DoD PERIODIC HEALTH ASSESSMENT (PHA) Fact Sheet #3 (November 2017 Update)
ARMY DoD PHA IMPLEMENTATION The Army will field the new Tri-Service, Department of Defense (DoD) PHA tool on 31 December 2017. Healthcare Providers will be able to electronically complete the DoD PHA within the Medical Operational Data System (MODS) platform, through the new Army Healthcare Portal at www.mods.army.mil. The DoD PHA is comprised of a standardized question set, designed to identify outstanding DoD individual medical readiness (IMR) requirements and healthcare screening elements. H e a l t h c a r e Providers from any branch of Service can now complete the assessment for any Service member (SM) regardless of Service affiliation.
ARMY HEALTHCARE PORTAL TRAINING
Army Healthcare Portal Training is NOW available on the Joint Knowledge Online (JKO) as course code DHA US298. The course title is MEDICAL READINESS HEALTHCARE PORTAL
TRAINING.
The course URL is: https://jkodirect.jten.mil/Atlas2/page/login/Login.jsf
All healthcare personnel (nurses, medics, and providers) who are part of the DoD PHA completion process must be a registered user in MODS and complete the HCP training in order to gain role-based access. Once portal training is complete, Healthcare Providers who have existing roles (within eProfile) will maintain those roles automatically. Each one and two star Command with Healthcare Providers must have at least one eProfile Access Manager (EPAM) to grant access to the Healthcare Portal (this may be delegated down as appropriate).
Mental Health Assessment (MHA) Training: Healthcare Providers who have not previously taken the DoD Mental Health Assessment training will need to assure completion of their training and have it added to their Digital Training Management System (DTMS) record, as has been required for the past three years. The MHA training is accessible via https://mhaquiz.dhhq.health.mil/. The DoD Mental Health Assessment Training requirement does NOT apply to behavioral health personnel (psychologists or licensed social workers).
2017 PHA CLOSEOUTS
All current/open PHAs must be closed out by 31 December 2017. PHAs that start before 31 December 2017 must be closed out by a provider by 31 December 2017. If a PHA starts (Part 1) and is not signed by a provider, it will need to be redone within the new DoD PHA platform via the Healthcare Portal. This is ONLY for PHAs that are coming due in this time period (current PHAs that are closed and current in the system will NOT need to be redone until their normally scheduled/annual due date).

The NEW DOD PHA FORMAT
The SM Self-Assessment Section includes the new self-assessment questionnaire and incorporates the same DoD Mental Health Assessment (MHA) in its current form (the DoD MHA has not changed). Upon completion of the self-assessment, SMs will have the ability to save, print, or email health education information that is electronically generated based on individual responses. SMs will be able to access the new DoD PHA through the new Army Healthcare Portal or through their AKO account as they do now.
The Record Reviewer Section can be completed by a nurse or medic. The RR can review SM responses, check existing records for DoD IMR requirements, and provide comments for the Healthcare Provider who will sign the DoD PHA. If a RR is not available, the Healthcare Provider signing the PHA can complete the entire PHA, which is current practice.
**To decrease provider workloads and increase DoD PHA processing efficiencies, all Components, Regions, and MTFs are encouraged to establish workflows that utilize the Record Reviewer section/format of the new DoD PHA**
The Healthcare Provider (HCP) Section will allow review of the RR summary and any auto-generated flags based on SM responses. The Healthcare Provider has the ability to view SM responses as necessary and will:
1) Conduct the person-to-person MHA, unless previously completed and signed by a behavioral health provider. All Healthcare Providers (NOT including BH providers) must have the MHA training/certificate on file in order to sign off the MHA section.
2) Document recommended referrals. A hyperlink to the United States Preventive Task Force (USPTF) Guidelines individual assessment tool will be available on the provider page of the DoD PHA. (The URL for this tool is: https://epss.ahrq.gov/PDA/widget.jsp).
Additionally, providers can download a mobile app to a smartphone which will also provide access to the same USPTF guidelines tool. (The mobile tool URL is: https://epss.ahrq.gov/PDA/index.jsp). The AHA/ACC Risk Calculator for cardiovascular risk will also be available (and is found at http://www.cvriskcalculator.com/).
3) Review/update/create or rescind profiles to accurately reflect duty limitations.
4) Sign the DoD PHA, completing the assessment within MODS.
DIFFERENCES IN THE NEW DoD PHA The MHA – If a behavioral health provider is available, they have the option to complete and sign only the MHA portion of the DoD PHA with the SM. A BH provider is not required, this is only an option.
Lab/EKG Requirements – Mandated lab and EKG requirements that previously prompted by the Cardiovascular Screening Program (CVSP) are not part of the DoD PHA. DoD PHA Healthcare Providers will make screening recommendations for individual SMs based on appropriate USPTF guidelines (links will be available on the DoD PHA provider page to the USPTF Guidelines as well as the AHA/ACC Risk Calculator for cardiovascular risk).
Vision Requirements – Individual visual acuity data is not entered within the DoD PHA tool.
This capability will be accessible via link to the Medical Protection System (MEDPROS) Web Data Entry (MWDE) within MODS (eventually located within the Healthcare Portal).
System Downtime – Because the DoD PHA contains electronically generated information to the SM and there is no comprehensive method for paper completion — there will be no paper/manual DoD PHA substitutions. If an electronic platform is not accessible at the time of the DoD PHA, the DoD PHA completion must be rescheduled.
