The contractor shall purchase DBA insurance only from carriers that are authorized to provide such coverage by the United States Department of Labor (DoL). This requirement also applies to all subcontractors. The DoL maintains a website that contains a complete list of authorized insurance carriers:
http://www.dol.gov/owcp/dlhwc/lscarrier.htm. Carriers authorized to provide DBA insurance are identified with the letters “DB” in the column titled “Acts Covered.” The estimated amount proposed for DBA insurance will not be evaluated; however, will be included in the total contract amount. The contractor will provide proof of DBA insurance (insurance policy) to the contracting officer within 14 days after award, and in no circumstances will performance begin until the contracting officer has received proof of DBA insurance. If one insurance provider will not sell DBA to the contractor, the contractor is responsible to seek insurance coverage through other approved DoL providers. If the contractor is unable to provide proof of DBA insurance within 14 days, the contracting officer will terminate the contract for cause (default if non-commercial) and award to the next low offeror. Upon proof of a PAID receipt from an approved DoL insurance provider (contracting officer will verify the validity of the insurance before payment), the contractor shall be reimbursed for the actual amount PAID for the DBA insurance under the NTE CLIN in the pricing schedule of the contract.
