About this file

This file is a voter registration notification letter issued by the North Carolina State Board of Elections (NCSBE) to voters whose driver's license numbers or Social Security Administration information could not be validated against state and federal databases. The letter notifies recipients that their voter records contain unvalidated identification information but confirms that receiving the notification does not affect their ability to vote or the validity of their votes. The letter explains potential reasons for validation failures, including data entry errors, name variations across records, or registration prior to 2004 when such information was not required. The NCSBE requests that voters update their records by providing corrected driver's license numbers, state ID card numbers, or the last four digits of their Social Security numbers through one of three submission methods: mail using a pre-addressed, postage-paid envelope; in-person submission at their county board of elections office; or online through the North Carolina DMV's secure website at payments.ncdot.gov.

The letter includes a form section requiring voters to provide updated identification information, confirm or update their residential address, and sign and date the submission. Voters are warned that fraudulently or falsely completing the form constitutes a Class I felony under Chapter 163 of the North Carolina General Statutes. The initiative addresses requirements under the federal Help America Vote Act (HAVA) and North Carolina state law mandating that voter registration records contain validated identification numbers. This mailing effort represents a systematic state-level voter roll accuracy improvement project targeting all registered voters in North Carolina who lack valid identification numbers on file, with no specified response deadline or contract award information provided in this notification document.

NORTH CAROLINA STATE BOARD OF ELECTIONS

[MAILING ADDRESS ▪ CITY ▪ STATE ▪ ZIP]

Phone: [Phone Number] ▪ Fax: [Fax Number] ▪ [Email Address]

[Recipient’s Name] [Recipient’s Mailing Street Address] [Recipient’s Mailing City, State, ZIP]

We are sending you this letter to ask for your help as we work to improve the accuracy of our voter records. Receiving this request does not affect your ability to vote, how you vote, or whether your vote counts.

Receiving this letter does not mean that there is a problem with your record. It does, however, mean that we were unable to match information in your voter record with a record in either the North Carolina DMV database or the Social Security Administration (SSA) database.1

A few situations could prevent us from matching your record. Here are a few examples:

Data Entry Error: Date of birth, driver license number, or social security number errors could result from mistakes like mixing up digits or entering data into incorrect fields.

Form Changes: Prior to 2004, applicants weren’t required to provide their driver license number or the last 4 digits of their social security number when registering to vote. Voters who registered before then don’t have these numbers attached to their voter records—so we wouldn’t be able to match their records with the NCDMV or SSA database records.

Name Variation: A name may be different across records due to spelling, hyphenation, or spaces and apostrophes, or if a prior legal name or maiden name is used in a record. Your name at the top of this letter reflects what we have in our records.

Please fill out the information section on the back of this form and submit it to your county board of elections office to update your voter registration record.

There are three ways you can provide us the requested information:

• BY MAIL: Complete the information section of this form. In the enclosed pre-addressed, postage-paid envelope, mail this letter to the State Board of Elections.

• IN PERSON: Take this letter to your county board of elections office to submit your form in person or to discuss options with your elections officials.

• ONLINE: Go to payments.ncdot.gov to access the North Carolina DMV’s secure website (You won’t be charged a fee for this action). Click “Yes” when asked if you would like to update your voter information. Note: You must possess either a valid NC driver license or NCDMV ID to use this option.

1 Both federal and state law require a person registering provide their driver license number, or—if they do not have a driver license—the last 4 digits of their social security number along with their first and last name and date of birth. An applicant with neither number must note that they do not have these numbers. The county board of elections then verifies that this information matches a record in the databases.

FRAUDULENTLY OR FALSELY COMPLETING THIS FORM IS A CLASS I FELONY UNDER CHAPTER 163 OF THE NC GENERAL STATUTES.
