Contract Amendment Summary

This is Amendment #4 to Contract YH23-0067 between the Arizona Health Care Cost Containment System (AHCCCS) and Arizona Board of Regents, acting on behalf of Arizona State University (ASU). The amendment, effective upon final signature with signatures dated December 15-22, 2023, modifies the Statement of Work (SOR) III Year 2 Price Sheet and Budget Narrative for the funding period September 30, 2023, through September 29, 2024. The contract is funded through the State Opioid Response III Grant (SOR III), a federal grant administered by the Substance Abuse & Mental Health Services Administration (SAMHSA) under CFDA 93.788.

The amendment updates ASU's budget allocation to $217,000 for Year 2 of the project, comprised of $150,277 in direct charges and $66,723 in indirect costs calculated at 44.40% of modified total direct costs. Direct charges include personnel costs of $94,294 (nine staff members ranging from Principal Investigator to Research Specialists), fringe benefits of $31,214, travel expenses of $2,369 (including local Arizona travel and attendance at the American Association for the Treatment of Opioid Dependence conference in Las Vegas), and other operating costs of $22,400 (consultant fees for hub team members, printing materials, advertising, shipping, and conference registration). The project focuses on technical assistance for recovery residences, medication for opioid use disorder (MOUD) resource dissemination, provider surveys, and data analysis. All other contract terms remain in effect, and the contractor is authorized to transfer up to 10% of the total budget between funded line items with prior written approval from the Program Manager.
