STATEMENT OF WORK (SOW)
FOR
Install Machine Foundation for 26’ Single Carriage Lathe
AT
Watervliet Arsenal Building #35
Project #: 7OD008
Installation Management, Public Works Division Facilities Engineering (TAWV-IMP-F)
PART 1 GENERAL
1.1 SUMMARY OF WORK
1.1.1 Project Description
a. This Statement of Work (SOW) defines the effort required for the installation of a new machine foundation in Building 35. The machine will be located in Bay C, Sections 19-22. This project will require removal of an existing machine foundation, wood block flooring and a reinforced concrete floor slab to meet the manufacturer’s provided design drawings. The project will also repair the surrounding areas of concrete and wood block to provide a safe and finished working environment.
1.1.2 Location of Work
a. The work is located at Building 35, Bay C19-22 approximately as indicated. The exact location will be shown by the Contracting Officer.
1.1.3 Period of Performance
a. The contractor shall be required to (i) commence work under this contract within 7 calendar days after the date the contractor receives notice to proceed, (ii) prosecute the work diligently, and (iii) complete the entire work ready for use not later than [90] calendar days after notice to proceed was given. The time stated for completion shall include final cleanup of the premises.
1.1.4 Hours of Operation
a. Normal duty hours are 7:00 AM through 4:00 PM, Monday through Thursday, and 7:00 AM through 3:00 PM on select Fridays (the arsenal is closed every-other Friday). Federal holidays are non-work days. If a federal holiday falls on a Saturday or Sunday, the preceding Friday or following Monday is a work holiday.
1.2 SUBMITTALS
1.2.1 General Submittal Requirements
a. The contractor shall provide submittal documents via e-mail to the Contracting Officer for review and approval. Provide all physical (non-document) submittals (including but not limited to material swatches, samples, etc.) directly to the Contracting Officer for review and approval. All submittals require government approval unless otherwise explicitly stated.
b. The contractor shall reference APPENDIX A “Consolidated List of Submittals” for the complete list of required submittals.
1.2.2 Contractor Safety Plan
a. The contractor shall submit a safety plan which is specific to the work activity being performed under the scope of this project for Government review and approval.
1.2.3 Lockout / Tagout (LOTO) Acknowledgement
a. The Contractor shall sign and submit a “Lockout/Tagout (LOTO) Notification Acknowledgement Record” prior to working on or de-energizing any electrical equipment.
1.2.4 Asbestos Notification Acknowledgement Record
a. The contractor shall sign the “Asbestos Notification Acknowledgement Record” and submit this form to the Project COR prior to starting project work.
1.2.5 Project Schedule
a. The contractor shall create a project schedule and submit for Government review and approval.
b. The project schedule shall detail the phases of work and the overall timeline for the execution of the work. The contractor shall include delivery dates, appropriate milestones, and shall clearly detail any requirements for work performed outside of regular hours of operation.
1.3 GENERAL REQUIREMENTS
1.3.1 Watervliet Arsenal Information for Contractors (WAIC)
a. The contractor shall perform all work in strict conformance with Watervliet Arsenal Instructions for Contractors (WAIC). The contractor shall read and understand this document prior to submitting a proposal. The WAIC is publically available at:

1.3.2 Utility Outage Requests and Utility Connection Requests
a. Schedule work to minimize outages. For utility outages and connections required during the execution of work that affect existing systems, coordinate schedule with COR. Schedule utility outages and connections to minimize disruptions to the Government. No additional payment will be provided for utility outages and connections required to be performed outside the regular work hours.
b. The contractor shall notify the Contracting Specialist a minimum of two (2) business days before the contractor starts work. The contractor shall notify the COR a minimum of seven (7) business days before the shut-down of a utility or building service.
1.3.3 Safety Requirements
a. The contractor is required to provide the proper control of hazardous energy (lockout/tagout) per
OSHA 29 CFR 1910.147.
b. The contractor shall inspect the project work site prior to starting work, and shall request a copy of the asbestos building inspection records to verify the location and description of known asbestos-containing materials.
