CONTRACTOR MANPOWER REPORTING REQUIREMENTS & INFORMATION

The Office of the Assistant Secretary of the Army (Manpower & Reserve Affairs) operates and maintains a secure Army data collection site where the contractor will report ALL contractor manpower (including subcontractor manpower) required for performance of this contract. The contractor is required to completely fill in all the information in the format using the following web address https://contractormanpower.army.pentagon.mil or https://cmra.army.mil.

The required information includes:

(1) Contracting Office, Contracting Officer, Contracting Officer’s Technical Representative;

(2) Contract number, including task and delivery order number (if applicable);

(3) Beginning and ending dates covered by reporting period;

(4) Contractor name, address, phone number, e-mail address, identity of contractor employee entering data;

(5) Estimated direct labor hours (including sub-contractor);

(6) Estimated direct labor dollars paid this reporting period (including sub-contractor);

(7) Total payments (including subcontractor);

(8) Predominant Federal Service Code (FSC) reflecting services provided by contractor (and separate predominant FSC for each sub-contractor if different) FSC Code: Y1LB

(9) Estimated data collection cost;

(10) Organizational title associated with the Unit Identification Code (UIC) for the Army Requiring Activity (the Army Requiring Activity is responsible for providing the contractor with its UIC for the purposes of reporting this information);

(11) Locations where contractor and sub-contractors (if applicable) perform the work (specified by zip code in the United States and nearest City, Country, when in an overseas location, using standardized nomenclature provided on website);

(12) Presence of deployment or contingency contract language, and,

(13) Number of contractor and sub-contractor employees deployed in theater this reporting period (by country).

As part of its submission, the contractor will also provide the estimated total cost (if any) incurred to comply with this reporting requirement. Contractors may use a direct XML data transfer to the database server or fill in the fields on the website. The XML direct transfer is a format for transferring files from a contractor’s system to the secure website without the need for separate data entries for each required data element at the website. The specific formats for the XML direct transfer may be downloaded from the website.

This requirement shall be completed with 30 days from the end of the service.

Reporting period will be the period of performance not to exceed 12 months ending September 30 of each government fiscal year and must be reported by 31 October of each calendar year.

REQUIREMENTS AND INFORMATION

SECTION 01 00 00 - GENERAL REQUIREMENTS

PART 1

1.1. FUNCTION AND AUTHORITY OF U.S. GOVERNMENT PERSONNEL

A. Contracting Officer. Contracting Officers are agents of the Government and are the only officials authorized to enter into any contract or agreement that binds the Government.

Only the Contracting Officer may authorize the performance of additional work or make changes not specified in the contract

B. Technical Representatives. The Contracting Officer will assign a representative (Contracting Officer's Representative, COR) to act as the technical representative for the project. The duties of the COR are addressed at the time of contract award.

1.2. SUMMARY OF WORK:

A. Project titled: “JKSE 192404 –REPAIR ENGINE SHOP FLOOR, B30” - This project includes extensive building demolition and related operations.

B. The Work consists of furnishing all supervision, plant, labor, equipment, appliances, materials and incidentals performing all Work in strict accordance with the Specifications and Drawings.

1.3. SCOPE

A. Project provides for furnishing all plant, labor, materials, equipment and supervision to perform Project No: JKSE 182401 at the Montana Air National Guard, Great Falls, MT herein and shown on the construction drawings.

B. The Contractor will be held responsible for all requirements described in the contract documents. Failure to familiarize itself with their requirements will not relieve the contractor of its responsibility to comply.

C. The organization of the specifications into divisions, sections, and articles and the arrangement of the drawings, shall not control the Contractor in dividing the work among subcontractors or in establishing the extent of the work to be performed by any trade

1.4. ENTRY TO THE MONTANA AIR NATIONAL GUARD

A. See Security Specification Section 01 00 10 for details.

1.5. WORK SCHEDULING

A. Work hours for the Contractor will normally be between the hours of 7:00 AM through 5:00 PM, excluding Saturdays, Sundays, and Federal Holidays. If the Contractor desires to work during periods other than above, notification must be provided to the Contracting Officer three (3) working days in advance of its intention to work during other periods to allow assignment of additional inspection personnel. When the Contracting Officer

determines that Government inspection personnel are reasonably available, the Contractor may be authorized to perform in excess of their normal duty hours/days. However, if Government inspection personnel are required to perform in excess of their normal duty hours/days due to scheduling by the Contractor, the actual cost of the inspection, at overtime rates, will be charged to the Contractor and will be deducted from the Contract amount.

