Update #8 ·

CPARS/Past Performance Evaluation
Due to the concern over the CPARS requirement by the majority of vendors who attended the undustry day meeting, and email correspondence prior to the industry day meeting, the government has determined it to be in the best interest of this procurement to rescind the requirement for CPARS. CPARS, however, will still be reported for this contract.

The government remains firm in its evaluation and will still place the highest weight on past performance. To facilitate the past performance evaluation past performance questionnaires will be considered; therefore, prospective vendors shall submit the following with their proposals:

past contract numbers,
government organizations for which the contracts were with,
contracting officers' name, phone, and email,
contracts periods of performance.

The number of instances of past performance has not changed. If the vendor chooses to submit additional contracts to be subject to the past performance questionnaire, only the number of contract required by the soliciation will be evaluated and the government will randomly choose the three instance of past performance.

The soliciation will be modified to reflect the change in the CPARS and past perforance evaluation.

Number of Pages Allowed
The maximum number of pages allowed will be changed in the soliciation due to concerns from prospective vendors that they cannot include all the necessary information in the 25 page limitation. The number of allowable pages will be increased to allow no more than 40 pages. The soliciation will be modified to reflect the maximum number of pages allowed.

Update #7 ·

The government plans on conducting an industry day for interested firms on 10 March 2016. Interested firms should contact the contract specialist via email at otha.b.henderson.mil@mail.mil to schedule. Those firms who cannot physically attend need to let the government know so we can make arrangements to allow you to participate via teleconference.

The main focus of this meeting is to discuss issues and answer questions in a group setting such that all interested parties will have the same information available and have clear understanding of the requirement. One-one-one meetings are currently not planned.

The industry day meeting is scheduled 0900 - 1100 (MST), and will be in the 97th Troop Command Conference Room, 12953 South Minuteman Drive, Draper, UT  84020. A teleconference option is available and call-in information has been, or shortly will be, provided to parties expressing an interest in attending via teleconference. Parties interested in attending the meeting in person should contact the contract specialist at otha.b.henderson.mil@mail.mil .

Update #6 ·

This is not a new requirement. It is currently being filled under contract W911YP-15-P-0044-P00002. The incumbant is Soft Power Solutions (CAGE 5C920).

The government elected to not use the SCA Wage Determination because we are evaluating under a price technical trade-off, and because the expected wages under this contract are expected to exceed the wage determination. Evaluation for the award will be based most heavily on technical and past performance.

If you have questions, please refrain from asking questions via telephone and voicemail. For auditability, please email questions to otha.b.henderson.mil@mail.mil .

Update #1 ·

The Department of Defense (DoD) and TRICARE Management Activity (TMA) values all staff involved in the delivery of high-quality care to service members. In 2009, a Medical Management Guide was issued by the Office of the Assistant Secretary of Defense for Health Affairs (ASD [HA]), TMA, Office of the Chief Medical Officer (OCMO), Population Health and Medical Management Division (PHMMD). The guide covers the components of a medical management (MM) program, including applicable principles, implementation concepts, processes, tools/databases for utilization management (UM), case management (CM), and disease management (DM). It complements the 2001 DoD Population Health Improvement Plan and Guide published by TMA and the Government Printing Office http://www.tricare.mil/ocmo/download/mhs_phi_guide.pdf. Under legislative mandates, the ASD (HA) submitted an annual report to Congress regarding healthcare delivery for Military Health System (MHS) beneficiaries. The 2009 report documented the MHS goals to provide high-quality care, improve performance through clinical and process outcomes, and increase patients' confidence in the care they receive. The Guide describes crucial components of MM within the MHS, including the link between MM and population health and the dynamics between UM, CM and DM in helping reduce unnecessary or inappropriate services and/or duplicated services. In the military setting, CM targets active duty service members and beneficiaries with complex, multi-system healthcare needs who require care coordination or case management services. Originally, CM almost exclusively targeted in-patients with catastrophic illnesses or injuries. However, a number of developments have served to change that strategy; notably:

     - The emergence of the National Committee for Quality Assurance (NCQA): http://www.ncqa.orgorg/.

     - The development of accreditation standards by the Utilization Review Accreditation Commission (URAC): http://www.urac.org.

While CM continues to focus on catastrophic illnesses or injuries, CM practices are also directed at addressing more prevalent chronic conditions in the general patient population. Case managers can affect patient outcomes through proactive interventions across multiple healthcare settings.  Additionally, case managers are expected to engage community resources and facilitate ongoing and consistent patient education.

The MHS has three primary goals for CM:

      - Improve the care, management, and transition of recovering service members.

      - Broaden the application of CM to include those with complex and at-risk needs before the Soldier requires complex care.

      - Evaluate how CM impacts the quality of military health care.

The Army National Guard (ARNG), recognizing the importance of medical readiness after several years of combat deployments, implemented the Case Manager Support Contract in August 2005 as a time and materials contract, not firm fixed price and Performance Statement of Work-based. The current contract was originally awarded August 2009 to Skyline Ultd (with 49% subcontract to Sterling Medical).

At the end of FY09, case managers in 31 states were using the medical non-deployable module to manage their caseloads. During the period of August 2008 to August 2009, 15,292 cases were put into the module; of these, 6,734 were closed as return to duty, fit for duty, or forwarded for an MEB/PEB evaluation. Additionally, readiness increased from 35% fully-ready in FY08 to 44% in FY09. This success resulted from increased targeted funding and a concerted effort by the National Guard Bureau.

On 29 March 2012, the ARNG achieved the Department of Defense (DoD) goal of 75% (FMR) Soldiers per DoD Instruction (DoDI) 6025.19, IMR. This monumental accomplishment marked the highest medical readiness percentage the ARNG has achieved in documented history. Over the past year, ARNG readiness has continued to increase to over 80%. Continued utilization of medical case managers is essential to maintaining established medical readiness goals for the ARNG.