1.4 ON-SITE PERMITS

1.4.1 Excavation (Dig Permit)
a. The clearance of buried utilities will be performed by the Government in advance of the Contractor performing subsurface work, unless otherwise explicitly stated.
b. The Contractor shall obtain a copy of the completed Utility Clearance Permit (WVA Form 420-2-E) from the COR prior to performing subsurface work. Verify existing utility locations indicated on contract drawings, within area of work.
1.4.2 Hot Work and Confined Space
a. Obtain permit for welding, burning, use of flame, or entry into confined space from WVA Fire Department by calling (518)266-5990 each day the permit is to be used.

1.6 CONTRACTOR EQUIPMENT
1.6.1 Equipment Identification
a. The contractor shall create and affix a label to all large equipment, dumpsters, and other mobilized property, which is weather-resistant (laminated, or within a see-thru label hanger), securely affixed to the equipment or visible through a window, and which clearly displays the General Contractor’s:
i. Company Name, POC Name (first, last), POC phone number, Project Number.
1.6.2 Waste Dumpster
a. The contractor shall place any waste dumpsters in a manner that does not interfere with thru-traffic.
b. Place waste dumpsters with a minimum of 33’ clearance from any building. The final location shall be coordinated with, and approved by the COR before any dumpsters are mobilized.
c. Cover all waste dumpsters when not in use, and before the end of each work shift. The cover shall be weather-tight.
d. Note: The Contractor shall be required to retain and submit copies of all Bills of Lading and Weight Slips associated with the disposal of construction and demolition C&D) debris.
PART 2 PRODUCTS
2.1 GENERAL
All work shall be in strict accordance with product manufacturer’s instructions and specifications, including but not limited to manufacturer guidance for storage, staging, application, installation, and disposal of the product.
2.1.1 Hazardous Materials
a. The contractor shall submit Safety Data Sheets (SDS) for all hazardous materials to be used.
2.1.2 Asbestos, Mercury, Lead, PCB
a. Unless otherwise stated, all materials for this project shall be new. All products shall not contain any asbestos, mercury, chromium/lead paint, Class I Freon, and Polychlorinated Biphenyls (PCB), and shall state such on the manufacturer’s original packaging.
2.1.3 DoD Green Procurement Program
a. All construction materials shall meet the applicable requirements of the DoD Green Procurement Program, specifically recycled content products, environmentally preferable products and services, biobased products, energy-and water-efficient products, alternate fuel vehicles and alternative fuels, products using renewable energy, and alternatives to hazardous or toxic chemicals as found at <http://www.acq.osd.mil/dpap/cpic/cp/docs/USA001967-08-DPAP.pdf>.
2.2 PRODUCTS
2.2.1 Block Flooring.
a. Blocks shall be two (2) inches thick, four (4) inches wide, and six (6) inches long. Permissible variations shall not exceed 1/16 inch in depth and 1/8 inch in width and length.
b. Blocks shall be composed of recycled/ reclaimed materials.
c. Blocks shall be Fibre[B]lock II, or equivalent. The company’s website is http://kaswell.com. Any variation must be approved by the government and a sample must be submitted for approval.
2.2.2 Block Floor Adhesive.
a. Kaswell’s S-2705 White Adhesive or equivalent.
2.2.3 Block Floor Sealer.
a. Kaswell’s D1261 Black Latex Sealer or equivalent.
2.2.4 Concrete.
a. Compressive strength of 5,000 psi in 28 days
2.2.5 Epoxy Paint.
a. CORLAR 2.8 HG, High Gloss Epoxy or equivalent
b. Dark Green in color.
2.2.6 Compressed Air Piping.
a. 1” Schedule (SCH) 80, black iron pipe.
2.2.7 Electrical Conduit.
a. 1” and 3” complying with NFPA 70, UL 6, UL 1242, and UL 797.
