- Volume II, the Offeror’s Past Performance Proposal
- Volume III, the Offeror’s Price Proposal
Reference Solicitation : W912HZ18R0004
Proposals are due on 29 May 2018, 0900 hours (9:00 AM CST).
mailto:traci.k.hoofman@usace.army.mil
The Government intends to evaluate offers and award one firm fixed price contract without discussions with
Offerors. However, the Government reserves the right to conduct discussions if later determined by the Contracting
Officer to be necessary.
1.1 The proposal shall be typed, all pages numbered and a Table of Contents provided. The first page
(cover sheet) of each volume shall identify the Offeror by name.
1.2 In accordance with the Federal Acquisition Regulation (FAR) 4.1102, prospective contractors must be registered in the System for Award Management (SAM) prior to the award of a contract. You may register at the SAM website, http://www.sam.gov.
1.3 It is the Offeror's responsibility to ensure the completeness and accuracy of his or her proposal.
Corrections will not be made by the Government. Errors in the proposal will be considered, among other factors, as a reflection of the Offeror's technical and management capability.
1.4 It is the policy of the Corps of Engineers to not provide any assistance to prospective Offerors in the preparation of their proposals. However, this is not meant to preclude Offeror's requests for information or clarification. Information will be provided if it is available and releasable, while clarification will be provided in all cases.
2. Volume I, Technical Proposal.
Offerors are cautioned that “parroting” of the Technical requirements or the PWS with a statement of intent to perform does not reflect an understanding of the requirement or capability to perform. Offerors are responsible for including sufficient details to permit a complete and accurate evaluation of each proposal. Proprietary information shall be clearly marked.
Offerors shall submit their Technical Proposal following the format below. Proposals not conforming to this format may not receive as high an evaluation as a properly formatted proposal would because the information is not as easily located, extracted and analyzed by the Government. Conformance with requested format for a proposal demonstrates ability to follow directions and understand requirements.
2.1 A cover sheet with the Offeror's name, the volume number, and the project shall be provided. The second sheet shall be a table of contents.
2.2 Offerors should include, at a minimum, the following information in their proposal:
(1) Technical Approach – The procedure which the offeror will follow in providing services under this contract, including descriptions or examples of:
• Policies
• Organizational structure
• Any proposed employee orientation sessions
• Training of supervisors and managers
• Program promotion and outreach efforts
• Standards and methodology used in counseling, case management, case files and reports
• Utilization of existing area community treatment/rehabilitation resources
• Methods of maintaining required confidentiality of client information and safeguards the Offeror will employ to protect information from improper disclosure
• Proposed methods of how employees, supervisors and/or managers are to contact and utilize EAP counseling staff
• Office location and hours of operation
• Reports and other information that the Offeror will provide to the Government
• Quality Control Plan http://www.sam.gov/
(2) Qualifications of Proposed Staff
• Provide resumes, background experience and credentials of all personnel (employees, associated counselors, etc.) who will provide services to the Government under this contract. Where employees’ names are not known, the use of position titles will be acceptable.
• The names, resumes, qualifications, specialization, and relevant experience of associated counselors who will be available to provide local face-to-face counseling services in each of the following geographical areas:
a. Vicksburg, Mississippi
b. Champaign, Illinois
c. Hanover, New Hampshire
d. Alexandria, Virginia
(3) Business Experience providing EAP Services for the type required under this contract
The Offeror shall submit a description of contracts performed or awarded in the past three (3) calendar years which are relevant to the efforts required by this solicitation. The description shall include a brief summary of each contract’s scope of work and size of population served. The listed contracts should be limited to experience that involves the type of work included in this solicitation or other experience that is considered by the Offeror to be relevant.
In determining relevancy, consideration should be given, but not limited to, such things as similarity of services, size and complexity, contract type, contract environment, division of company proposing and any subcontractor interaction.
3. Volume II, Past Performance.
Completed Past Performance Questionnaires should be submitted with your proposal as required by RFP Section L.
Offerors shall not incorporate by reference into their proposal PPQs previously submitted for other RFPs. This does not preclude the Government from utilizing previously submitted PPQ information in the past performance evaluation. While the Government may elect to consider data from other sources, the burden of providing detailed, current, accurate and complete past performance information rests with the Offeror.
