About this file
This is a Vendor Questions and Answers document issued by the Maryland Department of Health, Center for Cancer Prevention and Control (CCPC) for the Maryland Colorectal Cancer Control Program (MCRCCP), solicitation eMMA# PHPA 3771/BPM054917. The RFA seeks applications from Federally Qualified Health Centers (FQHCs), primary care clinics, and health systems to implement evidence-based interventions and quality improvement methods to increase colorectal cancer screening rates for populations aged 45 to 75 years in clinics with current screening rates below 60%. The initial program year runs from April 1, 2026 through June 30, 2026, with the full grant period extending through June 30, 2030. Selected applicants will participate in Year 1 and subsequent years as part of a multi-year award. If insufficient applications are received in the first round, the RFA will be re-posted for future CDC Program Years 2-5.
The program is structured as a multi-year grant requiring annual continuing applications from awarded grantees, which involve updating workplans and budgets based on any budgetary changes. Regarding eligibility, the standard requirement of two primary care clinics for Year 1 may be waived; applicants with only one primary care clinic may still submit proposals if they subcontract with partner organizations such as other FQHCs, community health centers, primary care organizations, or hospitals to meet the two-site requirement for the initial year, with provisions for additional clinics in subsequent years. No pricing information, set-asides, incumbent information, or specific funding amounts are detailed in this Q&A document.

Request for Applications eMMA#: PHPA 3771/BPM054917
Issue Date: January 14, 2026
Vendor Questions and Answers #1 Date: February 9, 2026
Question #1: As we apply to the PHPA 3771/BPM054917 solicitation, how do we know:
A. If we will be able to participate in Year 1 (April 1, 2026—June 30, 2026) or we will be considered for the out years?
Answer: Yes. If your application is selected for this RFA, you will participate in Year 1 (one) and subsequent years. If we do not get enough applications for this first round, we plan to re-post the RFA for future years, i.e. CDC Program Years 2-5.
B. If we are fortunate to receive the grant, we might consider this a multi-year grant? (or we will have the opportunity to apply every year?)
Answer: Yes, this is a multi-year grant that goes through June 30, 2030. We require the already awarded grantees to submit an annual continuing application (much shorter than the RFA application), which entails updating the workplan and budget (based on any budgetary changes we may have).
Question #2: we are a health center with a single site serving 8,160 patients. We do not meet the most basic criteria, which is having two primary care clinics for year 1. Does this disqualify us, or could we apply and subcontract to partner with another health center (FQHC, community health center, primary care organization, or hospital) so that, between us, we equal 2 sites for the first year, additional clinics for subsequent years, etc.
Answer: We will permit your proposal if you only have one primary care clinic
