SUBJECT: Offeror Past Performance Assessment in Support of Request for Proposal (RFP) Number W564KV-17-R-0001

Dear Sir/Ma’am;

The Theater Contracting Center is currently conducting a competitive source selection to evaluate offerors on subject RFP. As part of this evaluation, we have requested that the offerors provide information about their past performance on same or similar federal, state or local government or commercial contracts as compared to North American Industry Classification System (NAICS) 541990. You have been identified as the point of contact cited on the enclosure.

Your assessment of their performance is extremely valuable to our evaluation. Please complete the enclosure and return directly to the Contracting Officer and Contract Specialist no later than 17 November 2016. Please email the completed questionnaire(s) to Mr. Aaron Schulz, aaron.m.schulz.civ@mail.mil and Ms. Jana Bunch, jana.j.bunch.civ@mail.mil. DO NOT return completed questionnaires to the offeror/contractor. Your completed questionnaire will become a part of the official Source Selection record and shall be handled accordingly.

PAST PERFORMANCE QUESTIONNAIRE

The Theater Contracting Center (TCC) is soliciting for a requirement to provide Mission Command Training Support services in Germany and Italy.

We would appreciate your assistance in providing the evaluation information below. This survey will be used to evaluate the past performance for the contractor and the contracting action identified below. The TCC may contact you to clarify or verify any information provided. The point of contact at the TCC is Jana Bunch, Contract Specialist, DSN (314) 483-5568, or commercial (0631) 411-5568.

The following questions pertain to the contractor’s record of past (within the past five years) and current performance. The information that you provide will be used in the awarding of a federal contract. Therefore, it is important that our information be as factual and accurate as possible. Please provide examples and/or explanations (use additional pages if necessary). The following adjectival ratings shall be used in your response.

Exceptional: Performance meets contractual requirements and exceeds many requirements that benefit the end user. Work was accomplished with few, if any, minor problems for which corrective actions taken by the contractor were highly effective.

Very Good: Performance meets contractual requirements and exceeds some requirements that benefit the end user. Work was accomplished with some minor problems for which corrective actions taken by the contractor were effective.

Satisfactory: Performance meets contractual requirements. Work was accomplished with some minor problems for which corrective actions taken by the contractor were satisfactory.

Marginal: Performance does not meet some contractual requirements. Serious problems with contractor performance were experienced for which the contractor has either not yet identified corrective actions or the corrective actions taken appear only marginally effective.

Unsatisfactory: Performance does not meet most contractual requirements. Serious problems with contractor performance were experienced for which the corrective actions were ineffective.

Your candid response to the questions is important to our evaluation effort and may affect the award outcome. Please provide specific examples in your responses and feel free to add pages if required. In accordance with OFPP Policy Letter Number 92-5, the names of individuals supplying past performance information will remain confidential.
