contracts exceeding $100,000), recovery materials procurement, domestic preference for U.S.-produced goods, and affirmative steps to use small and minority businesses as subcontractors where possible. Proposals must be submitted electronically through michigan.gov/SIGMAVSS in modifiable native formats. The Solicitation Manager is Kerri Bielski (BielskiK@michigan.gov, 1-855-647-8724).
