About this opportunity

The Colorado Department of Human Services, Behavioral Health Administration is requesting proposals for Projects for Assistance in Transition from Homelessness (PATH) services statewide under RFP IBEH 2027000057, with proposals due October 29, 2026, at 2:00 PM Mountain Time. This is a federally funded SAMHSA grant requiring a mandatory 36% non-federal match in cash or in-kind contributions spent concurrently with federal funds.

Eligible applicants are political subdivisions of Colorado, Indian tribes, private nonprofit organizations, community-based nonprofit organizations, and veterans organizations. The maximum award is $500,000 per year per offeror. The contract runs from July 1, 2027, through June 30, 2028, with potential renewal for four additional one-year periods at the State's sole discretion.

The scope includes street and in-reach outreach, case management, behavioral health services, housing-related supportive services, and referrals for adults aged 18 and older experiencing homelessness or at imminent risk who have serious mental illness, including those with co-occurring substance use disorders. Minimum 50% of program hours must be dedicated to outreach and engagement. Housing services are capped at 20% of the budget, and food expenses are limited to $10 per person per day. All program data must enter the Homeless Management Information System (HMIS) and quarterly reports are due to the PATH Data Exchange (PDX) by the 30th day following each quarter end.

Contractors must comply with HIPAA requirements, maintain a Continuity of Operations Plan, submit monthly invoices by the 20th of the following month, provide required federal compliance documentation including System for Award Management (SAM) registration, and attend bimonthly BHA-facilitated meetings. Performance outcome measures include contacting individuals at planned levels, enrolling no less than 57% of eligible individuals, and ensuring at least 64% of enrolled clients receive community mental health services.

Evaluation is on a 100-point scale: outreach capacity (20%), community need and service delivery approach (5%), target population experience (15%), service delivery model (15%), community partnerships (10%), organizational capacity (10%), staffing qualifications (10%), data management (5%), federal/state program experience (5%), and cost proposal (5%). A pre-bid conference is September 29, 2026, from 12:00–1:00 PM Mountain Time via Google Meet. Written questions are due October 5, 2026, at 5:00 PM.