2.2.8 Polystyrene.
a. FOAMULAR 150 or equivalent.
b. ASTM C578 Type X, 15 psi, 1.3 pcf density.
PART 3 EXECUTION
3.1 ITEMS OF WORK
3.1.1 Demolition.
a. Demolition work that is excessively loud, dusty, or disrupts manufacturing operations shall be performed on second shift which is starting no earlier than 1600 (4 PM) and ending no later than 2400 (midnight) Monday through Friday. All other work may be performed during normal business hours. No work shall be performed on Federal holidays.
b. The contractor shall refer to the attached Specification 03 WVA 00, Machine Foundation Requirements, for excavation and other demolition requirements. The Government will provide markings for any underground utilities. No cutting or digging shall occur prior to completion of the “Dig” Permit.
i. Existing underground utilities will be determined by available records. Neither the exact location nor the information of these existing utilities is guaranteed to be complete or correct. Therefore, the contractor shall use reasonable care to avoid damaging underground utilities. Contractor shall plan the excavation so as to avoid damage or to minimize interference with the underground utilities in or near the work area
ii. The contractor shall protect and preserve the staking, marking, or other designation of underground utilities until no longer required for proper and safe excavation.
iii. The contractor shall have an observer to assist the equipment operator when operating excavation equipment around known underground utilities.
iv. The contractor shall support and protect exposed underground utilities from damage.
Contractor shall notify the Project COR immediately after discovering or causing damage to underground utilities. All breaks, leaks, nicks, dents, gouges, grooves, or other damage shall be reported and repaired.
v. The contractor shall be responsible to reroute any known utilities that are discovered within the footprint of the new foundation.
vi. The contractor shall remove all excess clean soil from the Arsenal at the end of the project. Any environmentally contaminated/hazardous soil/materials will be placed on two (2) layers of six (6) mil polyethylene, covered with one (1) layer of six (6) polyethylene, batted down and labeled by the contractor. The Arsenal will test and dispose of environmentally contaminated/hazardous soil/materials.
c. The foundation and surrounding area shall be cleaned of any oils, coolants, or other waste material.
The contractor shall refer to the attached Environmental SOW for requirements.
d. There is an existing Wohlenberg machine foundation in this area that was recently filled in. The attached drawings are for information only and are not supplied as official “As Built Drawings.”
e. The contractor may also encounter shale within the footprint of the new foundation that will need to be removed. As part of this bid, the contractor shall include a price per cubic yard for shale/rock removal. In the event the shale/rock is encountered, it will be handled as a Change Order. This is for non-digable shale/rock ONLY.
f. The contractor shall remove wood block from over the top of the floor slab and existing foundation.
Contractor shall saw cut the concrete around the perimeter of the new machine foundation, cable track, chip conveyor and utility channels. Both operations shall be limited to the area required for construction of the machine foundation ONLY.
g. The contractor shall completely remove the existing foundation and any items within this area necessary to install the machine.
h. The contractor shall disconnect and remove any airlines, conduit, or other utilities within or around the footprint of the new foundation.
i. The contractor shall remove any surface mounted metal items from the area. Metal items include but are not limited to: angle iron, exposed reinforcing bars, machine support pads, covers, plates, brackets, sleeves, pipes, etc.
j. The intent of the demolition is to provide a suitable base for construction of the new foundation.
3.1.2 New Foundation.
a. The contractor shall use the attached machine tool “sketches” for bidding purposes ONLY.