1.6. DIVISION OF WORK

A. All reasonable quantities of utilities will be made available from existing utilities to the Contractor without charge. Any temporary connection or lines that may be required will be installed, maintained, and removed by the Contractor at his own expense and in such a manner satisfactory to the Contracting Officer. Removal of such connections or lines will be accomplished prior to final acceptance of the construction. The Contractor shall provide any necessary portable power.

B. The various divisions of the Specifications shall not be considered as negotiations of the material and labor involved. The arrangement and order of these divisions have been made for convenience It is not the intent, nor shall it be so construed, that a particular trade or subcontractor must perform the work in any one division.

C. Any item mentioned, under any division heading, must be supplied even though it is not specified under the heading for the respective work, but is shown on the Drawings. No claims for extras arising out of real or alleged error in such arrangement or order of the various divisions will be given consideration

1.7. LIST OF SUBCONTRACTORS

A. The Contractor shall identify all subcontractors on the Standard Form (SF) 1413, "Statement of Acknowledgment", who will be employed to work in the contract. Federal labor standards apply to all subcontractors as well as the prime contractor.

1.8. CORRELATION OF DRAWINGS, SPECIFICATIONS AND CONTRACTS

A. The Specifications, Contract and the accompanying Drawings are intended to describe and provide for complete and usable facilities. They are intended to be cooperative, and what is called for by one shall be as binding as if called for by all. The Contractor will understand that the work herein described shall be complete in every detail, notwithstanding every item necessarily involved is not particularly mentioned. The Contractor shall be held to provide all labor and material necessary for the entire completion of the work intended to be described, and shall not avail itself of any manifestly unintentional error or omission, should any exist.

B. Should any error or inconsistency appear in the Drawings or Specifications, the Contractor, before proceeding with the work, shall make mention of same to the Contracting Officer for proper adjustment, and in no case shall it proceed with the work in uncertainty.

C. Any proposed changes to these specifications by the Contractor must be submitted in writing to the Contracting Officer for approval prior to implementation.

1.9. REPORT OF ERROR AND DISCREPANCIES

A. The Contractor shall be responsible for any and all discrepancies in work due to failure to obtain dimensions and investigate conditions at the site before fabrication and installation.

B. The Contractor shall bear all cost in replacing all materials and labor due to not observing the above paragraph and such replaced materials shall meet the approval of the Contracting Officer.

C. The Contractor shall promptly notify the Contracting Officer in writing of any discrepancies.

1.10. INSURANCE

A. The Contractor shall procure and maintain insurance as required in the Contract. The Contractor shall require and ensure compliance for insurance by subcontractors.

1.11. SAFETY:

A. The Contractor shall comply with all existing Occupational Safety and Health Act (OSHA) standards of safety at all times in the performance of this contract. Hazards to the safe use of the premises due to the Contractor’s work and/or equipment shall be suitably marked at all times. Pedestrian and vehicle traffic ways shall be kept clear and unobstructed.

PART 3 - EXECUTION

3.1. CONTRACTOR STORAGE AREA AND WORK SITE:

A. Prior to notice to proceed, Contractor will be provided a storage area on the base within three miles of the project site or as designated on the site plan. All contractor equipment and materials shall be stored in this area except for materials that will be used on the job within a 48 hour period.

B. Contractor is responsible for the security of assigned storage area. Storage areas shall be enclosed by 6’ chain link fence with access gates. Spare keys to any locked gates shall be provided to the base fire department dispatch office. Storage areas and job sites shall be clean, orderly, and free of debris, demolished/excess materials, etc.

C. Job sites will be cleaned daily and refuse and debris removed from the site. Fences will be maintained. Grass and weeds shall be cut weekly at the storage area and work site. All landscaping, trees and shrubs will be protected, irrigated and maintained. If irrigation system to an existing area is interrupted due to construction activities, the Contractor shall provide a means of temporary irrigation until the permanent system is operational.

D. If at any time during the life of the contract, the Contracting Officer determines that base standards are not being met, he/she may direct the Contractor to perform such actions as necessary to bring the area and/or facilities up to base standards at no additional cost to the Government. If the contractor fails to bring the area and/or facilities up to standards, the Contracting Officer may direct the Contractor to remove themselves and/or the facility/storage unit or materials from the base at no cost to the Government.

3.2. TEMPORARY FIELD OFFICES AND SHEDS

A. As soon as practical after award of Contract, and until final acceptance of the work, the Contractor shall provide, maintain and later remove, a suitable temporary office(s) for its own use. All field offices shall be maintained in good repair, and shall also be maintained in a clean and sanitary condition.

