1.0 Scope

The Elizabeth Mine Superfund Site (Site) is an abandoned copper mine located on privately owned land in the Towns of Strafford and Thetford, Orange County, in east-central Vermont. It is located approximately two miles southeast of the village of South Strafford, on the eastern flank of Copperas Hill. The Site is approximately 15 miles northwest of White River Junction, Vermont, and 9 miles west of the Connecticut River.

The Site includes three small watersheds containing Copperas Brook, Lord Brook, and Sargent Brook, which all discharge to the West Branch Ompompanoosuc River

(WBOR).

The Ompompanoosuc River flows into the Connecticut River about ten miles downstream of the Site. The topography of the area is steep mountainous terrain with elevations ranging from approximately 830 feet above mean sea level (MSL) (North American Vertical Datum [NAVD] 1988) adjacent to the WBOR, to 1600 feet above MSL at the top of Copperas Hill. The Elizabeth Mine was developed in open pits and in extensive underground workings. The mine area is a source of heavy metals and sediments to Lord Brook and Copperas Brook which carry the drainage from the mine and tailings deposits to the WBOR. Mine dumps and tailings from the Elizabeth Mine have been exposed both historically and currently to weathering processes that result in acid mine drainage (AMD) and acid rock drainage (ARD), resulting high metals concentrations and low pH values. Major sources of ARD included Tailing Pile 1 (TP-1, 30 acres), Tailing Pile 2 (TP-2, 5 acres), Tailing Pile 3 (TP-3, 6 acres) which consisted of a waste pile adjacent to the North Open Cut, and Tailing Pile 4 (TP-4) which consisted of a waste pile on the east side of Copperas Road adjacent to South Open Cut. A Non-Time Critical Removal Action (NTCRA) was performed to consolidate and cap the tailing piles (see Figure 1).

The major efforts under this Task Order include, but are not limited to rock stabilization and consolidation and capping of mining materials at the Lord Brook Source Areas, which comprise the South Mine (SM), South Open Cut (SOC), and TP-4, to control the discharge of dissolved metals to the Lord Brook drainage area. The primary objective is to ensure surface water leaving the Lord Brook drainage area is in compliance with all federal and state water quality criteria (chemical and physical criteria) at the specified point of compliance. The specified point of compliance for discharge to the South Open Cut branch of un-named tributary to Lord Brook is sample location SC04. For any discharge to Copperas Brook, the point of compliance is sample location CB16.

Compliance points are shown on Figure 1 below.

All work will be performed in accordance with the Comprehensive Environmental Response, Compensation and Liability Act (CERCLA), the EPA Action Memoranda signed in September 2002 and March 2004, the Record of Decision signed in September 2006 and the associated September 2015 Explanation of Significant Differences, the OU1 Remedial Design, State of Vermont Solid Waste and Sediment and Erosion Control requirements, and EPA green and sustainability principles.

The documentation shall include an Activity Hazard Analysis (AHA) for each phase of work as described below. All work shall be conducted in accordance with the APP, the U.S. Army Corps of Engineers Safety and Health requirements Manual (EM 385-1-1, most recent edition), UFGS-01 35 29.13 (attached), and all applicable federal, state, and local safety and health requirements. A copy of EM 385-1-1 can be accessed electronically at Headquarters USACE website under publications using the following link:

http://www.usace.army.mil/library/.

The APP shall detail how safety and health will be managed during the project. The APP shall address the requirements of applicable Federal, State and local safety and health laws, rules, and regulations. The Contractor shall comply with Federal Acquisition Regulation Clause No. 52.236-13 for Accident Prevention, which is added by reference. Special attention shall focus on the requirements of EM 385-1-1, specifically Section 01.A.11 through 01.A.18, Figure 1-2 AHA, and Appendix A, (Minimum Basic Outline for Accident Prevention Plan). The APP shall be developed by a qualified person. The contractor shall be responsible for documenting the qualified person’s credentials.

The contractor must prepare a Site Health and Safety Plan (SSHP) that covers the 14 elements identified in EM385-1-1, EM385-1-1, Section 33.B.02.

At a minimum, the SSHP shall contain the following elements: site description and contaminant characterization, safety and health hazard(s) assessment and risk analysis, safety and health staff organization and responsibilities, site specific training and medical surveillance parameters, personal protective equipment (PPE) and decontamination facilities with documented procedures for monitoring and sampling requirements, safety and health work precautions and procedures, site control measures, on-site first aid and emergency equipment, emergency response plans and contingency procedures (on-site and off-site), logs, reports, and record keeping. The SSHP shall be attached to the APP as an Appendix. SSHP elements adequately covered elsewhere in the APP need not be duplicated in the SSHP. Assume that all tasks can be performed in modified Level D.

The APP shall interface with the Contractor's overall safety and health program. Any portions of the Contractor's overall safety and health program referenced in the APP shall be included in the applicable APP element and made site-specific. The Government considers the Prime Contractor to be the "controlling authority" for safety and health of the subcontractors. Contractors are responsible for informing their subcontractors of the safety provisions under the terms of the contract, the penalties for noncompliance, and inspecting subcontractor operations to ensure that accident prevention responsibilities are being carried out. The Contractor shall ensure that its subcontractors, suppliers and support personnel follow all safety and health provisions established in the approved Site Safety and Health Plan (SSHP), APP and/or AHA.

The Contractor shall conduct a safety meeting at the project site on the first day of work, and whenever a new activity or phase of work begins. All safety meetings shall be documented. Records of the safety briefings shall be submitted to the GDA weekly.

Activity Hazard Analysis (AHA)

An AHA shall be submitted for each major phase of work. A major phase of work is defined as an operation involving a type of work presenting hazards not experienced in previous operations. The analysis shall define all activities to be performed, identify the sequence of work, the specific hazards anticipated, and the control measures to be implemented to eliminate or reduce each hazard to an acceptable level. Work shall not proceed on a phase of work until the AHA has been accepted by the GDA. A preparatory meeting shall be conducted by the contractor to discuss the AHA contents with all engaged in the activity. The preparatory meeting shall be conducted by the prime contractor and shall include all subcontractors and Government on-site representatives.