Manufacturer signed drawings will be provided upon purchase of the machine and may alter the overall dimensions or requirements.
i. The foundation shall be constructed in accordance with all manufacturer’s requirements and specifications, as well as, the attached Specification (03 WVA 00).
ii. The foundation shall allow full and complete installation of this machine tool by the manufacturer. After installation on this new foundation described herein, the foundation shall allow the OEM to meet all OEM alignment and geometric accuracies for full and complete use of the machine tool by the Arsenal.
b. The contractor shall review all contract documents and provide written notice to the Contracting Officer of any discrepancies, problems, or issues that are found.
c. The contractor shall construct the foundation to include all coolant channels, utility trenches and any task on the drawings labeled “Customer Supply” or “Made by Customer” with the exception of the chip basket and grout for the anchor devices.
i. NOTE: All of the channels and utility trenches shall be seamless (i.e. if they are fabricated in multiple sections, they shall be welded together in the field during placement).
d. The foundation shall comply with the following technical requirements (subject to change based on manufacturer stamped drawings):
i. The thickness of the foundation shall be 1850 mm minimum.
ii. The contractor shall install ASTM A615 Grade 60, #6 rebar reinforcement horizontally and vertically throughout the foundation.
i. Reinforcement shall be constructed into a 12” (304 mm) x 12” (304 mm) grid.
ii. Reinforcement must be free from loose rust and scale, dirt, oil, or other deleterious coating that could reduce bond with the concrete.
iii. Reinforcement shall have a minimum concrete coverage of 3” (76 mm) on all sides.
iv. If bars are moved more than one bar diameter to avoid interference with other reinforcement, conduits or embedded items, the resulting arrangement of bars, including additional bars required to meet structural requirements, shall receive approval before concrete is placed.
v. Place safety caps on all exposed ends of vertical concrete reinforcement bars that pose a danger to life safety.
vi. Provide bar supports complying with the requirements of ACI SP-66.
iii. The contractor shall set the foundation on a concrete sub slab. Thickness of the sub-slab shall be 100 mm minimum.
iv. The contractor shall form and construct both the foundation and sub slab in a manner that allows for one continuous (monolithic) pour.
v. The contractor shall isolate the foundation with 50 mm of polystyrene along the entire perimeter for vibration control. During installation, the contractor shall place a metal cap guard over the polystyrene. Cap guard shall be 4” (101 mm) x 2” (50 mm) x 4” (101 mm) and bent out of 14 gauge sheet metal.
vi. The foundation shall be constructed with an approximate 1% slope in the bed supporting surfaces towards the chip conveyor channel or the coolant discharge channel.
vii. The top of the foundation shall be level and even with the final finished height of the wood block flooring; not the building floor slab.
viii. The bottom of the overall foundation shall be placed on a bed of gravel, with a minimum thickness of approximately 200-300 mm.
ix. The concrete shall be vibrated when poured. Final surface shall be smooth and free of any loose or excess debris.
e. The contractor shall use ready-mixed concrete conforming to the latest edition of ASTM Specification C94/C94M.
i. Minimum Compressive Strength: 5,000 psi after 28 days.
ii. Maximum Water-Cementitious Materials Ratio: 0.45
iii. Slump Limit: 4” +/- 0.5 inch
iv. Air Content: 2% +/- 1.5% at point of delivery for 3/4” nominal maximum aggregate size.
f. The contractor shall ensure the top surface of the channels (utility/cable track/conveyor) and associated covers (diamond/perforated/bar grating) are level and even with the surrounding wood block floor.
i. Interior edges of the channels shall have a metal lip as per the drawings.
ii. All steel plating shall be provided and installed, as specified, and painted “dark green” with at least one (1) coat of primer suitable for application on bare/new steel followed by two (2) coats of an oil resistant epoxy top coat.
g. The contractor shall core drill all forty-one (41) anchor and leveling bolt holes as specified on the manufacturer’s drawings.
h. After the concrete has cured the contractor shall paint the horizontal and vertical foundation surface to include the conveyor channel, cable track channel, coolant pump pit and all surfaces of the recessed utility trench with oil resistant epoxy paint.
i. Paint shall be applied per manufacturer’s specifications and directions.
ii. Paint shall seal the foundation and prevent oil from contaminating the soil underneath the foundation, trenches, and coolant well.
iii. The epoxy paint shall be “dark green” in color.