B. The Contractor may construct temporary sheds as it may require for the use of its workmen and as required for tool cribs and storage of all work under this Contract. Temporary sheds shall be confined to the space assigned by the Contracting Officer. Sheds shall be of approved construction.

C. The Contractor is responsible to provide heat, electrical connections, phone, data, and bottled water with a dispenser for both hot and cold water for onsite job trailer. The Contractor is responsible for the base rate service and no long distance charges. This requirement is until final acceptance of the work.

3.3. TEMPORARY TOILET FACILITIES

A. The Contractor shall provide, at its expense, all temporary toilet facilities. All temporary toilet facilities shall meet the requirements of health authorities having jurisdiction and shall be pumped at regular intervals, kept clean, and in a sanitary condition at all times.

3.4. UTILITIES

A. The Contractor shall have meters installed to monitor consumption of water and electricity on the construction site. Any expense incurred to gain access to these utilities (temporary tap-ins, etc.) shall be the responsibility of the Contractor and all utilities shall be returned to their original configurations at the end of the contract. No alterations to existing utilities shall be accomplished without the written permission of the Contracting Officer.

B. All utility shutdowns require the prior approval of the COR. Request for utility shutdown shall be made in writing at least three (3) days prior to the expected date of implementation.

Every effort shall be made to make all shutdowns as brief as possible, and as limited in extent as possible.

3.5. TELEPHONE

A. If the Contractor desires to provide telephone services at its field office for its own use and for the use of subcontractors, it shall make all arrangements with the telephone company and shall pay all charges for installation, services and removal.

3.6. COMPRESSED AIR

A. The Contractor shall provide all compressed air used for work under this contract including temporary lines and connections. Remove all temporary lines, etc., at the completion of the project.

3.7. CONTINUED USE OF FACILITIES:

A. Building, structures, facilities, and utilities will continue to be in use by the Government during this contract. The Contractor shall notify the Contracting Officer’s Representative (Construction Inspector and/or Contract Specialist) when a no-work condition will occur for more than one day by the Contractor or sub-contractors (except weekends and federal holidays).

3.8. MAINTENANCE OF OPERATIONS:

A. It is required and essential that the primary use of the facilities continue uninterrupted throughout the contract period. Work, once commenced, shall be carried to completion with a minimum of interruption to Government operations.

3.9. UTILITY OUTAGES:

A. All utilities programmed to be interrupted during construction shall be scheduled at least 14 working days in advance of the outage and at a time convenient for the government. The “Utility Outage Notice” will be completed by the Contractor and submitted to Construction Management for approval. No interruptions shall be made until the outage notice is approved and returned to the Contractor. Notification forms that are to be filled out by the contractor, will be provided at the pre-construction conference. Utilities will include all overhead and underground utilities and road closures or partial closures.

3.10. SUBMITTALS:

A. Shop Drawings and Product data. The Contractor shall furnish submittals in the form of manufacturer’s brochures, pamphlets, or written specifications on all items to be installed unless specifically directed otherwise by these specifications or by the Contracting Officer.

B. Approval of Materials. Prior to the purchase of material, the Contractor shall electronically submit in a .pdf format to the Contracting Officer, for material approval/disapproval, brochures and technical literature covering, in detail, the materials it proposes to supply.

This shall include the specific catalog and model specification number designations.

Submittals shall demonstrate that the item conforms to all the requirements by calling out the exact model/item by either highlighting, boxing or arrowing the specific item (s). No unapproved or disapproved materials shall be used. Submittals shall be made for the items listed on the Schedule of Materials Submittals. The submittals shall be accomplished on, and in accordance with the instructions in the Air Force (AF) Form 3000, “Material Approval Submittal”. Unless there are objections or changes to the required submittal dates, the Contractor accepts the Schedule of Material Submittals. The number of submittals for each item and procedure for submission will be determined at the pre-construction conference. Following review coordination, one (1) set each of the submittal shall be maintained by the Contractor, the COR, and the Contracting Officer. The COR will accept and review up to three separate submittals of shop drawings for each item indicated on the Contractor’s accepted schedule of submittals.

C. The use of Brand Name is to set a standard of quality and is not intended to restrict the Contractor from offering an item from other sources of supply, as approved by the Contracting Officer. Pre-approval of products is "NOT" authorized

D. See the Submittal Schedule for items to be submitted on.

3.11. MEANING OF APPROVED, DIRECTED ETC.

A. "Approved", "Directed", "Required", "Applicable", or words of like or similar effect, when used in the specifications shall be interpreted to mean "Approved By", "Directed By", etc., the Contracting Officer unless otherwise specifically stipulated.

