This Amendment is issued to update the Pre-Solicitation with current information and to include the Solicitation Notice/Request for Proposal for a commercial requirement for VISN 6 Home Oxygen delivery and set-up to Veteran beneficiaries in Virginia, North Carolina, and West Virginia. The Department of Veterans Affairs, Veterans Integrated Service Network (VISN 6) has a need for Home Respiratory Supplies to include delivery and set-up for veteran beneficiaries in the VISN 6 area which includes the following eight medical facilities: Asheville, North Carolina; Beckley, West Virginia; Durham, North Carolina; Fayetteville, North Carolina; Hampton, Virginia; Richmond, Virginia; Salisbury, North Carolina; and Salem, Virginia. VISN 6 currently serves an estimated 5,000 oxygen using patients covering three (3) states: North Carolina, Virginia and West Virginia. Required services include storage, delivery, set-up, adjustment, patient/caregiver instructions and recovery of equipment. Home Oxygen delivery and set-up shall be available 24 hours a day, seven (7) days a week, including holidays. The Contractor must be Joint Commission (JC) accredited for Home Care or equivalent accredited standard and must maintain the certification or equivalent accredited standard for the duration of the contract. The NAICS Code for this requirement is 532291 Home Health Equipment Rental with a size standard of $7.0M. This acquisition is a 100% Small Business Set-Aside. A Firm Fixed Price Indefinite Delivery/Indefinite Quantity type contract with a one (1) year base period and up to four (4) one (1) year option periods, to be exercised at the Government's discretion is contemplated under this solicitation. A formal Request for Proposal (RFP) in accordance with Federal Acquisition Regulation (FAR) Part 12 Acquisition of Commercial Items and FAR Part 15 Contracting by Negotiation is included with this notice. Proposals are due by 4:00PM EST on July 29, 2013 unless changed by Amendment. It is the responsibility of potential offerors to monitor the website for downloading the solicitation and any amendments. Email/facsimile/telephonic requests for the solicitation will not be accepted. All inquires should be directed in writing to the Contract Specialist (Contractor) Mr. Larry Lapp, at larry.lapp@va.gov, or 100 Emancipation Drive, Bldg. 27, Hampton, VA 23667.