The Past Performance Questionnaire (PPQ) included in the solicitation is provided for the offeror or its team members to submit to the client for each project the offeror includes in its proposal for Factor 3. Ensure correct phone numbers and email addresses are provided for the client point of contact. Completed Past Performance
Questionnaires should be submitted with your proposal. If the offeror is unable to obtain a completed PPQ from a client for a project(s) before proposal closing date, the offeror should complete and submit with the proposal the first page of the PPQ (Attachment 1), which will provide contract and client information for the respective project(s). Offerors should follow-up with clients/references to ensure timely submittal of questionnaires. If the client requests, questionnaires may be submitted directly to the Government’s point of contact, Traci K. Hoofman via email at traci.k.hoofman@usace.army.mil prior to proposal closing date. Offerors shall not incorporate by reference into their proposal PPQs previously submitted for other RFPs. However, this does not preclude the
Government from utilizing previously submitted PPQ information in the past performance evaluation. Also include performance recognition documents received within the last 3 years such as awards, award fee determinations, customer letters of commendation, and any other forms of performance recognition.
In addition to the above, the Government may review any other sources of information for evaluating past mailto:traci.k.hoofman@usace.army.mil performance. Other sources may include, but are not limited to, past performance information retrieved through the
Past Performance Information Retrieval System (PPIRS), including Contractor Performance Assessment Reporting
System (CPARS), using all CAGE/DUNS numbers of team members (partnership, joint venture, teaming arrangement, or parent company/subsidiary/affiliate) identified in the offeror’s proposal, inquiries of owner representative(s), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract
Reporting System (eSRS), and any other known sources not provided by the offeror.
While the Government may elect to consider data from other sources, the burden of providing detailed, current, accurate and complete past performance information rests with the Offeror.
The Offeror shall also include a reference list of all the POC's who were sent a questionnaire. The POC list shall be submitted in Word for Windows Table Format to include the following fields: Solicitation Number, Offeror
Company Name, Contract Number, Customer’s Name (that received your services), POC last name, first name, and POC Title. The responsibility to send and track the completion of the Questionnaire rests solely with the
Offeror and shall not be delegated to any subcontractors, team contractors, or joint venture partners.
If an Offeror has no relevant past performance history, the Offeror must affirmatively state that it possesses no relevant directly related or similar past performance.
The Government wants to ensure timely contract award. To that end, Offerors are encouraged to submit
Past Performance information before the solicitation close date to enable the Government to begin contacting references.
New Corporate Entities -- New corporate entities may submit data on prior relevant and recent contracts
(within the last 3 calendar years) involving its officers and employees. However, in addition to the other requirements in this section, the Offeror shall discuss in detail the role performed by such persons in the prior contracts cited. Past performance information on work for state and local governments, private sector clients, and subcontractors that are similar to the Government requirement will be evaluated equally with similar federal contracts. Offeror may submit information on key personnel, major subcontractors, work performed as part of a team or joint venture and other previous reincarnation of its current organization.
4. Volume III, Price Proposal.
Price proposals will be evaluated for reasonableness by price analysis. The total contract price will be considered as an independent element from the technical and past performance evaluation.
Offerors should submit the price proposal in the following format. Failure to follow this format may be detrimental because information is not as easily located, extracted, and analyzed by the Government.
Conformance with the requested format for a proposal demonstrates ability to follow directions and understand requirements.
4.1 A cover sheet with the Offeror's name, the volume number, and the project shall be provided. The second sheet shall be a table of contents.
4.2 Price proposals shall consist of the following basic components:
a. Fully completed SF 1449 and bidding schedule;
b. Fully completed Representations and Certifications;
c. Unit (monthly) prices,
d. Extended annual prices (monthly price X 12), and
e. Total price for the entire contract period (sum of annual prices for base year and 4 option years plus 6-months option).
5. Relationship of Accepted Proposal Content to the Contract Requirements.
By submission of an offer pursuant to this solicitation, the Offeror agrees that the capability presented in the proposal which thereby becomes a contract requirement as accepted by the Contracting Officer upon award of a contract. No changes, substitutions, or deviations from the accepted proposal may be made without the approval of the
Contracting Officer. The Offeror agrees that the explicit capability presented in their proposal shall be provided under this contract at the stated price.