3.12. STANDARDS OF MANUFACTURE

A. For the purpose of establishing the standard of construction and the requirements to be met in the work of all divisions, the Drawings and these Specifications are based on the use of products hereinafter specified, adapted to the installation as required to meet the condition.

B. Where brand names are shown, these names are intended to describe a quality of product, and in no way are intended to limit products of equal quality. Therefore, products of other manufacturers may be employed for this work provided they are equivalent materials and construction, and equality adaptable to the conditions as approved by the Contracting Officer. All recognized regulatory/code standards shall be the latest published edition.

3.13. COMPLIANCE WITH CODES AND REGULATIONS

A. All work shall be done in accordance with the applicable codes and/or ordinances in force at the time of construction. It is the Contractor's responsibility to insure that where

Environmental Protection Agency (EPA), Department of Environmental Quality (DEQ) or other such regulations control the removal, handling, installation or disposal of materials, they shall be strictly adhered to whether or not specifically referenced in the construction documents.

B. To ensure compliance with OSHA and EPA, the Contractor shall data sheets available on site for materials used in the performance of the project.

3.14. START OF WORK NOTIFICATION:

A. The Contractor shall notify the Construction Management (CM) Office and the Contracting Officer 5 working days in advance of the start of all work. This shall include, but is not limited to, notifying CM when the initial work shall begin; notifying CM when work shall resume after a work stoppage of more than five work days; notifying CM when work shall begin following the end of all specified exclusion periods. The Contractor shall be aware that the Notice to Proceed is not sufficient notification to the CM when initial work will begin.

3.15. COORDINATION MEETINGS

A. Project coordination meetings may be conducted at regularly scheduled times convenient for all parties involved. Project coordination meetings are in addition to specific meetings held for other purposes, such as regular progress meetings and special pre-installation meetings.

B. Representation at each meeting shall be as required by the Contracting Officer for the construction activities involved.

3.16. PROGRESS MEETINGS

A. Progress meetings at the Project site shall be as directed by the Contracting Officer.

B. Attendees. In addition to the Contracting Officer, each subcontractor, supplier or other entity concerned with current progress or involved in planning, coordination or performance of future activities shall be represented at these meetings by persons familiar with the Project and authorized to include matters relating to progress.

C. Agenda. Review and correct or approve minutes of the previous meeting. Review other items of significance that could affect progress. Include topics for discussion as appropriate to the current status of the Project. The Agenda will be provided by the Architect and mailed to each attendee prior to the meeting

D. Schedule Updating. Revise the Contract Progress Schedule (AF Form 3064) when directed by the Contracting Officer, where revisions to the schedule have been made or recognized.

3.17. PERFORMANCE OF THE WORK

A. Scheduling. All work under the Contract shall be arranged and carried out in such a manner as to complete the work efficiently. A Contract Progress Schedule (AF Form 3064) shall be submitted at the pre-construction conference for the work under this contract. This chart shall show how the work is to progress from start to completion of the Contract as required by Contract Clause 52.236-15, Schedules for Construction Contracts. A Contract Progress Report (AF Form 3065) showing (in percentages) how the work has been completed for each

element of work listed in the Progress Schedule, shall be submitted to the Contracting Officer monthly.

B. Contractor use of the premises. Contractor shall conduct all work in such a manner as to minimize the inconvenience and disruption of the National Guard's daily schedule. Confine operations at the site to the area permitted under the contract. Portions of the site beyond areas on which work is indicated are not to be disturbed. Conform to site rules and regulations affecting the work while engaged in project construction

C. Keep existing driveways and entrances serving the premises clear and available to National Guard personnel at all times. Do not use these areas for parking or storage of materials. Lock automotive type vehicles, such as passenger cars and trucks and other mechanized or motorized construction equipment, when parked and unattended, so as to prevent unauthorized use.

D. Advise the COR whenever work is expected to be hazardous to National Guard personnel and/or operations.

3.18. INSPECTION:

A. There will be one primary inspector assigned to inspect for compliance. An alternate inspector will also be assigned and will perform all inspection duties in the absence of the primary inspector. The inspector will be the spokesman for compliance with the specifications and drawings. Controversies between the inspector and the Contractor will be resolved by the Contracting Officer. If, for some reason, a change in the primary or alternate inspector is required, the Contractor will be notified. The Contractor shall coordinate Contractor activities with Construction Management.

