About this opportunity

The San Francisco Department of Public Health is soliciting proposals for on-demand, non-emergency ambulance transportation services (RFP 11781) with proposals due July 20, 2026 at 2:00 PM PT and a pre-proposal conference scheduled for June 8, 2026 at 1:00 PM PT via Microsoft Teams. Local Business Enterprise rating bonuses of 10 percent for Small/Micro-LBEs and 5 percent for SBA-LBEs apply; no LBE subcontracting participation requirement is imposed.

The scope encompasses Basic Life Support (BLS), Advanced Life Support (ALS), and Critical Care Transport (CCT) services for patient transfers, discharges, and medically necessary appointments across Zuckerberg San Francisco General Hospital, Laguna Honda Hospital, and other DPH units. Proposers must meet two pass-fail minimum qualifications: at least ten years of continuous experience providing non-emergency ambulance transportation services to healthcare entities with third-party documentation, and a valid San Francisco Certificate of Operation with associated ambulance permits. All proposals must include all-inclusive per-ride rates covering labor, materials, certifications, and operational costs. The cost structure allocates 52 percent to BLS, 10 percent to ALS, 19 percent to specialty/critical care transport, and 19 percent to mileage charges; these proportions are for evaluation only and do not guarantee minimum volume.

The estimated total authorization for multiple agreements cannot exceed $4,431,540. Compensation is fixed for the initial five-year contract term, with any price adjustments for the optional five-year renewal period limited to the Consumer Price Index for the San Francisco-Oakland-Hayward region. DPH will award both a primary and secondary contractor based on evaluation scores: 20 points for cost, 20 points for firm qualifications, 25 points for staff qualifications, and 35 points for demonstrated understanding of scope of work, with interviews potentially adding 25 additional points. Questions are due June 15, 2026 at 5:00 PM PT. The Q&A addendum is estimated to post June 22, 2026. Award is anticipated August 24, 2026, with a contract start date of January 1, 2027. Proposers must comply with San Francisco labor laws including the Minimum Compensation Ordinance, Health Care Accountability Ordinance, Health Care Security Ordinance, Fair Chance Ordinance, and First Source Hiring Program requirements.
