About this opportunity

The Maryland Department of Health, Center for Cancer Prevention and Control (CCPC) is requesting applications to fund implementation of evidence-based interventions and quality improvement methods to increase colorectal cancer screening rates in Federally Qualified Health Centers and primary care clinics, with applications due November 2, 2026 via eMaryland Marketplace Advantage (eMMA). Up to six awards will be made; no single award exceeds $140,000 annually.

Eligible applicants are nonprofit organizations classified as tax-exempt under Section 501(c)(3), state or local government agencies, or public colleges or universities, and must be registered on eMMA. The scope targets clinics with current colorectal cancer screening rates below 60% serving populations aged 45 to 75 years. Year 1 spans April 1, 2026 through June 30, 2026 (three months), with four optional one-year renewal periods extending through June 30, 2030. Applicants must select two primary care clinics for Year 1 participation, though this requirement may be waived if applicants subcontract with partner organizations such as other FQHCs, community health centers, or hospitals to meet the two-site minimum. Additional clinics are added in subsequent option years until all clinics with screening rates below 60% participate by July 1, 2028.

Budget calculations are based on $8 per eligible patient aged 45 to 75 years, with Year 1 budgets limited to 80% of the annual amount. Indirect costs may not exceed 10% of total direct costs. Funds are restricted from food, lobbying, medical equipment, direct clinical services, and promotional materials. Required deliverables include baseline clinic data, readiness assessments, process maps, monthly expenditure trackers, quarterly invoices, internal data validation reviews, annual clinic data, end-of-year evaluation reports, and equity assessments in final option years. Applicants must maintain commercial general liability insurance of at least $1 million combined single limit and Worker's Compensation insurance of $1 million per occurrence. Project Narrative and Budget Narrative are evaluated with equal weighting on technical and financial factors. Written questions are due February 4, 2026 at 2:00 p.m. EST.