3.19. INSPECTIONS AND TESTS

A. ALL testing costs associated with this project are the sole responsibility of the Contractor.

B. Inspections and tests are for the sole benefit of the Government and shall not relieve the Contractor of the responsibility of providing quality control measures to ensure that the work strictly complies with the Contract requirements. No inspection or test by the Government shall be construed as constituting or implying acceptance. All items to be concealed shall be Government inspected prior to concealment

3.20. CONTRACTOR VEHICLES AND EQUIPMENT

A. The Contractor shall provide all vehicles and equipment necessary to accomplish the contract work. Power equipment shall be equipped with safety and noise limiting devices. The equipment shall be in a safe and efficient operating condition. The Contractor shall not park vehicles in the project area during non-working hours. A designated parking area for vehicles and equipment will be provided. All equipment will be clearly marked with the Company name.

3.21. HAUL ROUTES

A. The Contractor shall establish, prior to commencement of work and as approved by the Contracting Officer, entrance and exit routes at the construction site. When materials are

transported in execution of the work, vehicles shall not be loaded beyond the loading limit established by Federal, State or Local law or regulation. The Contractor shall provide protection against damage when it is necessary to cross curbing or sidewalks. The Contractor should be aware of existing bridge limitations and obtain approvals from the appropriate jurisdictions for haul routes.

B. Truckload Limits: The following load limits shall apply to all contractor-operated equipment on this project:

April 1 to June 1 350#/inch width of tire

All other Times 400#/inch width of tire

C. Maintenance of Traffic. It is understood and agreed that the Contractor shall provide for the free and unobstructed movement of vehicles on all areas with respect to its own operations and the operations of all its subcontractors. It is further understood and agreed that the contractor shall provide for the uninterrupted operation of visual and electronic signals (including power supplies thereto) used in the guidance of vehicles while operating to, from, and upon the site.

D. With respect to its own operations and the operation of all its subcontractors, the Contractor shall provide marking, lighting, and other acceptable means of identifying personnel, equipment, vehicles, storage areas, and any work or condition that may be hazardous to the operation of military vehicles, fire-rescue equipment, or maintenance vehicles at the construction area

E. The Contractor shall furnish, erect, and maintain weighted barricades, warning signs, and other traffic control devices as required to maintain traffic and insure the safety of military vehicles and the Contractor's equipment. The Contractor shall make its own estimate of all labor, materials, equipment, and incidentals necessary for maintaining efficient vehicular traffic patterns.

3.22. BLOCKING OF STREETS

A. At least three (3) working day prior to the blocking of any street, the Contractor shall advise the COR of its intentions, identifying the location and the estimated time of closure.

3.23. DAMAGES AND REPAIRS

A. Any damages by the Contractor’s operations shall be repaired, or replaced, at the Contractor's expense, as directed by the Contracting Officer. Any Government property damaged as a result of the work, materials, or operations of the Contractor shall be restored at no additional expense to the Government.

B. All existing sidewalks, curbs, and pavement disturbed, broken, or removed or otherwise damaged by the Contractor during performance of the work under this contract shall be replaced by the contractor at its own expense. Replaced sidewalks, curbs, and pavements shall be smooth, shall blend into the existing work, and shall not present depressions or humps.

3.24. HAZARDOUS MATERIALS

A. The Contractor shall establish a Hazardous Material (HM) storage and distribution system when HM is to be used. All HM required to support the contract shall be reported to the Hazardous Material Pharmacy (HMP) using the Contractor HM Identification Form. The Contractor HM Identification Form will be provided to the Contractor at or prior to the Pre- Construction meeting. Additional HM needed by the Contractor shall be identified to the Contracting Officer's Representative (COR) for approval by the HMP. Reference Attachment 18A.

B. The Contractor planning to use HM for the work shall register with the base HMP prior to start of work in order to support the installation's compliance with Executive Order 12856, Federal Compliance with Right-to-Know Laws and Pollution Prevention Requirements.

C. The Contractor shall maintain Contractor HM Identification Form for HM on the job site for inspection/verification.

D. The COR will verify that the HM identified to HMP in use on the job site.

E. The Contractor shall be responsible for the following items: Provide a list of each material and quantity of material for all proposed Hazardous Material (HM). HM shall be construed to mean any item that is:

(1) A health hazard or physical hazard as defined in 29 CFR, 1910.1200(c).

(2) Regulated in its disposal by EPA under 40 CFR.

(3) Hazardous as defined by DOT regulations under 49 CFR.

(4) Hazardous as defined by the Dangerous Goods Regulation of the International Air Transport Association.

(5) Provide a material safety data sheet (MSDS) for each item on the HM list.

F. The Contractor shall establish a construction specific HM storage and issue location that fully complies with Federal, State and Local environmental regulations. Materials issued shall be tracked for quantities used. Unused materials shall be inventoried and removed from the ANG installation prior to close out of the contract or expiration date of the HM. Reports of materials delivered, used and removed from the installation shall be submitted to the COR monthly and prior to contract close-out.