3.1.3 Install Compressed Air.
a. The contractor shall provide compressed air to the location identified by the machine manufacturer from an existing supply line at the nearest building column.
b. The contractor shall upsize the compressed airline from a point on the column to the machine foundation. New airline shall be one (1) inch, Schedule (SCH) 80 black iron pipe to match existing.
c. The contractor shall install a full port ball shut off valve within five (5) feet from the finished floor for isolation, as well as a minimum of a four (4) inch drip leg.
d. The contractor shall stub the compressed airline six (6) inches above the foundation.
e. The contractor shall be responsible for proper air shut off to include any lock out/tag out of equipment or notices given to project COR 48 hours in advance of main line shut down.
3.1.4 Install Electrical Service.
a. The contractor shall run a 250 amp drop with ground, 480 volts, 3 phase, 60 HZ from the buss duct to the machine electrical cabinet. The new machine is 175 FLA with a 200 AMP main breaker.
b. The contractor shall utilize the existing 400 Amp fused disconnect switch left over from the previous machine. The proposed location of the buss switch shall be approved by the Government.
c. The contractor shall dig a trench to provide for installation of a three (3) inch and one (1) inch rigid galvanized conduit. Conduits will be used to route the electrical and future communication wiring from source locations to termination point at the machine.
d. The new three (3) inch conduit shall run from the buss switch, down the nearest building column, and through the trench to the machine electrical cabinet. The one (1) inch conduit shall start at the base of the column and run through the same trench as the three (3) inch conduit. The bottom of the electrical cabinet is open and sits on top of a sixteen (16) inch wide trench. The trench will be lined with a three (3) millimeter thick steel plate, which will require cutting. The new service shall enter the cabinet in the middle, roughly 7’-10”.
e. Prior to being encased in concrete, the contractor shall provide and apply a compatible bituminous coating on the new piping and conduit for additional corrosion protection.
f. The contractor shall fill trench with concrete matching the properties of 3.1.2e above. Minimum encasement of the conduits shall be three (3) inches on top, bottom, and sides.
g. All materials shall be new and furnished by the contractor unless otherwise stated to include fuses.
h. The contractor shall provide all support material including but not limited to supports for conduit.
Supports shall not utilize any existing equipment (i.e. crane supports). Conduit shall be strapped and supported per the NEC code.
i. The contractor shall be responsible for lock out/tag out of equipment and providing notice forty-eight (48) hours in advance of any main electrical shut down.
3.1.5 Start-Up Testing.
a. As a condition of acceptance, the contractor is responsible for final termination at the machine electrical cabinet. The machine tool shall be successfully powered up and have the wiring verified by machine installation personnel for conformance with machine installation instructions.
3.1.6 Construction and Demolition Debris
a. The Contractor shall submit copies of all Bills of Lading and Weight Slips associated with the disposal of construction and demolition (C&D) debris and/or recycled materials, under this scope of work. Submit documentation per Appendix A, “Consolidated List of Submittals”.
3.1.7 Cleanup of Work Site
a. The contactor shall clean all work areas and remove all tools, equipment, debris, materials, and all waste from the premises.
b. The contractor shall remove all debris from any remote holding locations, to include any excavation and concrete equipment chute cleanout debris.
c. The contractor shall remove their equipment from Watervliet Arsenal premises within five (5) working days after completion of project work.
3.1.8 Restoration of Work Site
a. The contractor shall repair any items, building surfaces, equipment or miscellaneous items which were damaged during the execution of this SOW. Items that cannot be repaired to restore original function or aesthetics shall be replaced in kind by the Contractor at no additional charge to the Government.

APPENDIX B: Quality Assurance Surveillance Plan (QASP)
PURPOSE
This Quality Assurance Surveillance Plan (QASP) will be used as a guide to Government Quality Assurance Personnel (QAP). It provides a systematic method to evaluate the services the contractor is required to furnish.
SURVEILLANCE PROCEDURES
Timeliness: Progress is maintained to assure compliance with completion time.
Quality: Conformance with statement of work and industry standards.
