About this file

This is a Request for Proposal document from the County of San Mateo Health Administration seeking an Enterprise Master Patient Index (EMPI) solution. The county's Health Administration comprises 10 divisions providing health and welfare services across San Mateo County, including Administration & Central Finance, Aging & Disability Services, Behavioral Health & Recovery Services, Correctional Health, Emergency Medical Services, Environmental Health, Family Health, Health Information Technology, Public Health Policy & Planning, and San Mateo Medical Center. The primary objective is to replace the current Rhapsody-based system with a next-generation EMPI platform that delivers significantly higher patient matching accuracy through modern techniques such as referential matching and machine learning while supporting contemporary interoperability standards including FHIR. The project scope includes a comprehensive migration from the existing identity ecosystem, including the Rhapsody integration engine, to a scalable, cloud-based, and cost-effective environment with seamless integration capabilities to existing clinical and administrative systems.

The new EMPI solution is intended to automate identity resolution processes to reduce manual data stewardship requirements, thereby improving operational efficiency, patient safety, and data integrity. The upgraded system will enable real-time data exchange, support a growing portfolio of digital health applications, and serve as a foundational component for enterprise analytics, population health initiatives, and coordinated care efforts by ensuring a consistent and accurate longitudinal patient record across all systems. The solution must support contemporary interoperability standards and cloud-based deployment while maintaining integration with San Mateo County Health's existing clinical and administrative infrastructure across all 10 divisions.

County of San Mateo

Request for Proposals (RFP) for Enterprise Master Patient

Index (EMPI)

Health - Health IT

RFP No. 2026-RFP-00458

RFP Contact: Thomas Collins tcollins@smcgov.org

1. Introduction and Schedule

The County of San Mateo (the “County” or “SMC”) covers most of the San Francisco Peninsula. The region covers 744 mi and is home to nearly 800,000 residents and 20 incorporated cities. The County provides for the health and welfare of all people within its borders and serves as the local government for the unincorporated areas. Innovation thrives here in industries including bioscience, computer software, green technology, hospitality, financial management, health care, education and transportation. The

County prides itself on how that prosperity fosters its commitment to protecting and enhancing the health, safety, welfare and natural resources of the community.

1.1. Summary

The County of San Mateo Health Administration is comprised of 10 Divisions providing health and welfare across San Mateo County including (a) Administration & Central Finance, (b) Aging & Disability

Services, (c) Behavioral Health & Recovery Services, (d) Correctional Health, (e) Emergency Medical

Services, (f) Environmental Health, (g) Family Health, (h) Health Information Technology, (i) Public

Health, Policy & Planning, (j) San Mateo Medical Center.

San Mateo County Health is seeking a next-generation Enterprise Master Person Index (EMPI) solution to modernize and strengthen its identity management capabilities across the organization. The primary objective is to replace the current system (Rhapsody) with a more advanced platform that delivers significantly higher patient matching accuracy through the use of modern techniques such as referential matching and machine learning, while also supporting contemporary interoperability standards like FHIR.

A key focus is reducing the reliance on manual data stewardship by automating identity resolution processes, thereby improving operational efficiency, patient safety, and data integrity.

This initiative will also involve a comprehensive migration from the existing identity ecosystem, including the Rhapsody integration engine, to a more scalable, cloud-based, and cost-effective environment. The new solution is expected to provide seamless integration with existing clinical and administrative systems, enable real-time data exchange, and support a growing portfolio of digital health applications. Additionally, the upgraded EMPI will serve as a foundational component for enterprise analytics, population health initiatives, and coordinated care efforts by ensuring a consistent and accurate longitudinal patient record across all systems.

1.2. Background

San Mateo County Health currently relies on the EMPI to manage approximately 765,000 patient records across multiple EMR systems, with “Golden Records” and Enterprise Unique Identifiers (EUIDs) forming the core of its patient-matching framework, supplemented by Epic’s Identity module. As part of its modernization strategy, the organization aims to move away from a legacy on-premises setup toward a more scalable, cloud-based architecture. A key priority is improving matching accuracy by incorporating referential matching or machine learning capabilities to reduce the volume of manual review required by the existing probabilistic and deterministic methods. Additionally, there is a need for more flexible, developer-friendly FHIR APIs to better support an expanding suite of digital health and patient-facing applications. Enhancing administrative efficiency is also important, particularly by making the data steward interface more intuitive for managing overlaps and merges. Ultimately, The County is seeking platform under a new contract, that will enable more advanced referential matching capabilities.

2. Scope of Work

2.1. Objectives

San Mateo County Health’s objectives center on implementing a seamless, low-risk migration of existing master patient records and “Golden Records” while preserving all cross-links across downstream systems such as the EHR, billing, and CRM platforms, and ensuring a unique EUID for every individual. A primary goal is to significantly improve data accuracy by reducing the duplicate rate from 8% to 1% through the use of advanced matching techniques, including probabilistic, deterministic, referential, or mailto:ndelsarto@smcgov.org tel:(650)502-2976 mailto:tcollins@smcgov.org tel:(628)258-3275

Page 4 of 24 Release date of June 8, 2026 machine learning algorithms. The new EMPI is expected to function as a vendor-neutral, system-agnostic hub that integrates effectively with both the EHR and the data warehouse. Operational efficiency is also a priority, with a target of automating at least 95% of current manual data stewardship tasks, thereby improving patient safety by reducing treatment errors caused by fragmented records and enhancing financial performance by minimizing denied claims and billing delays. Additionally, the solution should support advanced analytics by providing a clean, reliable data foundation for population health and enterprise reporting. Finally, it must be highly scalable—capable of handling at least 800,000 records with strong performance—and support real-time interoperability through bidirectional data exchange using HL7 FHIR standards.

Scope of Work:

Functionality: The EMPI will enable seamless, secure data exchange among Electronic Health Records

(EHR) and other third-party applications, improving interoperability among clinical and administrative applications, reducing manual data entry and errors, while ensuring compliance with regulatory requirements.