G. The Contractor shall comply with all Federal, State and Local environmental standards.

H. The Contractor shall accompany the Bio-environmental Engineering Representative (BEE) and the (COR) on project close-out inspection to ensure all used/unused HM is removed from the installation. This requirement shall not be a punch list item and must be accomplished prior to the Government accepting beneficial occupancy of the facility or construction item. Reference Attachment 18B

3.25. CONCEALED WORK:

A. All items of work to be concealed shall be Government inspected by the COR prior to concealment.

3.26. CRANE OPERATIONS

A. The Contractor will immediately notify the FAA COR and the Airport Operations if a crane is to be brought onto the site for the work under this project. A 7460 Notice of Construction on airport will need to be filed by the Airport for any crane or over height equipment (equipment whose height exceeds 10’-0” above grade) which is to be brought on site. FAA approval of the 7460 form for the use of the crane can take up to 45 calendar days.

B. The Contractor will be required to furnish and install an FAA compliant aviation orange and white checkered flag during daylight hours and a FAA Compliant aviation red dual obstruction light during periods of darkness and limited visibility at the highest point on the crane at all times. The flag must be at least a 3-foot by 3-foot (0.9 meter by 0.9 meter) square having a checkered pattern of international orange and white squares at least 1 foot (300 mm) on each side.

C. When the crane is not in use, the Contractor will lower the crane boom to its lowest point.

D. The Contractor will adhere to the Requirements of FAA A/C 70/7460-1 for this project. A copy is available at www.faa.gov.

3.27. CONSTRUCTION NEAR UTILITY SYSTEMS:

A. All contractors shall be responsible for processing their own Work Clearance Request (AF Form 103) and coordinating with Government inspectors as required. The contractor is responsible for obtaining all coordination signatures for the permit. The approved permit shall be valid for a period not to exceed 90 calendar days.

To the maximum extent possible, eliminate permanent markings on pavement and curbs. The contractor shall be responsible for maintaining all utility markings. Location marks shall be removed after 14 calendar days if not used. All markings shall be accomplished with water-based chalk (Aervoe Pacific Inc. 1-800-227-0196 or equal). No paints will be used.

Detailed permit procedures and guidelines shall be provided to the contractor during the Pre-performance Conference.

Digging within three feet of buried communications cables, environmental monitoring wells, electrical cables, irrigation systems, and gas lines shall be performed by hand digging until the utility is exposed. The Project Inspector shall be notified three days prior to digging within a three-foot area near this utility. A representative from the Communications Office must be present during excavation of Communications Cables. The cable routes must be marked prior to excavation in the area. The Contractor shall be held responsible for any damage to a marked or identified utility by excavation procedures. Once the utility is exposed, mechanical excavation may be used if there is no chance of damage occurring to the cable or piping system.

The contractor is responsible for marking all excavations he will do, five (5) work days prior to needing existing utilities marked by the government.

B. Reburial of Exposed Utilities: When existing utility lines are reburied, a tape, detectable by pipe detector systems, shall be installed above the uncovered length of the utility at a depth of 12 inches below grade. Tape shall be a minimum five mil plastic tape with metallic tracer, http://www.faa.gov/

minimum six inches wide, lettering on tape continuously marked “BURIED UTILITY”, and brightly colored. If tape marked “BURIED UTILITY” is not readily available, special order tape with the correct lettering shall be acquired by the contractor.

C. Access to Communications Manhole or Handhole: No communications manhole or handhole shall be entered without first obtaining a fiber optic cable briefing. Coordinate through Construction Management with the Base Communications Officer.

D. All repairs due to cable cuts on communications cables shall be the responsibility of the Contractor. Repair actions must be accomplished by the current communications contractor and paid for and coordinated under this contract at no additional cost to the Government if the contractor is deemed to be at fault by the Contracting Officer. Work to restore lost service must begin within one hour after the cut and must continue unceasingly until the job is completed, tested, and accepted. Work will be inspected by Communications Flight and personnel.

E. All utilities or anticipated underground obstacle crossings must be located by manual and other non-destructive excavation (hydro-vac) prior to mechanical excavation. All non-destructive excavation shall be considered incidental to project completion.

3.28. SALVAGE

A. All property identified as Government Salvage on the drawings shall be returned to the Government at a place on 120AW designated by the Contracting Officer Representative.

The Contractor shall properly dispose of all other materials, not re-installed, OFF SITE.

a. Exterior siding panels trim etc: Contractor shall remove from site and disposal of at legal disposal area for or recycle at contractor option.