Tasks: Performed according to statement of work requirements.
Problem Resolution: Contractor submits deliverables in accordance with the contract and resolves any deficiencies identified by the government in the final submission. Following final submission, contractor resolves any additional deficiencies identified by the government and resubmits and or corrects within one week.
Delivery: Contractor submits each deliverable in accordance with the statement of work and terms of the contract.
PERFORMANCE AREAS
Management Responsiveness: Evaluate the adequacy of the contractor’s responsiveness and their ability to keep the government informed of work progress and provides for early problem identification and effective corrective action plans.
Schedule Control: Evaluate the contractor’s adherence to completion schedule.
Performance: The contractor will be evaluated as to the workmanship and adherence to the statement of work.
EVALUATION CRITERIA
(Outstanding). Indicates performance clearly exceeds contractual requirements. The area of evaluation contains few minor problems for which corrective actions appear highly effective.
(Satisfactory). Indicates performance clearly meets contractual requirements. The area of evaluation contains some minor problems for which the corrective actions appear satisfactory.
(Unsatisfactory). Indicates the contractor failed to satisfy contractual requirements and recovery is not likely in a timely manner. The area of evaluation contains serious problems for which the corrective actions were ineffective.
DOCUMENTATION
Documentation used to perform surveillance will consist of random government inspections and inspection records, as well as the evaluation of final work product delivery by the project COR/COTR.

Contractor will be given an overall rating of Outstanding, Satisfactory, or Unsatisfactory by the Government Technical point of contact or Contracting Specialist Representative, if appointed, 3 using the above evaluation criteria.

Foundation Extra’s Below items to be completed by the foundation contractor during/after machine installation:
1. The contractor shall furnish and install sheet plates in accordance with manufacturer drawings. All cuts and openings (approximately 50) on sheet plates shall be performed during assembly of the machine.
2. The contractor shall fabricate and install chip chutes in the locations specified. Size of sheet metal is approximately 46’ x 2’ (required on both sides of the conveyor channel). Sheet metal shall be 14 gauge, cut into sections and bent accordingly for attachment to the enclosure and machine. Contractor shall make sure to provide cut outs for all posts and e-stop wire. Once installed, the contractor shall caulk the joints.
Note: Actual dimensions of the chip chutes will be determined in the field by the Project COR after the machine and associated components have been placed.
3. The contractor shall construct and install a pump bracket inside the cooling pit for mounting of the pump.
4. The contractor shall provide and install piping from the pump down to the pit. Piping shall stop 4” from the pit floor. Machine installation personnel or the Government will provide the screen connector for the bottom of the pipe.
5. The contractor shall fabricate and install a bracket inside the pit for attachment of the flow switches.
Bracket shall be assembled using 2 - 2” x 2” x 3/16” angles and 1/8” plate steel.
6. The contractor shall drill a hole in the top of the coolant tank for the return pipe fitting. Contractor shall cut an access door in the top of the tank next to the return fitting for ease of installation. The door shall be equipped with hinges and painted to match the tank.
7. The contractor shall supply and install piping from the return fitting to machine installation personnel’s return line. Piping shall come out of the return fitting with a nipple, then ninety (90°) to the side of the tank and then ninety (90°) down the side.
Note: All piping to be SCH 80 black iron.
8. The contractor shall install covers over the coolant return hose and exposed wiring to provide protection and prevent trip hazards. Contractor shall paint the covers safety yellow.
9. The contractor shall construct and install gusset supports along the cable track channel, opposite the coolant recovery trough, to support the checked (diamond) steel plate. Gussets to be 4” high x 6” wide and made out of 3/8” thick steel. Gussets shall be notched as required to contour to the channel wall.
Every other gusset shall have additional angle supports to capture the ends of two (2) plates. Supports to be welded every 500 mm. When finished, contractor shall prime and paint gussets to match foundation.
Note: All dimensions are subject to change based on manufacturer’s stamped drawings.
