About this opportunity

The County of San Mateo Health Administration is requesting proposals for an Enterprise Master Patient Index (EMPI) solution to replace its current Rhapsody system, with proposals due July 24, 2026. The scope requires a next-generation platform delivering significantly higher patient matching accuracy through referential matching and machine learning, supporting FHIR interoperability standards, and enabling migration to a scalable, cloud-based environment with reduced reliance on manual data stewardship.

The solution must integrate seamlessly with existing clinical and administrative systems across San Mateo County Health's 10 divisions—Administration & Central Finance, Aging & Disability Services, Behavioral Health & Recovery Services, Correctional Health, Emergency Medical Services, Environmental Health, Family Health, Health Information Technology, Public Health Policy & Planning, and San Mateo Medical Center. Key requirements include HIPAA compliance certification, data encryption standards, Epic EHR integration capability, FHIR R4 API documentation, role-based access controls, audit trail capabilities, monitoring and alerting functionality, failover and redundancy features, and Service Level Agreements with defined incident prioritization and response times. The vendor must provide detailed acceptance testing procedures, post-implementation support services, and training delivery. Customer data must remain within the United States. The supplemental questionnaire addresses experience migrating from Rhapsody, identity continuity mechanisms, implementation timelines, third-party product dependencies, and vendor engagement strategy for ongoing product development input.

San Mateo County Health's standard contract template for agreements exceeding $200,000 applies. The contractor must maintain workers' compensation and liability insurance with combined single limits of $1,000,000, comply with California prevailing wage requirements and the County's Living Wage Ordinance, and provide equal employment opportunity and non-discrimination assurances. All work products and data become County property upon completion or termination. Either party may terminate with 30 days' notice; the County may terminate for cause with a 10-day cure period following breach notification.

San Mateo County Health is seeking a next-generation Enterprise Master Person Index (EMPI) solution to modernize and strengthen its identity management capabilities across the organization. The primary objective is to replace the current system (Rhapsody) with a more advanced platform that delivers significantly higher patient matching accuracy through the use of modern techniques such as referential matching and machine learning, while also supporting contemporary interoperability standards like FHIR. A key focus is reducing the reliance on manual data stewardship by automating identity resolution processes, thereby improving operational efficiency, patient safety, and data integrity.

This initiative will also involve a comprehensive migration from the existing identity ecosystem, including the Rhapsody integration engine, to a more scalable, cloud-based, and cost-effective environment. The new solution is expected to provide seamless integration with existing clinical and administrative systems, enable real-time data exchange, and support a growing portfolio of digital health applications. Additionally, the upgraded EMPI will serve as a foundational component for enterprise analytics, population health initiatives, and coordinated care efforts by ensuring a consistent and accurate longitudinal patient record across all systems.