3.29. DEBRIS

B. The Contractor shall maintain the construction site in as clean and orderly condition as possible. All refuse and/or salvage material shall be gathered and disposed of daily to maintain the site in this condition. Any debris that blows off the site into surrounding areas must be removed daily.

C. All refuse, debris and construction waste shall be legally disposed of OFF Federal Property at the Contractor's expense. The use of Government waste receptacles is strictly prohibited.

3.30. QUALITY CONTROL:

A. General. The CONTRACTOR is responsible for quality control which is considered by the Government to be a major inspectable item of this contract.

3.31. CONTRACT PROGRESS SCHEDULE AND REPORT:

A. Contractor shall complete Project Schedule Form 3064 using bi-monthly intervals, starting at the Notice to Proceed until Final Acceptance. The project schedule is required to be delivered to the Contracting Officer for review and approval prior to the start of work. Work may not commence until the project schedule is approved unless the Contracting Officer waives the requirement.

B. Contractor shall complete Progress Report Form 3065 and submit to the Contracting Officer every two weeks, starting at the Notice to Proceed until Final Acceptance. The Progress Report shall mirror the intervals approved on the Project Schedule.

3.32. APPLICATION FOR PAYMENT

A. Payments may be made monthly based on the Government percent of work completed as listed on the AF Form 3065, "Contract Progress Report". Requests shall be submitted to the Contracting Officer and shall contain the information required by Federal Acquisition Regulation Clauses 52.232-5, Payments under Fixed-Price Construction Contracts, and 52.232.27, Prompt Payment for Construction Contracts. (These clauses will be provided in full text to the successful contractor.)

3.33. FINAL CLEANING AND ACCEPTANCE OF THE WORK

A. Prior to the acceptance of the facility and at such times as directed by the Contracting Officer, the Contractor shall thoroughly clean all exposed surfaces of the building where work under this contract was completed.

B. All protective coatings, except lacquers, shall be removed from finished surfaces and the finished surfaces shall be washed and cleaned. Contractor shall be held responsible for all damaged materials, and at completion, shall replace, at its own expense, all such damaged materials.

C. The Government shall conduct pre-final and final inspections of the work under this contract prior to contract completion. The Contractor shall allow time to schedule and conduct the inspections within the contract performance period, allowing 10 calendar days’ notice to the Contracting Officer of each inspection. The design team shall perform one inspection of the project at pre-final and final inspection and develop a list of items that either do not comply with the contract or are not acceptable, to determine level of completion of the project. Any additional inspection by the design team beyond the pre-final and final inspection will be at the expense of the Contractor at a rate of $75 per hour, per person.

3.34. RED LINED DRAWINGS

A. The RED LINED drawings shall be complete and available at the final inspection. The drawings shall be completed with any deviation in actual construction.

B. The RED LINED drawings shall be submitted to 5 days after final inspection

3.35. AS-BUILT FIELD DATA:

A. The Contractor shall keep at the work site, an accurate record of all deviations, indicated in RED, from the approved design drawings and specifications which may occur in the work as actually constructed. The Contractor shall submit 2- CD’s and 2- 24X36 paper copies to the Contracting Officer, within 30 calendar days after pre-final inspection, complete information including descriptions, drawings, dimensions, marked prints, etc., as required for correction of the tracings to the as-built conditions in an AutoCAD 2014 compatible .DWG format and a .pdf format on a CD. Final payment will not be made until as-builts are submitted and approved

3.36. WARRANTY

A. The Contractor shall guarantee all materials and workmanship free from defects or failure for a period of one (1) year from date of contract completion. The Contractor further agrees that he will, at his own expense, repair and replace all such defective materials and all other material damaged thereby that becomes defective during the term warranty. All warranty periods shall run from the date of final contract acceptance. Manufacturer's material warranty shall remain in effect for length of material warranty specified, from the date of final contract acceptance.

3.37. MANUALS, PARTS, LISTS, ETC

A. Prior to final payment, the Contractor shall provide the Contracting Officer with one (1) bound sets containing complete maintenance, repair and operating instructions and parts lists for each piece of installed equipment.

B. Contractor shall provide 2 electronic CD PDF copies of O&M manuals, inclusion of repair and operating instructions, and parts lists for each piece of equipment installed

3.38. INSTALLED EQUIPMENT (REAL PROPERTY)

A. The Contractor shall furnish, upon completion of the project, a typewritten list, in triplicate, of all equipment installed in the facility, and the installed cost of each item. Furthermore, the listing shall include the location of each item and nameplate data.

B. Typically, the listing shall include: air conditioners, air handling units, condensers, fans, pumps, air compressors, transformers, unit heaters, regulators, direct current power supplies, latrine fixtures, motors, engines, motor or engine-driven equipment, cranes, drinking fountains, sinks, water coolers, generators, space heaters, water heaters, refrigerators, freezers, coolers, meters, gas detectors, humidifiers dehumidifier, air purifier, ovens, power units, fuel tanks, water tanks, elevators, welders, recorders, reels, scales, hydrants, intrusion detection equipment, fire detector and alarm equipment, emergency light sets, emergency eye wash deluge showers, washers, dryers, dishwashers, bridge cranes, and like items of equipment.

C. Final payment will not be made to Contractor until the Contracting Officer has received and approved the listing. The Contractor shall use the DD Form 1354 provided by the Civil Engineering Real Property Records Branch at the pre-construction meeting.

PART 1 - GENERAL

1.1. IDENTIFICATION OF EMPLOYEES:

A. The Contractor shall be responsible for obtaining identification for all employees.

Employees without a badge and REAL ID ACT compliant identification in their possession will be denied access to the base and work areas. AF Form 75 will be issued to Contractors working for thirty (30) days or less. Contractors working for thirty (30) days to one (1) year will be issued an Identicard 2000 Contractor ID Card upon presentation of a properly completed AF Form 355 from the Contracting Officer or designated representative. Contractors working for more than one (1) year will be issued a base decal upon presentation of a properly completed AF Form 355 from the Contracting Officer or designated representative.

PART 3 - EXECUTION

3.1. CONDUCT OF WORK:

A. Coordination and Access to Site: Coordination with using agencies shall be made through the Contracting Officer or designated representative to assist the Contractor in completing the work with a minimum of interference and inconvenience.

B. General Area Requirements: Security requirements and procedures shall be coordinated with the Contracting Officer. Activities of the Contractor and the Contractor's employees and subcontractors and their employees, while on the base, will be in accordance with base regulations, including those of the Fire Marshall, as well as security directives. This includes, but is not limited to, giving way to all emergency vehicles that are in a response mode. All personnel performing duties as an escort official(s) for contractors within a restricted area are briefed thoroughly on control methods and actions to take to enforce control.

Contractors will be briefed on security procedures and their responsibilities prior to the start of a contract. Contractors will be notified, as necessary, to changes in security procedures.

(1) Identification Credentials:

Starting October 10, 2015, visitors seeking access to military bases and almost all Federal facilities using their state-issued driver's licenses or identification cards must present proper identification issued by REAL ID compliant states or a state that has received an extension. When planning a visit to a Federal facility or military base, visitors should contact the facility to determine what identification will be accepted. State Enhanced Driver's Licenses (EDLs) designated as acceptable border-crossing documents by DHS under the Western Hemisphere Travel Initiative are acceptable for official federal purposes such as accessing a Federal facility or boarding a commercial aircraft. Individual agency policies may still apply.

If Montana does not receive another extension from Department of Homeland Security (DHS), starting January 1, 2020, a person with a Montana driver license or identification card who is visiting a military base or almost all federal facilities will be required to have an alternative form of acceptable identification to access the federal facility

(2) Commercial or company vehicles will be allowed access to the base provided company emblems are permanently attached to the sides of the vehicles. Magnetic signs are not acceptable. Personnel still require a badge or pass as identified in paragraph 1.

(3) Entry Authority List (EAL): EAL’s will be prepared by the unit or agency responsible for monitoring or administrating the contract. The EAL(s) must be signed by the preparers unit commander or agency chief, and forwarded to the Installation Commander or designated representative for final approval. Contractor vehicles must be listed on the EAL(s) including year, make, model, VIN and license number as stated on registration. The Contractor shall submit a list of personnel, including subcontractors, who will work within or adjacent to the restricted area(s). This list, which shall contain the name (Last, First and MI), address, Civilian Agency Affiliation, clearance information (if applicable), and social security number of each employee, no later than one (1) week prior to initial start of work. This list shall be submitted on stationary with the company's letterhead and be signed by a responsible member of that company. After the list has been submitted, it will be the Contractor’s responsibility to keep the list current. In addition to the 72-hour notification as noted below, not less than every two (2) weeks, the Contractor shall submit a complete up-to-date list of persons desiring access to restricted areas to include any additions or deletions made to

the list during the preceding two (2) weeks. The information listed above must be provided for new employees whose names were not listed on the initial list at least 72-hours prior to the time said new employees need access to the restricted area.

Only those employees with base entry badges whose names are on the list will be permitted entry into the restricted area.
